조기 위암의 내시경 치료

Abstract

Background: In Korea, the number of screening endoscopies to detect early stage gastric neoplasms has increased exponentially following the active implementation of the National Cancer Screening Program.
Current Concepts: Endoscopic treatment, including endoscopic mucosal resection and endoscopic submucosal dissection, is recognized as a minimally invasive treatment method with low morbidity and mortality for gastric dysplasia or early gastric cancer. Owing to improvement in the detection of early gastric cancer and advances in techniques, cases of endoscopic resection have increased and indications have been expanded. Endoscopic resection can preserve gastric function with excellent maintenance of the patient’s quality of life, and previous studies have shown better long-term follow-up outcomes compared to those with surgery. However, the fundamental limitation of endoscopic procedures is that gastric lymph-node dissection is not possible using endoscopic resection.
Discussion and Conclusion: Although the usefulness of endoscopic resection is proven for tumors with a very low risk of lymph-node metastasis, follow-up examination using endoscopy and computed tomography should be performed for at least 5 years after curative resection of early gastric cancer.

서론

1999년 시작한 국가암검진사업에 의한 진단 내시경 시행과 확대 내시경, 영상강화내시경 등 내시경 기술의 발전으로 조기 위암과 위선종의 조기 발견이 증가하게 되었다[1-3]. 이와 더불어 내시경 점막 절제술(endoscopic mucosal resection, EMR)과 내시경 점막하 박리술(endoscopic submcosal dissection, ESD) 등의 내시경 치료방법이 발전하면서, 적응증 내 조기 위암의 일차 치료는 외과 수술이 아닌 내시경 절제술로 확립되었다[4,5].
이러한 내시경 치료방법 중, 내시경 점막하 박리술은 대부분의 경우 병변의 크기 및 위치와 상관 없이 일괄 절제가 가능하고, 장기간 추적관찰 결과 임상적으로 좋은 예후를 보이고 있어 시술 건수가 매년 증가하고 있다. 하지만 조기 위암의 내시경 절제술은, 수술적 절제에 비하여 전신마취의 필요성이 적고, 빠른 회복과 짧은 입원기간 등의 장점이 있으나[6-9], 주변 림프절 절제가 불가능하기 때문에 림프절 전이 가능성이 없거나 매우 낮을 것으로 판단되는 적응증을 선별하여 치료하는 것이 매우 중요하다[10]. 또한 내시경 절제술의 경우 위 전체를 보존하기 때문에 수술과 비교하였을 때 잔존 위에서의 이시성 위암 재발률이 높아[6,7,11], 적응증에 해당하는 조기 위암에서 내시경 절제술로 완전 절제된 후에도 이시성 위암 재발을 감시하기 위한 지속적 내시경 및 컴퓨터단층촬영(computed tomography, CT) 등의 추적검사가 필요하다.
이 논문에서는 조기 위암에 대한 내시경 치료의 적응증과 시술방법, 임상 결과 및 추적검사 등에 대하여 기술하고자 한다.

적응증

1. 절대 적응증

내시경 절제술은 병변의 주변 림프절 절제를 하지 못하기 때문에, 조기 위암의 내시경 절제 시행 여부 판단에 있어서 가장 중요한 인자는 림프절 전이 위험도이다. 따라서 조기 위암에서 내시경 절제술이 가능할 수 있는 적응증의 근거는, 내시경 절제술 도입 이전의 수술적 위 절제술 결과를 후향적으로 분석 후, 림프절 전이가 거의 없는 조건을 선별하였고, 이후 연구들에서 림프절 전이의 위험도가 0-0.3% 정도로 매우 낮은 경우를 절대 적응증(absolute indication)으로 정의하였다[12-14]. 절대 적응증은, 분화형 선암(well or moderately differentiated tubular or papillary adenocarcinoma) 중 궤양이 없으면서 내시경 소견에서 점막하 침윤이 의심되지 않는 직경 2 cm 이하의 육안적 점막 침범으로 판단되는 조기 위암으로 규정하였다. 절대 적응증의 내시경 절제술의 결과 및 예후는, 여러 연구들에서 수술적 절제술과 비교하였을 때 비슷한 생존율 및 수술보다 양호한 삶의 질 등을 나타냈다[6,7,11,15]. 이러한 결과들을 바탕으로, 조기 위암의 절대 적응증에 해당하는 경우, 수술보다 내시경 절제술을 우선적인 치료법으로 권고하고 있다[5].

2. 확대 적응증

조기 위암에서 절대 적응증에 해당하는 병변에 대한 내시경 절제술이 시행되면서, 수술에 의한 위 절제술을 거부하거나 전신 질환 등으로 수술을 시행할 수 없는 환자 또는 내시경 절제술 후 조직검사 결과에서 절대 적응증을 벗어나 추가 수술을 시행하였지만 림프절 전이가 없는 사례 등이 보고되었다. 또한 기존에 이용되던 내시경 점막 절제술에서 시행이 어려웠던, 크기가 큰 종양의 일괄 절개가 가능한 내시경 점막하 박리술이 개발되었고, 다양한 내시경 도구 등이 나오면서, 조기 위암에서 내시경 절제술의 적응증 확대에 대한 연구들이 진행되었다. 기존의 수술 환자를 대상으로 시행한 림프절 전이에 관한 대규모 추가 연구들에서, 절대 적응증 범위를 벗어나지만 림프절 전이의 가능성이 매우 낮은 환자군을 분석하여, 내시경 치료를 적용할 수 있는 확대 적응증(expanded indication)이 계속 발표되고 있다.
현재 임상에 사용되는 내시경 소견에 따른 조기 위암의 확대 적응증은, 궤양이 없고 크기가 2 cm를 초과하는 분화형 점막암, 궤양이 있으면서 크기가 3 cm 이하인 분화형 점막암, 궤양이 없는 2 cm 이하의 미분화형 점막암(poorly differentiated tubular adenocarcinoma, poorly cohesive carcinoma, and/or signet ring cell carcinoma)으로 정의하고 있다. 내시경상 궤양이 없고 크기가 2 cm를 초과하는 분화형 점막암, 궤양이 있으면서 크기가 3 cm 이하인 분화형 점막암의 경우, 내시경 절제술 후 병리조직검사에서 맥관 침윤이 없고 절제면이 음성인 점막암으로 나타난 경우, 림프절 및 원격 전이 가능성은 약 0-0.21%로 보고되어, 위 절제술 후 사망률인 0.1-0.3%와 비교하여 크게 차이가 없고, 5년 생존율이 각각 92.0-97.2%와 93.3-96.4%로 역시 유의미한 차이가 없어 수술보다는 내시경 절제술을 우선적으로 시행할 수 있다[8,11,16-20].
미분화형 선암(undifferentiated type adenocarcinoma)의 경우 분화성 선암에 비해 림프절 전이 빈도가 높다는 이전 연구결과들에 의해, 절대 적응증에는 포함되지 않았었다[21,22]. 하지만 크기가 2 cm 이하이면서 점막에만 국한된 미분화형 선암에서 내시경 절제술을 시행하였을 때, 림프절 및 원격 전이의 빈도가 낮으며 5년 생존율이 95-98.6% 정도로 높은 결과를 보고하였고[17,23-27], 내시경 절제술을 시행한 군과 수술적 절제를 받은 군의 장기 성적을 비교한 연구들에서도 유의한 차이가 없는 것으로 보고하여, 크기가 2 cm 이하이면서 점막에만 국한된 미분화형 선암에서 내시경 절제술도 시행되고 있다[11,17,27-30].
2018년에 발표된 일본 가이드라인 개정 5판에서는 내시경 소견에 따른 적응증을 (1) 크기와 상관 없이 궤양을 동반하지 않은 분화형의 점막암, (2) 궤양이 있는 3 cm 이하의 분화형 점막암으로 확대했으며, (3) 궤양이 없는 2 cm 이하의 미분화형 점막암을 확대 적응증으로 발표하였고, 최근에는 2021년 보고된 일본 다기관 연구결과를 바탕으로, 궤양이 없는 2 cm 이하의 미분화형 점막암의 경우는 절대 적응증으로 분류하고 있다[31,32]. 하지만 미분화형 선암은 조직 소견에 대한 해석이 애매하거나 경계가 불확실한 형태가 많아 완전 절제율이 45.1-70%로 낮다는 국내 보고가 있고[24,25,33], 최근 국내 다기관 연구에서 상기 적응증인 궤양이 없는 2 cm 이하의 미분화형 점막암에서, 낮은 완전 절제 가능성, 림프절 전이 사례들이 보고되고 있어[34,35], 2020년 발표한 국내 조기위장관암 내시경 치료 임상진료지침에서는 궤양이 없는 2 cm 이하의 미분화형 점막암의 경우, 이전과 동일한 확대 적응증으로 권고하고 명확한 근거 마련을 위한 추가 연구의 필요성을 제시하였다[5].

완전 절제 기준

조기 위암을 내시경 절제술로 절제 후, 조직검사 결과를 통해서 완전 절제(curative resection) 여부를 판단하게 된다. 내시경 소견상 절대 적응증에 해당되는 병변 중, 시술에 의해 일괄 절제가 된 조직병리 결과에서, 수직과 수평 절제연 음성이면서 림프관 및 정맥 등 맥관 침윤이 없고, 궤양을 동반하지 않은 점막에 국한된 2 cm 미만의 분화형 선암을 절대 적응증 병변의 완전 절제로 간주한다. 조기 위암이 일괄 절제가 되었으며, 수직과 수평 절제연 음성이면서 림프관 및 정맥 등 맥관 침윤이 없는 경우 중, 분화형 선암이면서 궤양을 동반하지 않은 2 cm 이상의 점막암, 3 cm 이하의 궤양을 동반한 점막암, 3 cm 이하의 점막하 침윤암이면서 점막하층 침윤 깊이가 500 μm 이하인 경우, 그리고 2 cm 이하의 미분화형 선암이면서 점막에 국한된 경우를 확대 적응증 병변의 완전 절제로 정의한다. 상기 기준, 즉 완전 절제 기준에 해당하지 않는 병변의 경우 내시경 치료의 불완전 절제(non-curative resection)로 판단하여 수술 등의 추가적 치료를 권유하게 된다[35-37].

시술방법

내시경 절제술이 처음으로 도입된 1980년대에는 비교적 작은 병변에 대해서만 완전 절제가 가능하였다. 초기에는 2 cm 전후 크기의 병변에서 진단 및 치료 목적으로 조직을 얻거나 제거하기 위해서 시행되던 파지생검법(strip biopsy)을 내시경 치료방법으로 사용하였다. 크기가 작은 병변에서는 주로 올가미(snare)를 사용하여 병변을 제거하였고, 완전 절제율을 높이기 위해서 내시경 선단에 투명 캡이나 고무줄을 장착 후 흡인하여 절제를 하거나, 두 개의 겸자공을 이용하여 병변을 들어 올려 제거하는 방법 등이 이용되었다. 이러한 방법들은 크기가 작은 병변을 일괄 절제하기에는 무리가 없었으나, 2 cm 이상의 크기가 큰 병변의 경우에는 일괄 절제로 제거하기 어렵고, 이로 인한 재발 가능성이 높아, 내시경 점막 절제술 및 내시경 점막하 박리술 등이 개발되면서 시술 빈도가 점차 줄어들게 되었다. 하지만 파지생검법은 최근에도 작은 크기의 상피하 종양을 제거하거나, 위나 대장의 용종을 제거하기 위한 용종절제술(polypectomy)의 방식으로 이용되고 있다(Figure 1).
이후 내시경적 전기절개도(needle knife)가 개발되어, 병변 주변에 전 절개(pre-cutting) 시행이 가능하게 되었고, 전 절개 후 올가미 등으로 절제하는 내시경 점막 절제술이 1984년 소개되면서, 2 cm 전후 병변의 일괄 절제율이 높아지게 되었다(Figure 2) [38].
2000년대에 들어서 내시경 전기절개도의 개발이 진행되어 다양한 종류의 전기절개도가 소개되었고, 특히 선단에 절연체를 부착한 절개도(insulated tip knife)가 도입되면서 점막하층을 직접 보면서 박리하는 내시경 점막하 박리술 방법 이 가능하게 되었다. 내시경 점막하 박리술이 임상에서 이용되면서, 조기 위암 치료의 범위가 획기적으로 늘어날 수 있게 되었고, 이는 적응증의 확대로 이어졌다(Figure 3) [39].

임상 결과

조기 위암의 절대 적응증에서, 국내 연구들의 내시경 절제술 후 장기 생존율을 수술 후 장기 생존율과 비교한 결과, 내시경 절제술 후 5년 생존율은 93.6-96.4%로, 수술 후 5년 생존율인 94.2-97.2%와 차이가 없었고[6,7,11], 10년 생존율 또한 내시경 절제술 후 81.9%, 수술 후 84.9%로 의미 있는 차이를 보이지 않았다[6]. 조기 위암의 확대 적응증에 해당하는 경우, 내시경 소견 시 궤양이 없고 크기가 2 cm를 초과하는 분화형 조기 위암이나, 궤양이 있으면서 크기가 3 cm 이하인 분화형 조기 위암이면서 내시경 절제술 후 병리조직검사에서 맥관 침윤이 없고 절제면이 음성인 점막암인 경우, 내시경 절제술과 수술적 절제의 생존율을 비교한 연구에서 5년 생존율이 각각 93.3-96.4%와 92.0-97.2%로 의미 있는 차이가 없었다. 하지만 위를 보전하는 내시경 절제술의 특성상, 5년 누적 이시성 위암 재발률은 수술과 비교하였을 때보다 의미 있게 높게 보고되었다(0.9-1.1% 대 5.8-10.9%) [6,7,11].
확대 적응증 중, 크기가 2 cm 이하이면서 점막에만 국한된 미분화형 조기 위암의 경우, 내시경 절제술 후 5년 생존율은 95-98.6%로 나타났고, 내시경 절제술을 시행한 군과 수술적 절제를 받은 군의 장기 성적을 비교한 연구들에서도 유의한 차이가 없는 결과들이 보고되어, 상기 적응증의 경우 수술적 제거술 대신 내시경 절제술도 시도해볼 수 있다. 그러나 2020년 보고한 국내 다기관 연구결과, 확대 적응증에 해당되는 미분화 선암에서 초기 치료로 수술적 절제술을 시행한 383예 중, 6예인 1.56%에서 림프절 전이가 발견되는 등 작은 크기에서도, 드물지만 림프절 전이 및 원격 전이가 보고되고 있어[34], 크기가 2 cm 이하이면서 점막에만 국한된 미분화형 조기 위암의 경우에서는 신중한 환자군의 선택이 필요하며, 이에 대한 향후 추가적 연구가 요구된다.

치료 후 경과 관찰

위를 보전하는 내시경 절제술의 경우, 남아있는 위에서 위선종이 이시성 위암으로 발달할 가능성이 수술치료보다 높기 때문에, 정기적 내시경 및 CT 등의 검사가 필요하여, 국내외 가이드라인에서도 조기 위암의 절대 적응증 또는 확대 적응증으로 완전 절제된 환자에서, 최소 5년간 6-12개월 간격의 정기적인 검사를 권유하고 있다[37,40,41].
국소 재발은 내시경 절제술 후 같은 자리에서 암이 발생하는 경우로, 내시경 점막하 박리술 후 약 0.3%, 내시경 점막 절제술 후 약 5.2% 정도 발생하는 것으로 알려져 있고[42], 내시경 점막하 박리술에 의한 완전 절제 시에는 약 0.7%, 불완전 절제 시에는 약 2.4%의 빈도로 생김을 보고하였다[43]. 이시성 위암은 내시경 절제술 1년 이상 경과 후 다른 부위에서 발생하는 암으로, 빈도는 3.6-22.7%로, 국소 재발보다 높게 나타나고 있어 추적 내시경검사 시 주의를 요한다[18,44-46].
국내 보고에 의하면 내시경 절제술 후 이시성 위암의 연평균 발생률은 2.47%이며, 1년을 초과하여 추적검사를 시행한 경우 1년 이하 기간으로 추적검사를 시행하는 경우보다, 이시성 위암 발생 시 내시경 절제술 대신 수술이 필요한 경우가 높았다[47]. 따라서 조기 위암의 내시경 절제술 후 잔존 위에서 발생하는 국소 재발 및 이시성 위암의 경우, 조기 발견을 위한 노력이 필요하며 내시경 절제술 후 적어도 5년 동안 6-12개월 간격의 내시경 추적검사 및 드물지만 위강 외 림프절 재발을 발견할 수 있는 CT가 필요하다[18,48]. 이시성 위암의 경우, 치료 5년 이후에도 잔존 위에서의 발생은 지속적으로 증가하기 때문에 보이기 때문에, 5년 이후에도 내시경 추적검사를 시행하는 것을 권유한다[46,49].

결론

조기 위암에 대한 일차적인 치료인 내시경 절제술은, 적응증 내의 조기 위암 환자의 경우 수술적 절제술과 대등한 장기 추적 결과 및 더 나은 삶의 질 등의 결과를 보여주고 있다. 그러나 내시경 절제술 후에는, 잔존 위에서의 이시성 위암, 림프절 전이 혹은 원격전이 발생의 가능성을 항상 염두에 두면서 최소한 5년 이상 정기적인 내시경 및 CT 등 추적검사를 수행하는 것이 필요하다. 적절한 대상 환자의 선정, 병변과 환자 상태에 따른 시술방법 선택, 시술 후 검체에 대한 정확한 병리 평가, 이후 추적검사 등을 통해 조기 위암 환자의 치료 결과 및 삶의 질 향상을 이룰 수 있도록 노력해야 한다.

Peer Reviewers’ Commentary

이 논문은 조기 위암의 표준 치료법 중 하나인 내시경 절제술의 종류, 시술 방법, 임상 결과, 치료 후 추적검사에 관하여 최신 문헌을 토대로 잘 정리하여 설명해 주고 있다. 조기 위암의 유일한 표준 치료법이었던 외과 수술은 국내의 경우 높은 5년 생존율을 보이지만 수술 후 합병증과 수술로 인한 환자의 삶의 질이 떨어지는 문제가 존재한다. 이 논문에서는 우리나라에서 조기 위암의 일차 치료법으로 외과 수술과 함께 선택되고 있는 내시경 절제술의 치료 성적 및 임상적 결과를 외과 수술과 비교하여 일목요연하게 정리해 주고 있다. 또한 적절한 대상 환자의 선정, 병변에 따른 시술 방법 선택, 시술 후 검체에 대한 정확한 병리 평가, 이후 추적검사 등도 잘 정리하여 기술하고 있어 조기 위암 환자를 진료하는 의사에게 많은 도움이 될 것으로 판단된다.
