소아기 양성돌발현기증

Abstract

Background: Benign paroxysmal vertigo (BPV), one of the most common causes of pediatric vertigo, is characterized by recurrent sudden short-lived attacks of vertigo, which resolve spontaneously without residual disability in otherwise healthy children. In this report, we present a literature review on BPV in childhood.
Current Concepts: A child with BPV appears frightened and experiences a spinning sensation with occasional nystagmus and vomiting. Episodes usually last less than a minute and are not associated with hearing loss and altered consciousness. Most patients present with a relapsing-remitting clinical course for weeks to years, after which they usually resolve spontaneously. Based on current evidence, BPV of childhood is considered a variant of migraine, and vasospasm observed in patients with migraine plays an important role in the pathogenesis of this disorder. It is important to differentiate BPV from other diseases that present with vertigo, including basilar migraine, closed head injury, psychiatric dizziness, and acute labyrinthitis.
Discussion and Conclusion: BPV is a common but important disorder observed in children and should be considered in the differential diagnosis in children with the aforementioned clinical presentation. It is important to reassure patients and parents regarding the benign course of BPV.

서론

소아기 양성돌발현기증(benign paroxysmal vertigo of childhood, BPVC)은 건강한 소아에서 일시적인 어지럼 발작이 반복적으로 나타나는 질환으로 특별한 유발인자 없이 갑작스럽게 발생하고, 청력이나 뇌기능 이상 없이 수년 뒤에 저절로 완전히 호전되는 것이 특징이다[1,2]. 어지럼 발작은 수초에서 수분간 지속되는데 보통 4세 이전에 발생하지만, 드물게 8세 이후에도 발생할 수 있다[3]. 국내에서는 편두통성 어지럼(32.7%) 다음으로, BPVC (20.4%)가 두 번째로 흔하다[4]. 성인에서 머리 움직임에 따라 안진 및 현기증이 발생하는 양성돌발체위현기증(benign paroxysmal positional vertigo, BPPV)과 유사하다고 알려져 있다[5].
1964년 Basser [6]는 소아에서의 BPPV를 처음 기술하였는데, 청력 소실이나 이명 없이 갑자기 순간적으로 회전하는 듯한 느낌이 발생하면서, 균형을 잃고 비틀거리며, 공포, 창백, 발한, 구토 등의 증상을 보고하였다. 이러한 일화는 수개월부터 수년 동안 여러 차례 발생하였고, 대개 8세가 되어서 호전되었다[6]. 이후 비슷한 증상을 가진 소아들이 많이 보고되면서, 소아기 양성돌발현기증이라고 명명하였다[7,8]. 그러나 BPVC 환자의 상당수에서 청소년기 이후에 편두통이 발생한다고 보고되어, 전정 편두통과 하나의 스펙트럼일 가능성도 제시되고 있다[9-11].
이 논문에서는 BPVC의 증상과 진단, 그리고 치료를 중심으로 최신지견을 소개하고자 한다.

임상양상

BPVC는 대개 건강한 소아에서 증상이 몰려서 나타난다. 이러한 군집(clusters)은 매일 나타나기도 하고 수주에 걸쳐 서서히 줄어들다가 또다시 재발하는 양상을 보인다[12]. 어지럼 발작 후에는 후유증 없이 호전되는 것이 특징이다[12,13]. 어지럼 발작은 아이가 갑자기 겁먹고 무서워하면서 빙빙 도는 느낌을 호소하는데 균형을 잡으려고 손을 뻗치기도 한다. 간혹 안진과 함께 창백함, 식은땀, 구역, 구토와 같은 자율신경계 증상을 동반할 수 있지만, 머리의 움직임이나 자세 변화와는 관계가 없다. 쓰러지지 않으려고 주위 사람이나 지지대를 붙잡기도 한다. 이러한 삽화는 보통 수분 이내이고 의식의 변화를 동반하지는 않는다. 소아는 어지럼 호소보다도 자율신경계 증상 및 공포로 나타날 수 있다는 점을 유의해야 한다[14]. 부모는 아이가 평소와 달리 행동이 둔하면서 어색하거나 균형을 잘 못 잡는지를 살펴봐야 한다. 그네, 시소타기 등과 같이 미로(labyrinth)를 자극하는 경우나 발열, 피곤, 스트레스 등에 의해 유발될 수 있다. 일반적으로 수면과 함께 소실되며, 앉거나 누우면서 호전되기도 한다[15].
성인에서 주로 나타나는 BPPV와 증상이 비슷하여 잘못 진단하거나 간과하는 경우가 드물지 않다[10,16]. 하지만 몇 가지 분명한 차이점이 있다(Table 1). 둘 다 어지럼 발작이 짧은 시간 동안 지속되고 증상과 징후가 피로현상을 보이며, 악화와 호전을 반복한다는 점에서 동일한 모습을 보이지만, BPVC에서는 청력손상이나 체위 불균형 및 체위 안진은 없다[17]. 특히 BPPV는 머리 움직임에 의해 증상이 유발 또는 호전을 보이지만 BPVC는 이와 연관이 없다[18].

발생기전

발생기전에 대해서는 아직 명확히 밝혀지지 않았다. 어지럼 증상을 호소했던 소아들이 일정기간이 지나고 편두통의 빈도가 통계학적으로 높게 나타나는 것을 볼 때, 편두통과 병태생리가 유사할 것으로 추측하고 있다[2,8,19]. 또한 어지럼 증상은 편두통의 전구증상으로 흔히 동반되기 때문에 BPVC가 편두통의 전구 질환일 것이라는 주장도 있다[8]. 실제 8세 이전에 증상이 호전된 BPVC 소아들을 15년 가량 장기 추적관찰한 연구에서 21%에서 편두통이 발생하였다[20]. 하지만 현기증이나 균형장애는 보이지 않았다.
편두통의 발생기전은 신경염증(neurogenic inflammation)이 중요한 역할을 하는데, 삼차신경의 활성화, 신경전달물질 농도의 변화, 그리고 주변 혈관의 확장 및 염증반응이 관여하는 것으로 알려져 있다[21]. BPVC는 전정계 허혈과 연관이 있는데, 편두통 발생기전처럼 미로동맥의 혈관이 연축(spasm)되어 관류(perfusion)를 방해하면 내이(inner ear)가 허혈 되어 어지럼 발작이 발생한다고 여겨지고 있다[22-26]. 이러한 소견은 경두개도플러검사(transcranial doppler)에서 뇌 혈류속도의 변화 및 뇌간청각유발전위검사 (brain stem auditory evoked potential)에서 높은 자극률을 보이는 것으로 뒷받침되고 있다[24,27]. Zhang 등[28]의 연구에서는 BPVC 소아에서 약 절반이 이러한 검사에서 이상 소견이 있다고 보고하였다.
두 질병 간의 밀접한 관련성을 편두통-소아기 양성돌발현기증 복합체(migraine-BPVC complex)라는 개념으로 제시하면서 소아 어지럼의 가장 흔한 원인이라고 보고하기도 하였다[29]. 하지만 BPVC는 편두통과 달리 대부분 일정시기가 지나면 저절로 호전되기 때문에, 여전히 두 질환과의 상관관계는 아직 확립되어 있지 않다[1,30,31].

진단기준

BPVC는 10세 이전에 일시적인 어지럼 발작이 반복되지만 특별한 후유증 없이 자연적으로 완화될 때 진단할 수 있다[32]. 물론 소아가 자신의 증상을 정확히 표현하기 어려워 자세하고 명확한 병력청취가 쉽지 않기 때문에 진단에 어려움이 있다. 일반적으로 BPVC가 편두통 변형의 한 형태로 생각되고 있기 때문에, 국제두통질환분류 제3판(International Classification of Headache Disorders, 3rd edition)에서 제시한 진단기준을 사용하고 있다(Table 2) [33]. 이에 따르면 BPVC는 건강한 소아에서 갑작스럽게 발생하는 짧고 반복적인 어지럼 발작이 의식 소실 없이 수분에서 수시간이 지나서 호전되는 특징을 갖는다. 그러면서 5회 이상의 삽화가 있고, 안진, 실조, 구토, 창백, 공포감 중 한 가지 이상을 보이며, 증상 사이에 신경학적 검진, 청력 및 전정기능검사에서 정상 소견을 보이는 경우 진단할 수 있다.
영상검사를 포함한 대부분의 검사에서 정상 소견을 보이지만, 온수안진검사(caloric test)에서 말초 전정기능이상으로 인해 일측 또는 양측의 반고리관 마비(canal paresis) 소견을 보일 수 있다 [11,34]. 이러한 소견은 어지럼 발작이 끝나면 사라진다[35]. 그리고 일부에서 전정유발근전위검사에서 이상소견이 보일 수 있어 진단에 도움을 받을 수 있다[28,36].
어지럼은 시각, 말초 전정기관부터 대뇌, 소뇌, 뇌줄기 등의 중추신경계에 이르기까지 어느 한 부분이 문제가 생겨도 증상이 발생할 수 있으므로 감별해야 할 진단이 많다[37]. 특히 현기증의 빈도가 높은 전정 편두통(basilar migraine)과의 감별이 필요한데, 전정 편두통은 뇌줄기에서 기인하는 신경학적 증상이 동반된다[15]. 8세 이전에 대부분 증상 소실을 보이는 BPVC와는 달리 보통 10대에 증상이 나타나고 현기증 지속시간도 5분에서 72시간으로 좀 더 길다[38]. 또한 회전성 어지럼보다는 자발성 및 체위성 현기증이 많다. 그 외에도 불안장애, 우울장애, 해리장애, 신체화장애와 관련되어 나타나는 정신적 가짜현기증(psychiatric pseudovertigo)과 홍역, 수두, 볼거리, 풍진 등의 감염 이후 발생하는 전정신경염과의 감별도 필요하다(Table 3).

치료

예후는 양호하여 어지럼 증상은 수개월에서 수년 후에 신경학적 합병증 없이 사라지는 양성 경과(benign nature)를 보인다. 대부분 청소년기가 되기 전에 좋아지지만, 현기증이 없어지는 시기에 두통이 나타날 수 있다. 치료는 증상 예방을 위해 환경이나 음식 등의 가능한 유발요인을 제거한다. 급성기 어지럼 발작 시에는 어지럼과 동반된 심한 자율신경 증상을 조절하기 위해 항히스타민제로서 항어지럼 및 항구토제인 benzodiazepine, dimenhydrinate, promethazine, meclizine을 사용해 볼 수 있다[39,40]. 발작이 잦은 경우에는 편두통 예방약제인 flunarizine, topiramate, cyproheptadine 등을 고려할 수 있다[15].

결론

양성돌발현기증은 소아기 어지럼증의 주요 원인으로 정확한 발생기전은 알 수 없지만 반복적인 어지럼 발작이 삽화적으로 발생하고, 시간이 지나면서 저절로 호전되는 특징을 가졌다. 신경학적 이상소견 없이 회복되지만 일부에서 청소년기 편두통으로 이환될 수 있다. 이 질환을 정확히 이해하여 빠르게 발견하는 것이 중요하고, 이를 통해 부모에게 이 질환의 양성 경과를 상담해 줌으로서 그들의 불안을 덜어줄 수 있을 것이다.

소아 어지럼증은 최근 우리나라에서도 관심이 높아지고 있다. 이 논문에서는 소아청소년 어지럼증의 주요 원인 중에서 양성돌발현기증의 진단과 치료에 대하여 정리하여 설명해 주고 있다. 소아기 양성돌발현기증은 뇌병변으로 오인될 수 있는 매우 염려스러운 증상을 보이기도 한다. 이 논문에서는 소아기 양성돌발현기증과 감별진단해야 하는 다양한 어지럼증 유발 원인을 일목요연하게 정리하고 있다. 이 논문은 소아기 양성돌발현기증의 임상 양상, 발생 기전, 진단 기준, 치료에 대해 체계적으로 기술하고 있어, 임상현장에서 소아청소년의 어지럼증을 감별진단하는 데 많은 도움이 될 것으로 판단된다.
