Abstract

Lung cancer is the leading cause of cancer-related deaths worldwide despite major advances in platinum-based chemotherapy and targeted therapy based on activating driving mutations. Immune checkpoint inhibitors (ICIs) have revolutionized the treatment paradigms in lung cancers. When used as a second-line or later treatment for non-small cell lung cancer (NSCLC), ICIs improve overall survival and exhibit better safety profiles than the standard chemotherapeutic agent, docetaxel. In front-line treatment, ICI monotherapy is significantly associated with improved clinical outcomes and fewer adverse events than platinum-based chemotherapy in patients with advanced NSCLC, who express programmed death-ligand 1 in at least 50% of all tumor cells. Moreover, ICIs combined with platinumbased chemotherapy have become the standard first-line treatment for patients with metastatic NSCLC without sensitizing mutations in the epidermal growth factor receptor gene or translocation of the anaplastic lymphoma kinase gene, regardless of programmed death-ligand 1 expression. Additionally, maintenance treatment using ICIs has also been demonstrated to improve clinical outcomes in patients with stage III unresectable NSCLC following chemoradiotherapy. Recently, the addition of ICIs to chemotherapy as the first-line treatment for extensive-stage small-cell lung cancer resulted in significantly longer overall survival and progression-free survival compared with chemotherapy alone. Although immune checkpoint inhibitors significantly improved overall survival and showed a durable response in lung cancer compared with platinum-based chemotherapy, we should foster further prospective studies to identify predictive biomarkers to determine those individuals who may benefit more from ICIs. It is also essential to overcome the development of drug resistance in patients treated with ICIs.

서론

폐암은 진단 당시 절제 불가능한 국소 진행성 혹은 전이성 질환으로 발견되는 비율이 높고 예후가 가장 불량한 암 중에 하나이다[1-3]. 하지만 폐암의 분자유전학적 이해가 심화되면서 지난 수십 년간 항암치료에서 일련의 진전이 이어져 왔다[4]. 첫 번째 치료 패러다임의 변화는 백금화합물 근간의 항암화학요법이 진료현장에 적용되면서 유의한 생존향상을 가져왔다[5-7]. 그럼에도 불구하고 환자의 개인적 특성, 조직학적, 분자유전학적 차이에 대한 고려 없이 획일적인 항암 치료에 대한 미충족 수요가 증가되었다[1,5-7].

2000년도에 들어서면서 표피생장인자수용체(epidermal growth factor receptor)를 표적으로 하는 타이로신인산화효소억제제(tyrosine kinase inhibitors)의 개발을 시작으로 폐암 환자에서 표현되는 여러 다양한 유전자 돌연변이들에 대한 연구들이 이루어졌고 표적치료의 적용을 통해서 두 번째 변화를 이끌었다[8-10]. 표적치료는 개인별 발암 유전의 특성에 따른 맞춤치료라는 새로운 개념을 정립하였다[8-10]. 하지만 발암유전자 중독을 통해서 폐암의 발생 및 진행되는 환자의 비율은 일부분이며 절반 이상의 폐암 환자들은 표적치료제의 적응증에 해당되지 못하는 한계가 존재한다[4,8-10].

최근 면역세포들과 암세포들 간의 상호작용에 대한 면역학적 이해를 통해서 대표적인 면역회피 유전자인 programmed death-ligand 1 (PD-L1)과 cytotoxic T lymphocyte antigen 4 (CTLA-4)를 표적으로 개발된 면역관문억제제(immune checkpoint inhibitor)가 임상현장에 소개되었다[4,11-14]. 면역관문억제제는 폐암에서 새로운 치료방향을 제시하였고 다양한 임상연구를 통해서 그 역할을 더욱 공고히 하였다[4,11-20].

본문에서는 폐암 환자의 임상적인 특성과 병기, 조직학적 분류에 따라서 이루어진 면역관문억제제를 이용한 항암치료 임상연구들 중에서 2021년 현재 우리나라 진료현장에서 표준요법으로 사용되는 약제들을 중심으로 논의하고자 한다.

전이성 비소세포폐암

1. 백금화합물 근간의 복합항암화학요법에 불응 및 진행한 환자군에서 2차 이상으로 사용된 면역관문억제제

백금화합물 근간의 복합항암화학요법 이후 진행하거나 불응한 비편평상피 비소세포폐암 환자들을 대상으로 이루어진 CheckMate 057 연구 및 편평상피 비소세포폐암 환자군을 대상으로 이루어진 CheckMate 017 임상연구에서 표준 항암제 docetaxel을 투여 받은 그룹과 비교할 때 nivolumab을 투여 받은 환자군에서 통계적으로 유의하게 우월한 반응률, 무진행 생존율, 전체 생존율, 안전성 프로파일(safety profiles)을 관찰할 수 있었다[14-16].

한편 PD-L1 ≥1%으로 발현되며 이전에 치료받은 비소세포폐암 환자들을 대상으로 docetaxel과 pembrolizumab의 효능 및 안전성을 비교한 2/3상 임상시험인 KEYNOTE-010 연구에서는 pembrolizumab 투여군에서 투여용량(2 mg/kg 대 10 mg/kg)에 상관없이 docetaxel 투여군과 비교하여 통계적으로 우월한 무진행 생존율, 전체 생존율 및 안전성 프로파일을 확인하였다[12,17,18]. 또한 PD-L1 ≥50% 고발현군에서, 저발현군을 포함한 전체 환자군과 비교할 때 통계적으로 우월한 임상결과를 제시하였고, 이를 바탕으로 PD-L1의 발현율이 반응률과 전체 생존기간과 유의미한 상관관계를 가질 수 있다는 증거를 제시하였다[12,17,18].

이후 PD-L1을 표적으로 개발된 atezolizumab을 이용한 3상 임상시험인 OAK 연구가 소개되었다[19]. 이전에 이루어진 3상 임상연구에서는 병리 검사상 PD-L1의 발현을 측정할 때 종양세포의 발현율로만 평가되었으나, OAK 연구의 특징은 Ventana사(Tucson, AZ, USA)에서 개발한 SP142 항체를 이용하여 종양세포뿐만 아니라 면역세포에서의 PD-L1의 발현율을 함께 측정하여 고발현군(종양세포3: PD-L1 ≥50% 혹은 면역세포3: ≥10%)을 새롭게 정의하였다[19-21].

OAK 3상 임상연구는 이전 항암치료를 받은 후 진행 및 불응한 비소세포폐암 환자군에서 docetaxel 투여군과 비교할 때 atezolizumab 투여군에서 PD-L1의 발현 유무와 상관없이 통계적으로 유의하게 우월한 임상결과 및 안정성을 보여주었다[19-21].

결론적으로 pembrolizumab, nivolumab, ateozolizumb 등 PD-L1/programmed death 1 (PD-1)을 표적으로 개발된 면역관문억제제는 백금화합물 근간의 복합항암화학요법 이후 진행 혹은 불응한 비소세포폐암에서 일관되게 docetaxel 표준요법과 비교하여 통계적으로 우월한 임상결과를 보고하였다(Table 1) [11-21]. 이러한 임상연구 근거로 현재 치료가이드라인에서 표준요법으로 자리를 잡게 되었다.

2. 일차요법으로 면역관문억제제 단독요법

전이성 폐암 환자에서 일차요법으로 면역관문억제제 단독요법이 백금화합물 근간의 복합항암화학요법과 비교하여 우월성을 보여준 3상 임상연구는 Table 2와 같다[22-27]. KEYNOTE-024 연구는 PD-L1 50% 이상의 고발현을 보이면서 표적 치료의 대상이 되는 표피생장인자수용체 돌연변이 및 역형성림프종인산화효소 유전자재배열(anaplastic lymphoma kinase rearrangement)이 관찰되지 않는 환자들을 대상으로 일차요법으로 pembrolizumab 단독요법과 연구자 판단에 따라서 결정된 백금화합물 근간의 복합항암화학요법군으로 무작위배정이 되었다[22]. 표준치료군과 비교하여 pembrolizumab 단독투여군에서 통계적으로 유의하게 향상된 무진행 생존기간 및 전체 생존기간을 보 여주었다[22]. 따라서 KEYNOTE-024 연구를 근간으로 일차 항암제로 표적치료 적응증에 해당되지 않으면서 PD-L1 (pharmDx 22C3; Dako North America, Carpinteria, CA, USA) 발현이 50% 이상의 고발현을 보이는 환자군의 표준치료로 자리매김하게 되었다[22].

한편 KEYNOTE-042 연구는 조직학적 아형에 상관없이 전이성 비소세포폐암 환자들 중 PD-L1 (pharmDx 22C3) 발현이 1% 이상인 환자군을 대상으로 일차 치료제로 pembrolizumab 단독요법과 백금화합물 근간의 복합항암화학요법를 비교하였다[23,24]. 중앙 전체 생존기간은 16.7개월과 12.1개월로 pembrolizumab 단독요법군이 백금화학물 근간의 복합항암화학요법군과 비교하여 통계적 우월성을 증명하였다[23,24]. 또한 PD-L1 저발현군과 비교할 때 고발현군에서 임상적 이득이 극대화되는 것을 확인되었다[23,24]. 하지만 무진행 생존기간의 측면에서 관찰했을 때 PD-L1 20% 이상군과, 1% 이상군에서는 양군 간에 통계적으로 유의한 차이를 관찰할 수 없었다[23,24].

IMpower 110 연구에서도 PD-L1 고발현군에서 atezolizumab 투여군이 표준 항암제 투여군과 비교할 때 통계적으로 우월한 전체 생존율 및 안전성 프로파일을 보고하였다[25,26]. 반면에 CheckMate 026 연구에서는 PD-L1 5%를 기준으로 등록 환자들의 층화 배정이 이루어지면서 PD-L1 저발현 환자들이 다수 포함되었고 다른 연구와 달리 일차요법으로 면역관문억제제 단독요법의 통계적 우월성 증명을 실패하였다[27]. 하지만 본 연구는 종양 돌연변이 부담(tumor mutational burden)에 따른 하위 분석 결과에서 높은 종양 돌연변이 부담을 가지는 환자군에서 통계적으로 유의하게 표준 항암제 투여군과 비교해서 nivolumab 투여군이 우월한 임상결과를 보고하였다[27].

이러한 연구결과들을 바탕으로 표적치료 적응증에 해당되지 않고 PD-L1 (pharmDx 22C3) 50% 이상의 고발현을 보이는 절제 불가능한 비소세포폐암 환자들의 일차 표준치료방법으로 면역관문억제제 단독요법을 사용할 수 있는 근거를 마련하게 되었다(Table 2).

3. 일차요법으로 면역관문억제제와 백금화학물 근간의 세포독성 항암제 복합요법 및 PD-1/PD-L1과 CTLA-4 억제제의 복합요법

KEYNOTE-189 및 KEYNOTE-407 연구는 표적치료 적응증에 해당되는 유전자 변이가 없는 전이성 비편평비소세포폐암 환자군과 편평 비소세포폐암 환자군을 각각 임상연구 대상으로 등록하였다[28,29]. 두 연구 모두에서 pembrolizumab을 포함한 백금화합물 근간 항암화학요법을 함께 시행한 경우 표준항암제 치료군과 비교하여 PD-L1 발현율에 상관없이 통계적으로 유의하게 향상된 임상결과를 보고하였다[28,29]. 그럼에도 불구하고 PD-L1 고발현군이 저발현군 및 무발현군과 비교할 때 가장 큰 임상적인 이익을 얻었다는 사실은 일관되게 유지되었다[28,29]. 또한 안전성 측면에서 pembrolizumab을 추가한 치료군이 표준항암치료군과 비교 시 이상반응의 유의한 증가는 관찰되지 않았고, 면역 매개 이상반응도 pembrolizumab 단독요법과 비슷한 비율을 보고하였다[28,29].

한편 IMpower 150 연구는 이전에 항암치료 과거력이 없는 전이성 비편평 비소세포폐암 환자들을 대상으로 atezolizumab과 bevacizumab, paclitaxel 및 carboplatin을 이용한 4제 약제요법과 표준요법인 bevacizumab, paclitaxel, carboplatin을 포함하는 3제요법 및 atezolizumab, paclitaxel, carboplatin으로 조합한 또 다른 3제요법으로 각기 배정되어 임상적 유용성 및 안전성을 비교한 3상 임상연구이다[30,31]. 전체 환자군을 대상으로 이루어진 분석에서 PD-L1 발현 및 유전자 변이(표피생장인자수용체 돌연변이, 역형성림프종인산화효소 유전자재배열) 유무와 상관없이 4제요법군이 3제요법군과 비교하여 통계적으로 유의하게 향상된 무진행 생존 및 전체 생존기간을 보고하였다[30,31]. 하지만 하위그룹 분석결과를 보면 일관되게 PD-L1 고발현군이 저발현군보다 통계적으로 의미 있게 우월한 임상적인 이익을 얻게 됨을 재확인하였다[30,31]. 이러한 연구결과들을 바탕으로 PD-L1 발현율에 상관없이 전이성 비소세포폐암 환자에서 면역관문억제제와 백금화합물 근간 세포독성 항암제 복합요법이 일차 표준치료전략으로 권고될 수 있다(Table 3) [28-33].

반면에 일차요법으로 PD-1/PD-L1과 CTLA-4 억제제를 함께 사용한 CheckMate 227 연구에서는 PD-L1이 1% 이상 발현되는 환자와 PD-L1이 발현되지 않는 환자로 구분되어 등록되었다[34]. 이 연구는 PD-L1 발현과 상관없이 백금화합물 근간 표준 치료군과 비교 시 nivolumab과 ipilimumab을 함께 사용한 환자군에서 통계적으로 우월한 전체 생존기간을 보고하였으나, 2제 면역관문억제제 투여군에서 심각한 이상반응 발생률 및 치료 중단 비율이 높았다[34].

2제 면역관문억제제를 이용한 또 다른 3상 임상시험으로서 MYSTIC 연구는 durvalumab과 tremelimumab 복합요법이 백금화합물 근간 항암화학요법과 비교하여 우월성을 증명하지 못했다(Table 4) [35,36]. 이러한 연구결과를 바탕으로 PD-L1 발현과 상관없이 특별히 종양 돌연변이 부담이 높은 전이성 비소세포 폐암 환자들을 대상으로 nivolumab과 ipilimumab을 이용한 2제 면역관문억제제 복합요법을 고려할 수 있는 근거가 제공되었다[34-36].

가장 최근 발표된 CheckMate 9LA 연구에 따르면 nivolumab과 ipilimumab을 이용한 2제 면역관문억제제 복합요법과 2주기의 백금화합물 근간의 복합항암화학요법을 추가했을 때 표준치료군과 비교하여 유의하게 향상된 전체 생존기간을 보고하여 새로운 치료방법으로 제시되었다[37].

국소 진행성 III기 비소세포폐암

PACIFIC 연구는 절제 불가능한 III기 비소세포폐암 환자들을 대상으로 동시 방사선 항암화학요법을 시행 후 질병의 진행이 없는 경우 durvalumab과 위약군으로 무작위배정하여 비교한 3상 임상연구이다[38-40]. 중앙 무진행 생존기간 및 중앙 전체 생존기간 등에서 통계적으로 유의하게 면역관문억제제를 사용한 환자군에서 우월한 임상결과를 보고하였다[38-40]. 이를 바탕으로 현재 절제 불가능한 국소 진행성 비소세포폐암에서 동시 방사선 항암화학요법 이후 유지요법의 durvalumab은 표준치료로 자리매김하게 되었다[38-40].

소세포폐암

소세포폐암은 비소세포폐암과 비교하여 지난 30년 이상 치료의 발전이 없었고 일차요법으로 백금화합물 근간의 치료를 시행할지라도 예후가 매우 불량한 암종이다. 이러한 가운데 면역관문억제제의 적용을 통해서 치료의 불모지였던 소세포폐암에서 새로운 치료의 진전을 이루었다[41-44].

IMpower 133 및 CASPIAN 연구는 확장성 병기의 소세포폐암으로 진단받은 환자들을 대상으로 표준 항암화학요법인 etoposide와 백금화학물을 근간으로 면역관문억제제 및 위약군으로 무작위배정을 하여 임상적 효과 및 안전성을 비교한 3상 임상연구이다[41,42]. 두 연구에서 일관되게 면역관문억제제 추가군에서 통계적으로 우월한 전체 생존기간 및 무진행 생존기간을 보고하였다[41,42]. 안전성 측면에서도 예상 가능하고 조절 가능한 비율의 독성이 확인되었고 이전 보고와 다른 새로운 독성 증가는 관찰되지 않았다[41,42]. 이를 통해서 확장성 병기의 소세포폐암에서 백금근간 항암화학요법에 atezolizumab 혹은 durvalumab 등 면역관문억제제를 추가하는 치료전략이 표준요법으로 자리잡게 되었다(Table 5) [41-44].

결론

면역관문억제제는 전이성 비소세포폐암을 중심으로 3기 국소 진행성 비소세포폐암 및 확장성 병기의 소세포폐암으로 그 적응증이 점차 확대되어 임상현장에서 표준치료로 사용되고 있으며 향후 그 적응증은 더욱 확대될 것이라 예상된다. 하지만 PD-L1 유전자 이외에 더욱 신뢰할 만한 바이오마커에 대한 추가 연구들이 이루어져야 한다. 더불어 면역관문억제제에 대한 내성기전 및 극복방안에 대한 연구도 요구된다.
