진행성 암환자에게 항암치료의 이득과 손해를 어떻게 저울질할 것인가?

Abstract

Background: The overall survival rate of advanced cancer patients has improved thanks to the development of modern medical treatments, particularly new and innovative chemotherapeutic agents such as targeted therapies and immune checkpoint inhibitors.
Current Concepts: Chemotherapy is administered in neoadjuvant, adjuvant, and palliative settings, and its ultimate goal is to improve overall survival. Chemotherapy has several proven and valuable clinical benefits, but also many side effects that cannot be ignored, especially in patients with poor European Cancer Oncology Group performance status. Therefore, we must carefully weigh and trade off the benefits and harms from many chemotherapy agents. In fact, it can be difficult to determine whether advanced cancer patients really benefit from chemotherapy, which is why a number of value measurement tools such as the American Society Clinical Oncology-Value Framework and the European Society for Medical Oncology-Magnitude of Clinical Benefit Scale have been developed.
Discussion and Conclusion: We need to include individual cancer patients in decision-making processes and use appropriate shared decision-making to decide whether or not to administer chemotherapy. Furthermore, we should perform rational trade-offs in consideration of limited health resources.

서론

암을 치료하기 위한 방법인 세 가지 축은 수술, 항암화학 치료(이하 항암제), 방사선 치료로 병의 진행정도와 환자의 상태에 따라 한가지 이상의 치료방법을 병행하거나 단독으로 시행한다. 아직까지 암은 국내 사망원인의 1위인 질병이지만[1], 최근 의료기술의 발달로 5년 생존율이 1995년도 42.9%에 불과했던 바에서 2020년도에 71.5%로 현저히 상승하였다[2]. 이런 괄목할 만한 성적은 1943년도 림프종에 항암치료가 성공한 이래[3], 2001년도 BCR-ABL tyrosine kinase inhibitor (imatinib)의 표적항암제(target chemotherapy) 유효성 증명 이후 개발된 수많은 표적항암제[4-8] 및 2018년에 노벨의학상을 받은 면역관문억제제(immune checkpoint inhibitor)로 이어지는 부단한 항암제의 개발과 성공적인 임상적용이 큰 기여를 한 덕이다[9,10]. 그렇지만 현재까지 아직 독성 없는 항암제는 존재하지 않으며, 진행성 암일수록 종양 이질성과 새로운 돌연변이로 인한 회피기전 발현 등으로 항암제로 완전한 조절을 기대하기 어려운 상황이다.

항암제의 이득

항암제의 이득은 완치, 생명기간의 연장, 삶의 질 향상으로 축약된다. 항암제는 종양이 제한적이고 국소 병기일 때 수술과 (혹은) 방사선 치료의 시행 전이나 시행 후로 각각 선행항암화학요법(neoadjuvant chemotherapy)이나 보조 항암화학요법(adjuvant chemotherapy)으로 투여하며, 진행성 병기, 타 장기에 전이가 동반된 경우는 완화적 항암화학요법(palliative chemotherapy)으로 투여한다. 선행항암화학요법은 수술 불가능인 진행성 고형암(유방암, 두경부암 등) 상황에서 수술 가능한 상황으로 유도하며 생존기간의 연장을 기대할 수 있는 이득을 제공하고 보조항암화학요법은 일부 고형암(유방암, 대장암 등)에서 수술 후 재발의 위험을 낮추고 생존기간의 연장 이득을 보여 준다[11].
실제 종양이 제한적이고 국한병기일 때는 수술과 (혹은) 방사선 치료로 완치가 되지만 40-50%의 암환자들은 완치적 치료 목적에 부합되지 않아 암으로 사망하며 이럴 때 항암제가 치료의 주된 전략이 된다[12]. 일부 진행성 고형암, 예를 들어 전이성 생식세포종(metastatic germline tumors)은 다양한 항암병합요법으로 완치가 될 수 있고[13], 완치가 불가능하다고 여겨졌던 진행성 흑색종과 진행성 비소세포폐암 환자 일부가 면역관문 항암제로 완치를 기대할 수 있게 되었다[14,15].
실제 많은 다양한 고형암이 있으므로 각 고형암의 항암제 감수성과 병의 진행병기에 따라 항암제의 치료 목적과 이득의 정도가 달라진다. 궁극적으로 항암치료의 유효성 평가에 가장 우선을 두는 것은 전체생존기간(overall survival)의 연장이다. 현재까지도 수많은 항암제가 개발되고 있고 기존의 세포독성 항암제와 표적치료제, 면역항암제를 2제 이상 조합하여 더 유효한 항암상승효과를 기대할 만한 임상시험 결과는 계속하여 발표 중이다[16-20].

항암제의 손해

초기 1세대 항암제는 세포 분열이 빠른 세포를 공략하는 기전인 점을 고려할 때 골수기능저하, 탈모, 불임 등의 많은 부작용이 있고[21], 암세포 특이 항원이나 분자, 종양유전자(oncogene)를 공격하는 표적항암제 또한 발진, 설사, 간염과 같은 세포독성 항암제에서 잘 나타나지 않았던 합병증이 나타난다[22,23]. 면역관문 항암제 역시 면역의 활성화로 정상세포에도 면역반응이 나타나는 자가면역질환의 부작용이 관찰되어[23,24] 어떤 항암제 투여 시에도 독성은 다 동반될 수 있다. 항암제 독성은 대표적으로 European Organization for Research and Treatment of Cancercommon toxicity criteria (EROTC-CTC)로 평가하며 각 지표마다 경미한 독성(1-2등급)부터 증등도, 생명위협적 독성(3-5등급)까지 등급이 정의되어 있다. 다양한 지표가 있지만 보통 중등도 독성부터 생명위협적 독성 발현시 항암제로 인한 이득보다는 손해가 많다고 판단할 수 있다. 이런 치명적이고 생명 위협적인 항암제 독성을 감내하고도 항암치료를 결정할 때의 절대적 기준은 치료 목표가 완치 가능하거나 항암치료 독성으로 인해 삶의 질이 떨어짐에도 불구하고 생명연장의 이득이 우월하다고 판단될 때이다.
한편 환자와 보호자들 입장에서 경험하는 항암제의 독성 중 최근에 논의가 활발히 되고 있는 쟁점은 재정적 독성(financial toxicity)이다. 항암제의 신약개발은 많은 고가 항암제 출시로 이어져 국내뿐만 아니라 미국임상암학회(American Society of Clinical Oncology, ASCO)에서도 고가 항암제로 인한 의학적 가난(medical poor)이 생기며 재정적 독성을 상쇄할 만큼 환자가 얻는 이득이 반드시 비례하지 않음을 보고하여 건강보험 측면에서도 필히 고려되어져야 할 독성으로 제시하였다[25,26].
이외 최근 시간 독성(time toxicity)도 논의되고 있는데 이는 항암치료 중 병원에 내원하는 시간, 약물 투여 시간, 검사 받거나 대기하는 시간 등으로 실제 진행성 암환자가 항암치료로 얻게 되는 1-2개월의 생존기간 연장이 위의 시간 소모로 맞교환이 된다면 환자가 그 생존기간의 연장을 의미 있게 여길 수 있을 것인지에 대한 것이다[27], 특히 진행성 암환자의 경우 생존기간의 연장뿐만 아니라 생존기간 동안의 삶의 질, 치료 장소 선호도도 항암치료 결정에 필히 고려되어야 할 사항이므로 시간 독성 문제는 향후 더 활발히 대두될 것이다.

항암제 투여 손익 저울질

항암제의 이득과 손해는 진행성 암환자 개인별로 판단해야 하는 복잡하고 난해한 결정이다. 예를 들어 진행성 유방암 환자에게 항암치료 시행 시 일반적으로 생존기간의 연장을 기대할 수 있지만 노인들의 경우 anthracycline 기반의 항암치료는 심폐 부전의 위험을 증가시키므로 생존기간의 연장 대비 항암치료 독성을 환자의 연령, 기저질환, 선호도를 고려하여 현명한 결정을 해야 한다. 이런 고민은 ASCO와 유럽내과종양학회(European Society for Medical Oncology, ESMO)에서 발표한 가치척도, 각각 ASCOValue Framework [28], ESMO-Magnitude of Clinical Benefit Scale을 탄생시켰다[29]. 이 가치척도는 항암신약의 임상시험 시 보고된 유효성(생존기간 연장, 부작용, 증상완화 등)을 점수화하여 항암치료 선택이나 항암제 보험급여 시 유용하게 참조를 하고자 함이었고, 국내 종양내과의사들도 항암제 투여 시 이런 항암제 가치척도를 고려한 결정을 해야함을 인지하고 있었다[30].
그러나, 진행성 암환자에게 근거화된 가치 평가 척도를 고려하고 항암치료의 이득과 손해를 저울질하여 시작한 항암치료라고 할지라도, 항암치료로 인한 손해의 무게가 더 커지고 있다고 판단할 시점에 행해야 하는 항암치료 중단 결정은 쉽지 않다. 수많은 신약 항암제는 의사나 환자, 보호자 입장에서 치료 선택에 대한 더 많은 고민을 하게 하며, 진행성 암환자에게 항암치료는 일차적인 질병의 치료 전략이기 때문에 환자나 보호자 입장에서는 1차 항암치료 이후 질병의 진행이 되면 2차, 3차 항암치료 순으로 계속 이어가면 지속적인 생존기간의 연장이 있을 것으로 기대하기 때문이다. 그러나 이런 기대는 현실적이지 않으며 ASCO에서도 임상적인 이득이 없었던 항암치료가 굉장히 흔하고 불필요하게 투여된다고 지적하였다[31]. 일반적으로 전신수행상태가 양호할수록 항암치료에 따른 독성을 견딜 수 있을 것으로 여기며 ASCO에서 Eastern Cooperative Oncology Group (ECOG) 전신수행상태가 3 이상일 때(제한적으로 자가 돌봄은 가능하며 깨어 있는 시간의 50% 이상을 누워 있거나 휠체어를 이용함) [32], 항암치료의 반응은 거의 없으며 독성은 많고 짧은 생존기간을 관찰하였다[33,34]. 특히 진행성 암환자에서 이루어지는 항암치료 임상시험은 전신수행상태가 양호한 ECOG 2 미만(ECOG 0: 모든 활동 가능, 어떤 제한 없이 병에 걸리기 전과 동일한 일 수행; ECOG 1: 육체적인 힘든 일은 제한이 있지만, 거동이나 가벼운 일은 가능) 환자들을 대상으로 한 유효성 성적이므로 현실적인 임상성적으로 여기긴 무리이다. 실제 면역관문 항암제를 진행성 비소세포폐암 환자에게 1차 항암치료로 투여하였을 시의 성적을 참조하면 ECOG 2-4 환자와 0-1 환자에서 무병생존기간(progression free survival)은 2개월 남짓 차이가 있었으며(median progression free survival: 4.1 vs. 2.0 months, respectively, P<0.001). 전체생존기간도 ECOG 0, 1, 2, 3-4로 분류하였을 때 각각 20.4, 15.5, 5.0, and 1.9개월로 차이가 있어 항암치료의 손익 결정에 불량한 전신수행상태일수록 생존기간의 연장은 크지 않으며, 동반된 부작용이 해가 될 수 있음을 보여주었다[35].
그럼에도 불구하고 전신수행상태가 불량한 진행성 암환자의 항암치료는 흔히 시행되고 있으며 국내 연구에 따르면 임종 한 달 전 간암 환자 67%, 유방암 환자 56%가 항암치료를 받고 있고, 임종 전 대부분의 의료비 중 항암제 지출이 상당 부분 차지하였다[36]. 또한 말기 진행암(end stage cancer, 원격 전이 동반된 1차 항암치료에 불응성인 의사가 예측하는 여명이 6개월 미만인) 고형암 환자들을 대상으로 항암치료 효과를 조사한 연구는 ECOG 전신수행상태가 2 이하 환자에서 항암치료는 삶의 질을 향상시키지 않았고 ECOG 전신수행상태 1이었던 환자에게는 오히려 생존기간의 연장은 미약하면서 삶의 질을 해쳤다는 보고도 있다[37].
그럼 의사들은 진행성 암환자에게 항암치료의 손익에 대한 저울질을 현명하게 하여 항암치료를 시작과 중단을 잘 판단할 수 있을 것인가? 이것에 대한 근거는 임상 데이터 이외 환자의 개인 가치와 다양한 항암치료 선택의 접근성에 따라 개개인 환자마다 결정이 다를 수도 있다. 최근 연명의료결정 서식 및 이행서 작성을 한 환자 및 보호자를 대상으로 심층 인터뷰를 진행한 바에 따르면 보호자들은 환자가 진단을 받는 과정에서부터 인터넷 등을 통해 기본 의학지식을 접하고 환자를 오랜 기간 동안 곁에서 지켜보면서 환자의 상태를 파악하게 되지만, 직접 의료진을 통해서 치료방법의 유효성과 남아있는 생존기간에 대해 전문적 판단을 자문 받고 싶고 특정 치료(항암치료) 방법을 받게 되었을 때 기대되는 점과 우려되는 사항에 대해 설명 받고 싶어 하였다[38].

결론

괄목할 만한 항암치료의 발전과 임상현장에서의 적용으로 진행성 암환자의 생존율과 생존기간은 분명히 연장되었다. 그럼에도 불구하고 의료진은 항암치료의 독성과 한계를 인지하여 막연한 생존기간 연장의 기대를 가지고 항암치료를 지속하여서는 안되며, 항암치료의 시작과 중단 모두 진행성 암환자 및 보호자와 충분한 의사소통으로 공유하여 결정해야 할 것이다. 또한 적절한 건강보험재정안에서 현명한 항암치료 선택을 할 수 있는 제도 정립과 한국실정에 맞는 항암제 가치척도 개발도 고려해야 할 것이다. 때론 완치하며 자주 완화시키며, 항상 편안하게 “To cure sometimes, to relieve often, and to comfort always”의 원칙을 지킬 수 있게 신약 항암제들 출시와 다양해지는 항암치료의 선택에서 현명한 결정을 할 수 있는 안내가 절실히 필요할 때이다.
