Abstract

Background: Postoperative pain management is a critical component of perioperative care in pediatric patients. Adequate pain control not only improves patient comfort, but also mitigates adverse physiological responses such as tachycardia, hypertension, immunosuppression, and impaired wound healing. With the growing emphasis on opioid-sparing strategies due to concerns over opioid dependency and misuse, regional analgesia has emerged as a key element of multimodal analgesia protocols. Concerns regarding opioid dependence and misuse have positioned regional analgesia as an essential part of multimodal analgesia, helping to reduce opioid consumption and its associated side effects in pediatric patients.
Current Concepts: Regional analgesia includes a range of techniques such as neuraxial, peripheral nerve, and fascial plane blocks. These methods have proven highly effective in reducing postoperative pain and the need for opioids in children. The advent of ultrasound-guided regional anesthesia has greatly improved the safety and accuracy of these techniques. Pediatric-specific anatomical and physiological factors, including immature descending pain modulation pathways and heightened sensitivity to local anesthetics, highlight the importance of customized approaches to regional anesthesia in this demographic.
Discussion and Conclusion: Regional analgesia is a cornerstone of multimodal analgesia in pediatric patients, effectively reducing opioid consumption and promoting recovery. Despite challenges such as provider expertise and anatomical considerations, advancements in ultrasound-guided techniques have increased safety and improved precision. Future directions should focus on expanding education and training in pediatric regional anesthesia, as well as conducting high-quality studies to further refine best practices.

서론

수술은 통증을 유발한다. 수술 후 통증은 환자를 고통스럽게 할 뿐만 아니라 빈맥, 고혈압, 나트륨과 물의 정체, 면역력 억제, 상처 회복의 지연, 위장관 폐쇄 등을 야기하고, 흉부 또는 복부의 통증은 심호흡을 방해하여 무기폐와 저산소증을 유발한다[1]. 따라서 통증 조절은 회복에 매우 중요한 요소이며, 적극적인 통증 조절을 통해 수술 후 재원기간을 줄이고 환자 만족도를 높일 수 있다. 최근 아편유사제 의존이나 오/남용이라는 심각한 사회 문제가 발생하면서, 아편유사제 사용 최소화(minimizing opioid consumption)를 위한 수술 후 통증 조절 전략(opioid sparing strategies for perioperative pain management)의 중요성이 매우 강조되고 있다[2].

수술 후 회복 향상 프로그램(enhanced recovery after surgery)에서는 주술기 다중모드 통증조절(multimodal analgesia)의 요소로, (1) 아세트아미노펜(acetaminophen), 비스테로이드성소염제(nonsteroidal anti-inflammatory drugs, NSAIDs) 등 비마약성 진통제(non-opioid analgesics)와 더불어, (2) 신경축 차단(neuraxial block), 말초신경 차단(peripheral nerve block), 면 차단(plane block), 구획 차단(compartment block), 국소마취제 침윤(local anesthetics infiltration) 등의 다양한 부위 마취(regional analgesia) 기법을 적극적으로 사용하도록 권장하고 있다(Figure 1) [3]. 다중모드 통증조절을 통해 아편유사제(opioids)의 사용과 관련된 호흡 저하(respiratory depression), 구역과 구토(nausea and vomiting), 소양증(pruritus), 변비(constipation) 등을 줄이고, 아편계 내성(opioid tolerance), 아편계 사용 장애(opioid use disorder), 아편계 남용(opioid abuse) 등 장기적인 부작용을 예방할 수 있다[4].

소아 환자에서 주술기 통증이 잘 조절되지 않으면 장기적인 행동 변화(behavioral change)를 유발하여 향후 통증 반응(response to pain)에 영향을 끼칠 수 있다[2]. 지난 10여 년 동안 초음파 장비가 수술장에 많이 도입되었고[4], 대규모 레지스트리 연구 결과를 통해 소아 부위 마취의 효과와 안전성이 입증되어 다양한 부위 마취 방법이 소아 마취 영역에서 사용되고 있다[5-7]. 이 논문에서는 소아 수술에서 부위 마취 기법을 적용할 때 고려할 점을 살펴보고, 각 수술에 적용할 수 있는 다양한 소아 부위 마취 기법을 알아보고자 한다.

소아 통증 조절에서 부위 마취의 역할과 고려해야 할 점

부위 마취는 소아 환자에게 사용할 수 있는 가장 강력한 통증 조절 수단으로, 전신마취제 사용량과 이와 관련된 신경독성(neurotoxicity) [8], 아편유사제 사용량, 스트레스 반응(hormonal stress response), 수술 중 출혈(intraoperative blood loss)을 줄이고 수술 후 빠른 회복을 유도하여 수술 후 기계 환기(mechanical ventilation) 기간을 줄이고 조기 식이(early feeding)와 조기 보행(early mobilization)을 가능케 하여 의료진과 보호자가 소아 환자를 더욱 쉽게 돌볼 수 있게 해준다[4,9]. 부위 마취는 환자와 보호자에게 보다 나은 회복 경험을 제공할 뿐만 아니라, 의료자원과 의료비용을 감소시킬 수 있다.

1. 소아 부위 마취에서 해부학적으로 고려해야 할 점

소아 환자에서 통증을 전달하는 상행신경로(ascending pathway)는 이미 완성되어 있으나, 통증을 조절하는 하행신경로(descending pathway)는 미성숙하여 통증을 더 심하게 느낀다. 감각신경이 담당하는 부위가 넓어서 통증의 위치를 정확히 특정하기 어렵고, 신경의 수초화(myelination)가 완성되지 않아서 국소마취제(local anesthetics)의 효과가 더 강하게 나타날 수 있다[2]. 또한 (1) 혈중 결합 단백질(binding protein)의 농도가 낮아서 단백질과 결합하지 않은 국소마취제(unbound local anesthetics)의 비율이 높다는 점, (2) 심박출량이 높아 전신에 국소마취제가 빠르게 퍼질 수 있다는 점, (3) 심장의 소듐 채널(sodium channel)이 많이 열려있고, 국소마취제와 친화력이 높다는 점, (4) 9개월 미만에서는 간기능이 미성숙하다는 점 때문에 국소마취제의 전신 부작용에 취약하다. 소아에서 허용되는 국소마취제의 최대 용량은 Table 1과 같다. 부피바케인(bupivacaine)의 심장독성(cardiac toxicity)에 대한 우려로 로피바케인(ropivacaine)이 소아에서 더욱 널리 사용되고 있다.

신경이 가늘고, 신경을 둘러싸는 신경 내막(endonurium)이 얇다. 또한 신경 주위를 둘러싸고 있는 주요 혈관이나 힘줄(tendon)들의 크기가 작고, 서로 가까이 위치하고 있다[2]. 따라서 소아에서 부위 마취를 할 때는 해부학적 구조물을 정확히 파악하고, 시술 부작용을 예방하기 위해 초음파 유도(ultrasound-guidance)를 사용해야 한다[6,9]. 소아 부위 마취를 안전하게 시행하기 위해서는 소아 환자의 해부학적 구조물과 초음파 영상에 익숙해지는 과정이 필수적이다. 목표로 하는 구조물까지의 거리가 성인에 비해 짧고 비교적 잘 보인다는 장점은 있지만, 실제 부위 마취를 시행할 때 피부하 조직, 결합조직과 근육이 얇고 매우 쉽게 뚫리기 때문에 목표로 하는 구조물까지의 거리를 고려하여 바늘이 더 깊이 들어가거나 신경에 손상을 주지 않도록 조심스럽게 조작해야 한다.

신경축(neuraxial) 마취를 시행할 때는 성인과 해부학적인 차이가 있다는 점을 고려해야 한다[2,9]. 신생아와 영아에서는 경막낭(dural sac)이 S3-S4 (성인 S2)에서 끝나기 때문에 미측 경막외 신경차단술(caudal epidural block)을 시행할 때 경막이 뚫리지 않도록 주의를 요한다. 척수(spinal cord)가 L3 (성인은 L1)에서 끝나기 때문에 척수마취(spinal anesthesia)를 시행할 때 반드시 L4 레벨 아래에서 시행해야 하고, 척수마취시 랜드마크로 사용하는 장골능간선(intercristal line)이 L5-S1 (성인은 L4-L5)에 위치한다는 점을 고려해야 한다.

2. 소아의 통증 평가에서 고려해야 할 점

수술은 통증을 유발하고, 이전에 통증에 대한 경험이 적은 소아 환자는 통증을 보다 심하게 느낄 가능성이 높다. 통증은 환자가 주관적으로 느끼는 불편인데, 학령기 이전의 어린 소아 환자는 통증에 대한 경험이 적어 스스로 객관화해 평가하고 표현하기 어렵다. 더불어, 소아 환자가 수술 후 울거나 힘들어할 경우, 이를 통증이 아닌 각성기 섬망(emergence agitation)이나 분리 불안(separation anxiety) 때문으로 오해하여 통증이 적절히 조절되지 않는 경우가 많다. 수술 후 통증에 대해서 설문조사를 했던 연구에 따르면 충수돌기절제술을 받은 환자의 24.8% 편도절제술을 받은 환자의 20.2%가 수술 후 통증 조절이 모자랐다고 대답했고, 아편유사제의 사용과 그에 따른 부작용이 증가했다[10]. 수술 후 통증 조절이 모자란 것에 연관되는 인자를 분석한 결과 비마약성 진통제를 두 가지 이상 사용했을 때 통증 조절이 모자랐다고 대답하는 비율이 감소하였다. 특히 수술 직후에 과행동을 보일 때 각성기 섬망 때문인지 통증 때문인지 구분이 어려울 수 있는데, 각성기 섬망은 통증이 전혀 없을 때에도 과행동을 보이는 것이기 때문에, 수술이 유발한 손상(injury)의 정도에 합당한 통증 조절을 미리 제공하는 것이 중요하다. 따라서 마취에서 회복하기 전 비마약성 진통제나 부위 마취로 적극적으로 통증을 조절해야 한다.

소아에서 통증을 평가할 때는 연령에 따른 발달상태나 인지기능에 맞는 통증 평가 척도를 사용해야 한다. 6개월 미만에서는 CRIES (C-Crying; R-Requires increased oxygen administration; I-Increased vital signs; E-Expression; S-Sleeplessness) scale, 2개월에서 7세까지는 FLACC scale (face, legs, activity, cry, consolability scale)을 주로 사용하며, 3세 이후부터나 의사소통이 가능한 소아에서는 faces pain scale을 사용할 수 있다. 5세 이상이거나 의사소통이 가능한 소아에서는 성인과 마찬가지로 숫자통증등급(numeric rating scale)을 사용할 수 있다. 회복실이나 중환자실, 병실에서 다양한 통증 평가 척도를 사용할 수 있도록 정기적인 의료진 교육이 필요하다. 통증 평가는 1일 3회 이상 정기적으로 평가하고 기록해야 하며, 4/10점 이상일 때는 통증 조절을 위한 중재(intervention)와 재평가가 필요하다.

3. 소아에서 부위 마취를 시행할 때 고려해야 할 점

통증 조절을 위한 부위 마취를 시행할 경우 환자와 보호자의 동의가 필요하고 부위 마취와 관련된 근력 저하나 감각 저하, 지속시간에 대해서 충분한 설명이 필요하다. 특히 구획증후군(compartment syndrome)이 발생할 수 있는 환자에서는 부위 마취가 통증 등 여러 감각을 무디게 할 수 있으므로 주의를 요하는 것이 좋다[4]. 부위 마취를 시행하는 곳의 원위부의 근력이나 감각에 대한 평가가 필요한 경우 초음파 유도 하에 카테터를 거치해 놓고, 평가가 끝난 뒤에 약물을 주입하는 방법을 사용할 수도 있다. 부위 마취의 범위와 지속시간에 대해서 진료과와 상의가 필요하며, 마취회복실, 병동, 중환자실 등에서 비마약성 진통제나 자가통증조절장치(patient controlled analgesia)가 계획대로 투여되도록 인계가 필요하다.

소아에서 부위 마취를 시행할 때는 의식 하에 환자의 협조를 얻기 어렵기 때문에 대부분 전신마취 유도 후 수술 전이나 수술 후에 시행한다[9]. 전신마취 하에 시행하므로 신경근 차단(neuromuscular block) 중에는 신경자극기(nerve stimulator) 사용이 어렵다는 점, 발작(seizure)과 같은 국소마취제 전신독성(local anesthetic systemic toxicity, LAST)을 발견하기 어렵다는 점을 고려해야 한다. French-Language Society of Pediatric Anesthesiologists (ADARPEF)에서 시행한 대규모 연구에 따르면 6개월 미만에서는 6개월 이상의 환자에 비해 4배 정도 부위 마취 관련 부작용이 증가하는 것으로 알려져 있다[7]. 10만 건 이상의 소아 부위 마취 케이스를 분석한 Pediatric Regional Anesthesia Network 연구에 따르면, 전체 소아 부위 마취 중 전신마취 하에 진행한 경우가 93.7%였고, 의식이 있거나 진정 하에 진행한 환자들 중 대부분(81.9%)이 10세 이상이었다[6]. 이 연구에서 신경 부작용(neurologic complication)은 10,000건당 2.4건, LAST는 10,000건당 0.76건이 발생했으며, 1세 미만의 영아에서 주로 발생했다. 따라서 안전하고 효과적인 소아 부위 마취를 위해 초음파 유도를 반드시 사용하는 것이 좋다[9].

수술에 따른 부위 마취 방법

1. 소아 정형외과 수술을 위한 부위 마취

상/하지 수술을 위해 정형외과 영역에서 사용되는 다양한 부위 마취 방법은 소아에도 그대로 적용할 수 있다(Table 2) [11-13]. 다만 소아 환자에서는 수술이 필요한 질병의 종류가 성인과 다르다. 소아 정형외과 영역에서는 골절(fracture) 등의 부상(injury)이 가장 흔하고, 선천 기형(congenital anomaly), 척추측만증(scoliosis), 종양(malignancy) 등이 뒤를 따르며, 모든 수술 과정에서 아이의 성장을 고려해야 한다. 특히 뇌성마비(cerebral palsy)가 있는 환자에서는 호흡기계 위험으로 아편유사제 사용이 어렵고 수술 후 통증이 경직(spasticity)을 악화시키기 때문에, 적절한 부위 마취를 제공하는 것이 매우 중요하다[12].

2. 소아 비뇨기과 수술을 위한 부위 마취

미측 경막외 신경차단술은 소아 환자의 배꼽 이하 수술에서 공히 고려할 수 있는 마취 방안이다[14]. 통상적으로 0.125-1.25% 부피바카인 또는 0.1-0.375% 로피바카인이 기본 약제로 사용되며, 투여량은 0.5-1.5 mL/kg 정도로 한다[15]. 약물 단회 주입의 경우, 클로니딘(clonidine) 1-2 μg/kg, 덱스메데토미딘(dexmedetomidine) 1-2 μg/kg 또는 아편유사제 10-30 μg/kg를 첨가하는 것이 효과적일 수 있다[15]. 단회 약물 주입 및 경막외 카테터 거치 양쪽의 경우 모두에서 초음파 유도하 시술을 하는 경우가 그렇지 않은 경우에 비해서 성공률, 합병증 등에서 더 나은 결과를 가져다준다는 것이 알려져 있다[15,16].

미측 경막외 신경차단술은 요도 하열(hypospadias) 교정술, 음경성형술(penoplasty)에서 효과적인 통증 조절 효과를 제공한다. 그러나 요도 하열 교정술에서의 미측 경막외 신경차단술은 음경 신경 차단술과 비교하였을 때 요도피부샛길(urethrocutaneous fistula)의 발생 빈도가 높아질 수 있어 주의가 필요하다[17]. 그에 따른 대안으로 음부 신경 마취(pudendal nerve block) [18] 또는 음경 신경 차단술(penile nerve block)을 적용할 수 있다.

방광요관역류(vesicoureteral reflux)의 교정을 위한 요관방광문합술(ureteroneocystostomy) 또는 신우요관 폐색(ureteropelvic junction obstruction)을 위한 신우성형술(pyeloplasty)에서도 미측 경막외 신경차단술이 통증 경감에 도움이 될 수 있다.

고환고정술(orchiopexy)의 경우 장골서혜신경(ilioinguinal nerve)/장골하복신경(iliohypogastric nerve) 차단술이 수술 후 통증 경감에 도움이 됨이 전통적으로 알려져 있다[19]. 또한, 포경수술(circumcision)의 경우는 음경 신경 차단술이 미측 경막외 신경차단술보다 더 효과적이라고 알려져 있다. 최근 횡근막면 차단술(transversalis fascia plane block)이 소아의 요관방광문합술에서 수술 후 통증 경감 효과가 있음이 보고된 바 있다[20].

3. 소아 외과 수술을 위한 부위 마취

복직근 초 차단술(rectus sheath block)은 복직근의 뒤쪽 근막(posterior rectus sheath) 내부에 국소마취제를 주입하여 복직근 아래를 지나는 전방 가지 신경을 차단하여 주로 T7-T11 수준의 체성 신경을 차단한다[21]. 배꼽 정중선 절개가 있는 개복 수술 또는 복강경 수술에서 시행할 수 있으며, 배꼽 탈장(umbilical hernia repair), 유문절개술(pyloromyotomy), 위루술(gastrostomy insertion) 등에서 적용될 수 있다[22,23]. 소아외과 복강경 수술의 많은 경우 배꼽 포트를 포함하므로 다양한 수술 종류에서 적용 가능하다.

복부 횡단면 차단(trasversus abdominis plane block)은 내복사근과 복횡근 사이에 국소마취제를 주입하여 복벽의 감각신경을 차단하는 방법으로 충수 절제술, 담낭 절제술, 담낭 낭종 절제술, 간절제술, 카사이 수술 등을 포함한 복강경 및 개복 수술에서 적용할 수 있다[21-23]. 국소마취제 주입 위치에 따라 차단 범위가 달라지므로, 늑골하, 측면, 후방 측면 방법 중 적절한 방법을 선택하여 시행하는 것이 추천된다[21].

요방형근 차단(quadratus lumborum block)은 체성 및 내장 통증 차단이 가능하며, T6-L1 수준의 차단이 가능하여, 서혜부 탈장 복원술, 하복부 수술, 골반강 수술을 포함한 다양한 수술에서 적용할 수 있다[21,24].

장골서혜신경-장골하복신경 차단은 내복사근과 복횡근 근막 사이에 위치한 장골서혜신경과 장골하복신경에 국소마취제를 주입하는 방법으로 주로 L1 신경을 차단한다. 이는 서혜부 탈장 복원술 시 시행할 수 있으며, 비교적 낮은 부작용과 높은 성공률을 보인다[5].

척추기립근면차단(erector spinae plane block) 또는 방척추신경차단술(paravertebral block)은 흉복부 척수신경의 후지와 전지 모두를 차단할 수 있는 방법으로, 다양한 복강경 또는 개복 수술의 통증 관리에 유용하다[24,25].

4. 소아 성형외과 수술을 위한 부위 마취

안면 수술을 시행하는 경우 삼차신경의 안신경, 상악신경, 하악신경 차단을 통해 두피의 전면부, 얼굴, 그리고 구강 내 상당 부분에 마취 및 진통 효과를 제공할 수 있다[23,26]. 두피나 이마 수술에서 상안와신경(supraorbital nerve) 및 상활차신경(supratrochlear nerve) 차단술을 시행할 수 있다[23,26]. 상관골상악신경 차단(suprazygomatic maxillary nerve block)은 구순열, 구개열, 비성형술, 양악 절골술 등의 수술에서 효과적이며, 권골(zygoma)과 후방 안와연(posterior orbital rim)의 교차점에서 익구개와(pterygopalatine fossa)를 향해 주사하는 방법으로 시행한다[5,27-30]. 안와하신경(infraorbital nerve), 대구개신경(greater palatine nerve) 및 턱끝신경(mental nerve)은 각 신경이 분지되는 구멍을 향해 구강 내에서 주사하여 시행될 수 있어, 혈종이나 바늘 자국이 남지 않게 시술을 할 수 있다는 것이 큰 장점이며 구순열, 구개열 수술에서 효과적이다[5,26].

치조골성형술과 같이 장골 이식술이 필요한 경우 복부 횡단면 차단과 횡근막면 차단이 효과적인 것으로 알려져 있다[21,31]. 장골 근막 횡단면 차단은 국소마취제를 복횡근의 건막 확장부(transversus abdominis aponeurotic extension) 장골 근막 아래쪽의 신장주위 지방조직에 주사하는 기법이며, 복부 횡단면 차단은 내복사근과 복횡근 사이에 국소마취제를 주사하는 방법이다[5,21,23]. 요방형근 차단 역시 효과적인 방법이다[32].

귀 성형술 또는 코 성형술과 같은 두개안면 재건술에 필요한 자가늑연골 이식 시 늑간신경 차단술, 전거근 평면 차단(serratus anterior plane block), 척추기립근면차단, 흉골연 차단(parasternal block), 방척추신경차단술의 효과가 알려져 있다[33-35]. 연골 제거시 초음파로 plane 확인이 어려워 수술 전 시행하는 것을 추천하며, 늑간신경 차단술의 경우 유지시간이 길지 않아 카테터를 삽입하여 유지하는 것도 효과적인 방법이다[35].

성형외과 수술에서는 절개 전 국소마취제 주입이 외과의에 의해 선행되는 경우가 많다. 따라서, 수술 후 통증을 위한 부위 마취를 진행할 때는 기존 주입된 마취제 양을 고려해 전신독성이 발생하지 않도록 주의해야 한다[5].

5. 소아 흉부외과 수술을 위한 부위 마취

흉골절개술(sternotomy)로 심장 수술을 받는 소아 환자의 통증 관리에 대해서 2023년 유럽부위마취학회 PROSPECT 가이드라인을 소개하려고 한다[36]. 가이드라인에서는 (1) 수술 중과 수술 후 paracetamol과 NSAIDs, (2) 수술 중 마그네슘이나 덱스메데토미딘 사용을 고려할 수 있고, (3) 흉골연 차단 또는 수술 부위 국소마취제 도포(surgical wound infiltration)를 통증 조절 방법으로 제시하고 있으며, (4) 아편유사제는 구조(rescue) 약물로만 사용할 것을 강조하고 있다. 가이드라인에서는 방척추신경차단술은 통증 조절 효과가 확실치 않고, 척추기립근면차단은 근거가 충분하지 않으며, 흉추경막외차단(thoracic epidural)은 효과는 탁월하지만 출혈이나 항응고요법(anticoagulation)으로 합병증이 발생할 수 있다는 이유로 흉골연 차단을 시행할 것을 권장하고 있다. 흉골연 차단은 심장수술을 받는 소아 환자에서 효과적으로 통증을 줄인 것으로 보고되어 있다[37,38].

흉골연 차단은 흉골연의 늑간신경(intercostal nerve)을 차단하는 것이 목적으로 차단을 시행하는 위치에 따라서 깊은 흉골연 차단(deep parasternal block)과 얕은 흉골연 차단(superficial parasternal block)으로 나뉜다[39-41]. 깊은 흉골연 차단은 내늑간근(internal intercostal muscle)과 흉횡근(transversus thoracis muscle) 사이의 공간을 목표로 하는데, 한번의 주사로 여러 레벨을 동시에 차단할 수 있다는 장점이 있는 반면, 내흉동맥이나 정맥(internal thoracic vessels)이 지나갈 수 있으므로, 초음파로 혈관의 위치를 파악하고 피해서 주사해야 한다는 단점이 있다[38]. 얕은 흉골연 차단은 대흉근(pectoralis major muscle)과 내늑간근 사이의 공간을 목표로 하는데, 혈관이 없어서 비교적 안전하다는 장점이 있으나, 한 번의 주사로 여러 레벨을 동시에 차단할 수는 없어서 한쪽에 두 번(T3, T5) 정도의 주사가 필요하다[40]. 흉골연 차단은 수술이 끝나고 수술 부위에 드레싱을 적용한 후에 자세 변화 없이 반듯이 누운(supine) 자세에서 시행할 수 있으며, 해부학적 구조도 단순하다는 장점이 있다(Figure 2). 흉골연 차단으로는 배액관(drainage)의 통증이나, 기관 내 삽관으로 인한 통증을 줄여주지는 못하기 때문에 배액관 부위에는 추가로 국소마취제 침윤(local infiltration)을 적용하는 것이 좋다[40]. 진행된 폐동맥고혈압이나 심실부전의 증거가 없는 비교적 간단한 소아 심장수술을 받는 환자에서는 흉골연 차단술과 덱스메데토미딘을 활용하여, 수술장에서 기관 발관(extubation)을 시행하는 패스트트랙(fast-track)을 적용할 수 있다[42].

아직 성장 중인 소아에게 심박동기(pacemaker)가 필요할 때는 심외막(epicardium)에 전극(lead)을 위치시키고, 심박동기는 심장과 가까운 상복부(epigastric area)에 위치시키게 된다. 상복부에 위치한 심박동기는 5-10년마다 교체가 필요한데 이 때 늑하부 복부 횡단면 차단술(subcostal transversus abdominis plane block)을 사용하면, 시술 부위의 통증을 효과적으로 조절할 수 있다[43].

흉강경수술(video-assisted thoracoscopic surgery)을 받는 소아 환자의 통증 관리에 대해서는 2022년 유럽마취학회의 PROSPECT 가이드라인을 근거로 기술한다[44]. 흉공연절개술과 마찬가지로 (1) 수술 중과 수술 후 paracetamol과 NSAIDs, (2) 수술 중 덱스메데토미딘과 더불어, (3) 방척추신경차단술 또는 척추기립근면차단을 통증 조절 방법으로 제시하고 있으며, 소아에서도 그 효과가 증명되어 있고, (4) 아편유사제는 구조 약물로만 사용할 것을 강조하고 있다. 흉추경막외차단은 효과는 탁월하지만 앞서 소개한 두 가지 차단술보다 훨씬 침습적이고 저혈압과 요저류가 발생할 확률이 높아서, 개흉술(thoracotomy)에만 사용하도록 권장하고 있다[45].

6. 소아 신경외과 수술을 위한 부위 마취

개두술(craniotomy) 후 아편유사제 사용은 뇌신경학적 검사(neurologic exam)를 지연시키고, 구역과 구토를 유발하여 두개뇌압(intracranial pressure)을 상승시킬 수 있기 때문에 구조 약물로만 사용해야 한다. 따라서 2023년 유럽부위마취학회 PROSPECT 가이드라인에서도 (1) 수술 중과 수술 후 paracetamol과 NSAIDs [46,47], (2) 수술 중 덱스메데토미딘과 더불어, (3) 두피신경차단(scalp nerve block) 또는 수술 부위 국소마취제 도포(incisionsite infiltration)를 통증 조절 방법으로 제시하고 있으며, (4) 아편유사제는 구조 약물로만 사용할 것을 강조하고 있다[48]. 특히 대한민국을 비롯한 동아시아권에서 호발하는 모야모야병(moyamoya disease)의 경우 통증이 충분히 조절되지 않으면 우는 과정에서 과호흡이 발생하여 일과성 뇌허혈발작(transient ischemic attack)이나 뇌경색(cerebral infarction)이 발생할 수 있기 때문에[49], 두피신경차단 등 부위 마취 기법을 적극적으로 적용하는 것이 환자의 예후에 큰 도움이 될 수 있다.

결론

부위 마취는 주술기 다중모드 통증조절에서 가장 강력하고 중요한 요소로, 아편유사제 사용을 줄이고, 빠른 회복을 가능케 하며, 의료자원과 의료비용을 아낄 수 있게 한다. 성인 환자에서는 수술 후 회복 향상의 한 요소로 다양한 부위 마취 기법들이 소개되고 사용되고 있으나, 소아 환자에서는 여러 문헌이 소개되었음에도 불구하고 소아 환자 자체나 부위 마취의 안전성에 대한 막연한 두려움, 경험자의 부재 등으로 아직 널리 사용되지 못하고 있는 현실이다. 하지만 어린 나이에 수술이라는 큰 스트레스를 경험함에도 자신의 통증을 표현하기 힘든 소아 환자들이야말로 부위 마취가 가장 필요한 환자군으로, 소아 부위 마취는 단순한 통증조절을 넘어서 주술기 환자관리를 개선하고 아편유사제 사용을 비롯한 소아 환자의 의료 관련 행동 양상에 긍정적인 영향을 끼칠 것이다. 따라서 소아의 수술 후 통증은 의료진 주도하에 적극적으로 예방해야 하며, 주술기 아세트아미노펜, NSAIDs와 수술 중 덱스메데토미딘 등과 더불어 수술 부위에 맞는 부위 마취 기법을 마취에서 회복하기 전에 선제적으로 적용하는 것이 중요하다. 부위 마취의 효과를 극대화 하기 위해서는 마취통증의학과 의사뿐 아니라 외과의사를 비롯한 주술기 환자관리에 참여하는 모든 의료진의 변화가 필요하다[50]. 최신 가이드라인에서는 복잡하고 위험한 부위 마취 기법보다는 보다 단순하고 안전한 기법을 권장하는 쪽으로 트렌드가 변하고 있기 때문에, 처음부터 다양한 부위 마취를 도입하기보다는 특정 소아 수술을 대상으로 부위 마취를 꾸준히 적용하는 것이 의료진의 진료 관행을 바꾸는 데 효과적일 것이다.
