급성 호흡부전의 치료: 비침습적 기계환기

Abstract

Background: Noninvasive ventilation (NIV) has been an important strategy to support patients with respiratory failure, while preventing complications assorted with invasive mechanical ventilation. Physicians need to be aware of the various roles of NIV and the challenges encountered in clinical practice.
Current Concepts: Traditionally, the application of NIV has been well-known to be associated with reduced mortality in patients with chronic obstructive pulmonary disease (COPD) or acute pulmonary edema and those suffering from acute respiratory failure. However, despite some positive results of NIV treatment in patients with de novo hypoxemic respiratory failure such as acute pneumonia or acute respiratory distress syndrome, NIV failure (or delayed intubation) can have deleterious effects on patients outcomes. Besides, the aggravation of lung injury should also be taken into consideration when applied to patients exhibiting high respiratory drive. Nonetheless, NIV has potential for wide applications in various clinical situations such as facilitation of ventilator weaning, post-operative respiratory failure, or palliative treatment.
Discussion and Conclusion: In addition to the strong evidence in patients with acute respiratory failure due to COPD or acute pulmonary edema, the NIV treatment can be potentially used for various clinical conditions. However, compared to European countries, the prevalence of NIV use continues to remain lower in South Korea. Nevertheless, when applied in appropriately selected patients in a timely manner, NIV treatment can be associated with improved patient outcomes.

서론

우리나라에서는 급성호흡부전 환자에서 비침습적 기계환기(noninvasive ventilation [NIV] or noninvasive positive pressure ventilation)의 사용 빈도는 서구에 비해 낮은 편이다[1-5]. NIV는 전통적인 기계환기(mechanical ventilation)와는 달리 기도삽관을 하지 않기 때문에 호흡기계의 방어기전을 보존할 수 있고, 폐렴이나 기흉, 기도외상, 진정제 사용 등 기계환기 치료와 관련된 여러 합병증을 줄일 수 있다. 그 동안 NIV가 고이산화탄소혈증을 동반한 급성호흡부전에서 매우 중요한 치료법이라는 점은 잘 알려져 있지만(Table 1), 저산소성 급성호흡부전에서는 도움이 안 될 수 있기 때문에 환자를 잘 선택해야 한다.
급성호흡곤란 환자에게 적용되는 NIV 치료는 주로 응급실이나 중환자실에서 이루어지는 경우가 많다. NIV 성공을 위해서는 환자가 마스크와 인공호흡기에 적용할 수 있도록 치료 초기에 의료진이 충분한 시간 동안 환자를 관찰하는 것이 매우 중요하다. 또한 급성호흡부전과 만성호흡부전에서는 치료방법과 사용 장비가 다를 수 있기 때문에 의료진들은 이에 대한 차이점을 인식하고 있어야 한다[6].
NIV의 사용에 대한 European Respiratory Society/American Thoracic Society와 British Thoracic Society/Intensive Care Society 가이드라인이 최근에 발표되었다[7,8]. 임상에서의 적응증과 치료 측면에서의 권고사항이 잘 설명되어 있어 의료진들이 환자를 선택하고 적절한 시점에 적절한 치료를 제공하는 데 도움을 받을 수 있을 것으로 생각한다. 이 논문에서는 기존에 알려져 있는 NIV에 대한 기본적인 지식(적응증)과 임상 현장에서 고려해야 할 몇 가지 중요 사항에 대해 기술하고자 한다.

고이화탄소혈증 급성호흡부전(Hypercanpic Acute Respiratory Failure)

1. 만성폐쇄성폐질환의 급성악화

NIV 치료는 만성폐쇄성폐질환(chronic obstructive pulmonary disease, COPD) 급성악화의 치료에서는 표준 치료법이다. 치료효과를 얻기 위해서는 호흡성 산증이 뚜렷해야 하는데 PaCO2>45 mmHg이면서 pH≤7.35인 경우 적응증이 된다. pH 기준에 하한선은 없지만 pH가 낮을수록, 즉 산증이 심할수록, 치료에 실패할 가능성이 크다. 따라서 이러한 환자는 집중 관찰이 필요하고 치료 반응이 없다고 판단되는 경우 빠른 시기에 기도삽관으로의 전환을 고려해야 한다[7,9,10]. 하지만 산증이 없는 COPD 환자에서 예방을 목적으로 NIV 치료를 시행하는 것은 권장되지 않는다[7]. 이 경우에는 사망률이나 기도삽관의 비율을 낮추지 않는다고 되어 있다.

2. 기타 만성호흡부전 질환의 급성악화

천식 환자에서는 NIV 치료가 1초간강제호기량(forced expiratory volume in 1 second)과 최대호기유량의 호전을 보여주었지만 사망률 감소나 중환자실(intensive care unit) 치료기간 등에서는 의미 있는 결과를 보여주지 못했다[11]. 따라서 천식의 급성악화에서는 NIV가 권장되지는 않는다[7].
만성호흡부전을 보이는 질환으로는 amyotrophic lateral sclerosis, Duchenne muscular dystrophy, myasthenia gravis와 같은 신경근육질환, 척추측후만증, 비만저환기증후군(obesity hypoventilation syndrome) 등이 있다. 이러한 환자들도 급성악화 시에는 고이산화탄소혈증이 악화되는 경우가 많고 충분히 NIV 치료법을 적용시킬 수 있다[12,13]. 하지만 급성악화의 원인과 환자 상태를 잘 판단하고 적용해야 한다. 객담 배출이 안 되는 경우가 많기 때문에 NIV가 아닌 기도삽관을 통한 침습적 기계환기가 필요한지를 잘 평가해야 한다. 특히 신경근육질환 중에서 기앵-바레증후군(Guillain-Barre syndrome)의 호흡부전에서는 NIV가 권장되지 않는다는 점에 유의해야 한다[14].

저산소혈증 급성호흡부전(Hypoxemic Respiratory Failure)

저산소혈증 급성호흡부전에서 NIV 치료효과에 대해서는 오랜 기간 논쟁이 있어 왔고, 아직 그 효과는 불분명하다. 이 환자들은 시간이나 중증도 측면에서 NIV를 적용할 수 있는 범위(widow)가 작고, 고이산화탄소혈증 환자와는 달리 NIV 실패가 높은 사망률로 직결될 수 있다는 점을 인지해야 한다. 프랑스에서 15년 동안 이루어진 추적관찰 결과 COPD와 심인성폐부종에서의 NIV 치료는 그 빈도가 비슷하게 유지되었지만 저산소혈증 급성호흡부전에서는 사용 빈도가 감소했다는 보고가 있었다[3]. Antonelli 등[15]은 높은 중증도, 고령, 급성호흡곤란증후군(acute respiratory distress syndrome, ARDS), 폐렴이 NIV 실패의 위험요소이고, 적용 1시간 후 임상적 호전이 없으면 실패할 가능성이 크다는 결과를 보고하였다. 따라서 집중 관찰을 통해 기도삽관이 필요한 시점을 놓치지 않는 것이 중요하다.

1. 급성심인성폐부종(Acute cardiogenic pulmonary edema)

급성심인성폐부종 환자에서 NIV 치료를 시행하는 경우 정맥환류량(venous return)을 감소시키고, 심장의 후부하를 감소시켜 산소화 등의 생리적 지표가 빠르게 호전되고 환자의 호흡일(work of breathing)이 감소된다[16]. 지금까지의 연구를 종합해 보면, 급성심인성폐부종에서 NIV 요법은 기도삽관의 빈도와 병원사망률을 모두 감소시키는 것으로 나타났다[17-19]. 여기에는 지속기도양압(continuous positive airway pressure) 혹은 bilevel-NIV을 사용할 수 있는데 두 가지 치료법에는 차이가 없다고 할 수 있다. 단, 기존의 연구에는 급성관상동맥증후군(acute coronary syndrome)이나 심장성쇼크(cardiogenic shock) 환자는 포함되지 않았기 때문에 이러한 환자군에서의 NIV 시행은 주의가 필요하다.

2. 급성호흡곤란증후군

ARDS 환자에서의 NIV 요법에 대한 연구는 많지 않다. 소규모 비교연구와 관찰연구를 이용한 메타분석에서는 heterogeneity로 인해 적절한 결론을 내리기 어렵지만, 기도삽관이나 사망률의 감소를 가져오지는 않았다[20,21]. 물론 초기 ARDS (mild ARDS) 환자에서 NIV 치료를 이용함으로써 기도삽관 비율을 감소시켰다는 보고가 있지만, 유의한 결과를 보인 연구 대부분이 다른 동반 질환이 없는 매우 선택된 환자들만 포함시켰다는 점이 있다[22-24].
고이산화탄소혈증과는 달리, 폐렴이나 ARDS와 같은 de novo acute respiratory failure에서는 NIV에 의한 호흡일(work of breathing)의 감소가 뚜렷하지 않다. 호흡일을 감소시키기 위해서는 높은 흡기 압력이 필요한 경우가 많은데 이런 경우는 공기누출, 복부 팽만, 치료 순응도 감소로 이어질 수 있다[25]. ARDS 환자에서 NIV 치료의 어려운 점은 일회호흡량(tidal volume)을 조절하기 어렵다는 것이다. 높은 일회호흡량은 transpulmonary pressure를 높이게 되어 폐손상을 악화시킬 수 있고, 환자의 강한 자발 호흡도 폐손상을 더욱 조장할 가능성이 있음을 인지해야 한다(self-inflicted lung injury) [26.27]. 한 연구에서는 일회호흡량 >9 mL/kg가 치료 실패의 위험인자임을 제시하였다[27].
NIV 요법은 ARDS 환자의 치료법으로 권장하지는 않지만 경증의 환자(mild ARDS)에서 기도삽관의 예방 목적으로는 많이 시도되어 왔다. 하지만 치료 1-2시간 후 PaO2/FiO2와 PaCO2의 호전이 없는 경우는 치료 실패 가능성이 크다는 점을 인지해야 한다. 최근 소규모 무작위 비교연구에서 helmet을 이용한 경우 facial mask에 비해 기도삽관 비율을 낮추고, 90일 사망률을 낮추었다는 보고가 있었다[28]. Helmet을 이용하는 경우 공기누출을 줄여서 높은 호기말양압을 유지할 수 있으며, 높은 유량(100-200 L/min)을 전달할 수 있어 CO2 rebreathing의 위험성을 낮출 수 있다는 장점이 있다. 고유량 코삽입관(high-flow nasal cannula) 산소요법 또한 최근의 대규모 무작위 연구에서 표준 산소치료나 NIV 요법에 비해 ARDS 환자의 생존율을 증가시켰다[29]. 온종일 NIV 치료를 시행하는 것은 환자에게 힘들 수 있다. 따라서 고유량 코삽입관 산소요법과 번갈아 가면서 사용하는 방법도 환자의 순응도를 높이는 방법이 될 수 있다.

3. 면역억제 환자

최근에 발표된 Lemiale 등[30]에 의한 다기관 무작위 연구와 Frat 등[31]의 post-hoc analysis 연구에서는 NIV 치료가 면역억제 환자에서 의미 있는 효과를 보이지는 못했다. 하지만 그 이전에 나온 여러 연구들을[32-35] 포함한 메타분석에서는 급성호흡부전을 보이는 면역억제 환자에서 이른 시기에 시작한 NIV 치료가 기도삽관과 사망률을 낮출 수 있다는 결과를 보여주었다[7]. 이런 이유로 가이드라인에서는 면역억제 환자에서 NIV 치료를 조건부로 권장하고 있지만(conditional recommendation), 앞으로 대규모의 연구가 필요할 것이다.

4. 폐절제술 혹은 복부 수술 환자

폐나 복부 수술 시에 마취제, 통증, 횡경막 기능 저하 등으로 인해 폐의 용적이 감소하고 허탈(atelectasis)이 발생하여 저산소증과 호흡부전이 발생할 수 있다. 폐절제술을 받은 환자에서 NIV 치료가 기도삽관과 사망률을 감소시켰다는 무작위 연구가 있었다[36], 복부 수술을 받은 환자에서도 여러 연구를 통해(관찰연구 및 무작위 연구) 수술 후 발생한 호흡부전에서 지속기도양압 혹은 Bilevel-NIV를 시행한 경우 기도삽관, 감염, 사망률이 감소하였다는 점이 확인되었고[15,37,38], 메타분석에서도 의미 있는 결과를 보여주었다[7]. 단, NIV 치료를 시작하기 전에 환자 상태의 악화 원인이 anastomosis leakage 혹은 intra-abdominal sepsis 때문은 아닌지 반드시 검토가 우선되어야 한다.

기계환기 이탈에서의 비침습적 기계환기 치료

비선택적(unselected) 환자군이 포함된 연구에서는 탈관(extubation) 이후에 시행한 NIV 치료가 재삽관이나 사망률을 낮추지는 못하였다. 하지만 고위험군을(나이>65세, 심장이나 폐질환) 대상으로 한 Nava 등[39]과 Ferrer 등[40]의 다기관 무작위 연구에서는 NIV 치료가 재삽관과 중환자실 사망률을 낮추었다는 보고를 하였고, 기계환기 이탈을 위한 자발 호흡 연습 시에 고이산화탄소혈증이 발생하는 환자에서도 NIV 치료효과가 의미 있게 보였다는 결과가 발표되었다[41]. 따라서 고위험의 환자군에서 계획된 탈관이 이어진 후 초기에 NIV 치료를 시행한다면 재삽관율과 사망률을 낮출 수 있다고 볼 수 있다.
하지만 이미 호흡부전이 발생한 환자에서는 NIV 치료에 주의가 필요하다. Esteban 등[42]에 의한 무작위 연구를 보면 탈관 후 48시간 이내에 호흡부전이 발생한 환자에서 NIV 치료를 시행한 경우 재삽관 비율을 낮추지 못했고, 오히려 사망률이 높았다. 이후의 메타분석에서도 유의한 효과를 보여주지 못했다[43]. 따라서 인공호흡기를 이탈한 고위험 환자군에서는 호흡부전의 발생을 예방하기 위해 초기에 NIV 치료를 시행하는 것은 도움이 되지만, 이미 호흡부전이 발생한 경우에는 도움이 되지 않고 오히려 해로울 수 있다는 점을 인지할 필요가 있다.
COPD 환자나 고이산화탄소혈증을 보인 환자군을 대상으로 한 메타분석에서는 전통적인 인공호흡기 이탈방법, 즉, 압력을 서서히 낮추거나 자발 호흡 연습을 하는 것에 비하여 NIV 치료를 이용하여 이탈을 시도한 경우가 사망률과 재삽관 및 폐렴의 비율이 유의하게 낮았다[44]. 따라서 저산소혈증 환자에서의 NIV 치료의 근거는 부족하지만 고이산화탄소혈증을 보이는 환자에서는 NIV를 이용한 이탈이 도움이 될 수 있다.

완화치료를 위한 비침습적 기계환기 치료

말기암 환자를 대상으로 한 두 개의 무작위 연구에서 NIV 치료가 환자의 호흡곤란을 호전시켰다는 결과를 보여주었다[45,46]. 호흡 횟수나 산소화에서 효과가 있는지는 아직 불확실하지만, European Respiratory Society/American Thoracic Society 가이드라인에서는 NIV 치료가 말기암 환자의 호흡곤란을 호전시키고 opioid 사용을 의미 있게 줄일 수 있었다고 보고하면서 조건부로 권장하였다(conditional recommendation) [7].

인공호흡기 장비(machine, circuit, and mask)

급성호흡곤란증 환자에게는 비강 마스크(nasal mask)보다는 안면 마스크(facial mask)가 권장되고, 인공호흡기 mode는 일회호흡량을 유지하기 위해 압력형 모드(pressure mode)를 사용한다고 할 수 있다. 인공호흡기 장비로는 일반적으로 NIV-dedicated ventilator가 많이 사용되고 있다[4,6]. 이는 일반 병동과 중환자실에서 모두 사용이 가능하고, 중환자실의 일반 인공호흡기처럼 의료용가스를 이용하여 FiO2를 100%까지 올릴 수 있다. 특징은 exhalation hole이 있는 single-limb circuit을 이용하고 있고, 마스크에도 구멍이 있을 수 있다(vented mask). 이러한 NIV-dedicated ventilator는 중환자실의 일반 인공호흡기와 달리 circuit 에서의 압력 변화를 모니터링하여 일회호흡량을 추정하고 있고, circuit 과 마스크 주위로 나가는 공기누출(intentional and non-intentional air leak)을 보정할 수 있다. 또한, 환자의 자발 호흡과 synchrony를 최대한 맞출 수 있도록 설계되었다고 할 수 있다.
하지만, 드물게는 중환자실의 일반 인공호흡기를 이용하여 NIV를 적용하는 경우도 있다. 이런 경우는 흡기와 호기를 분리한 double-limb circuits이 그대로 이용되고, 구멍이 없는 non-vented mask를 사용하게 된다. 하지만 인공호홉기에 따라 마스크 주위로 생기는 공기누출에 대해 보정이 안 되는 경우가 있고, 환자 호흡과의 synchrony 측면에서 문제가 발생할 수 있다. 최근의 중환자실 인공호흡기에도 프로그램 안에 NIV mode가 포함되어 있는 경우가 많으므로 되도록이면 이를 이용하는 것이 환자에게 도움이 되리라 생각한다.

결론

NIV는 COPD 급성악화, 급성 심인성폐부종 환자에 적용하여 생존율을 향상시킬 수 있는 근거를 가지고 있으며, 고이산화탄소혈증 환자의 기계환기 이탈에서도 효과를 볼 수 있다. 이외에는 면역억제 환자와 폐 혹은 복부 수술 환자의 급성호흡부전에서도 시행해 볼 수 있다. 경증의 ARDS 환자에서도 NIV 치료법을 사용해 볼 수는 있지만, 중증도가 증가할수록 치료 실패율과 환자 사망률이 증가할 수 있다. NIV 치료법의 실패는(특히 저산소혈증 환자) 결국 높은 사망률과 관련이 있으므로 대상 환자를 잘 선택해야 하고, 적절한 시점에 시작을 해야 하며, 집중 관찰을 해야 한다.

이 논문은 최근 우리나라에서도 유병률이 급증하고 있는 급성 호흡부전 환자에서 기도삽관을 하지 않고 마스크를 이용하여 인공호흡기를 적용하는 비침습적 기계환기에 관한 적응증 및 임상 현장에서 고려해야 할 중요한 점을 정리한 논문이다. 급성 호흡부전은 폐렴이나 만성폐쇄성폐질환, 사이질폐질환, 천식 등 주요 폐질환에서 가장 중증의 임상적 형태이며 최근 COVID-19에서 가장 심각하고 중요한 합병증이기도 하다. 이 논문에서는 현재 기도삽관을 통한 기계환기를 대신해서 급성 호흡부전 환자에게 적응할 수 있는 비침습적 기계환기의 대상 환자, 사용 시 주의점, 특징 및 부작용 등을 일목요연하게 잘 정리하여 기술하고 있다. 또한 비침습적 기계환기 사용 시 반드시 숙지해야 하는 circuit에 관한 내용도 잘 기술하고 있다. 이 논문은 향후 COVID-19을 포함한 여러 호흡기 질병에서 급성 호흡부전 환자를 진료하는 임상 현장에 많은 도움이 될 것으로 판단된다.
