Abstract

Purpose: This study aimed to identify the healthcare election promises made by political parties whose members participate in the Health and Welfare Committee of the 22nd National Assembly. We evaluated whether Korea’s major political parties address various components of the health system, pursue balanced goals for healthcare improvement, and utilize diverse instruments to reform the health sector.
Methods: We identified healthcare promises from the top 10 promises submitted by each political party to the National Election Commission, focusing on those classified under “health and welfare” or containing relevant healthcare content. The unit of analysis was the implementation methods within each promise. These promises were then classified using the WHO’s six building blocks of a health system, the quintuple aim for healthcare improvement, and the five control knobs for health sector reform.
Results: A total of 33 election promises were identified. Promises related to regional health services and the health workforce were most common, followed closely by caregiving. In terms of the health system building blocks, service delivery was the dominant focus, and among the quintuple aims, improving the patient experience prevailed. Regulation emerged as the most frequently used control knob.
Conclusion: Healthcare promises mirror societal demands concerning the health workforce, regional healthcare disparities, and caregiving burdens. Although the content varied among parties, significant commonalities were observed. A balanced approach to healthcare financing, along with a broader range of policy reform instruments beyond regulation, is necessary to address health system challenges.

서론

선거 후보자나 정당은 공약을 통해 특정 분야의 발전에 관한 방향성을 제시하고 정책 과제에 우선순위를 부여함으로써 자신의 정책 의지를 드러낸다[1]. 유권자는 공약을 비교하여 후보자나 후보자 소속 정당의 정책 능력을 검토함으로써 자신의 대표자를 선택하기도 한다. 이는 정책 지향적인 선거 문화를 가능케 하는 규범적 토대다[2]. 따라서 정당의 총선 의료 공약을 살펴보는 것은 정당이 의회를 통하여 추구하는 의료 정책의 방향성을 파악한다는 의미가 있다. 유권자는 이를 바탕으로 알 권리를 누릴 수 있고 선거에서 합리적인 선택을 하는 데 도움을 받을 수 있다. 또한 의료계는 정당의 의료 정책 청사진을 파악하여 정책 변화에 적절하게 대처할 수 있다.

그간 총선 또는 대선 의료 공약에 대한 분석은 간헐적으로 이루어진 바 있다. 2016년, 정형준은 20대 총선에서 각 정당이 제시한 의료 공약을 비교하였고[3], 2017년 Lee와 Park [4]은 19대 대선 주요 후보자의 의료 공약을 분석하였다. 최근에는 대한의사협회 의료정책연구원이 정당별 21대 총선 의료 공약을 비교하였고[5], 경기도 공공보건의료지원단은 22대 국회의원 선거에 후보자를 낸 정당의 의료 공약을 분석하였다[6]. 이들 연구는 키워드를 기준으로 각 정당의 공약을 분류하고 당시의 사회적 및 정치적 쟁점을 함께 제시하여 의료 공약의 배경과 내용을 파악하기에 용이하다. 하지만 기존 연구는 공약의 내용에 집중하여 각 정당의 공약이 의료 시스템의 어느 분야를 포괄하였는지, 그 지향점이나 목표가 무엇인지, 개혁 수단이 무엇인지까지는 관심을 기울이지 못하였다.

이 연구는 공약의 내용을 파악하는 것뿐 아니라 한국의 주요 정당이 공약을 통하여 의료 시스템의 여러 분야 과제를 다루는지, 균형 있는 정책 목표를 지향하고 있는지, 다양한 의료 개혁 수단을 적용하려는지 검토하고자 한다. 또 다수의 기존 연구가 선거에 후보자를 낸 모든 정당의 공약을 분석하였다면[4-6], 이 연구는 법안 발의나 예산안 심의 과정에서 영향력이 크고 참여도가 높은 국회 보건복지 위원회 배정 국회의원 소속 정당의 의료 공약을 대상으로 삼음으로써 분석의 현실성을 높이고자 하였다.

방법

1. 공약 분석 대상 정당 선정

2024년 4월 10일에 실시된 제22대 국회의원 선거 결과, 총 300명의 국회의원이 선출되었다. 이 중 지역구 당선인이 254명, 비례 대표 당선인이 46명이었다. 국회의원은 입법, 재정, 외교, 일반 국정 등 여러 분야에서 활동을 하는데[7], 그중 대표적인 것이 법률 제·개정과 예산안 심의다. 국회법 제79조 및 제81조에 따르면 국회의원은 특정 상임 위원회에 속해 있지 않더라도 의원 10명 이상의 동의를 얻어 법안을 발의할 수 있지만 발의된 법안은 주제에 따라 관련 상임 위원회에 회부되어 심사를 거쳐야 본회의에 부의될 수 있다[8]. 또 같은 법 제84조에 따르면 예산안 심의와 결산의 경우에도 상임 위원회가 먼저 예비 심사를 해야 하며, 예비 심사 결과가 회부되는 예산 결산 특별 위원회도 각 예산 항목의 금액을 증가시키거나 새 항목을 만들 때 상임 위원회의 동의를 받아야 한다[8]. 이렇듯 정당이 공약을 법률과 예산으로 실현하는 데에는 상임 위원회 소속 의원 유무가 중요하므로 제22대 국회 보건복지 위원회에 배정된 의원의 소속 정당인 더불어민주당(Democratic Party of Korea), 국민의힘(People Power Party), 조국혁신당(Rebuilding Korea Party), 개혁신당(New Reform Party)의 의료 공약을 분석하기로 하였다.

2. 각 정당 의료 공약 특정

더불어민주당, 국민의힘, 조국혁신당, 개혁신당 등 4개 정당과, 더불어민주당의 비례 위성 정당인 더불어민주연합(Democratic Alliance of Korea), 국민의힘의 비례 위성 정당인 국민의미래(People Future Party) 등 2개 정당을 포함한 총 6개 정당의 총선 공약을 연구 자료로 삼았다. 이때 비례 위성 정당의 공약은 나중에 합쳐진 모정당의 공약과 합쳐서 분석하였는데 중앙선거관리위원회(이하 중앙선관위)가 2024년 6월에 발행한 ‘제22대 국회의원 선거 정당 정책 모음집(이하 모음집)’ [9]에 실린 공약을 연구 자료로 삼았다. 총선에 참여하는 정당은 중앙선관위에 10대 공약을 제출하는 데 각 공약은 정책 분야, 목표, 이행 방법, 이행 기간, 재원 조달 방안 등의 항목으로 구성되어 있다. 연구진은 먼저 각 정당의 10대 공약 중 (1) 정책 분야가 ‘보건복지’로 분류되었거나, (2) 그 외 분야로 분류되었지만 목표에 보건복지에 해당하는 내용이 들어간 공약 22개를 추출하였다(더불어민주당 6개, 국민의힘 12개, 개혁신당 2개, 조국혁신당 2개). 다음으로 두 연구자가 추출한 22개 공약의 정책 목표 및 이행 방법을 독립적으로 읽으면서 ‘의료’ 분야에 해당한다고 판단한 공약을 특정하였고, 두 연구자의 의견이 일치하지 않은 공약에 대해서 논의를 통하여 합의를 도출하였다. 그 결과, 14개의 공약을 의료 공약으로 선별하였다(더불어민주당 4개, 국민의힘 7개, 개혁신당 1개, 조국혁신당 2개). 이때의 ‘의료’는 좁은 의미의 의료, 즉 질병 진단과 치료 중심의 의료로 간주하였다[10].

3. 공약 분류를 위한 분석 단위 결정

그런데 ‘모음집’에 수록된 각 정당의 공약은 구체성의 정도가 다르고 형식도 다양하였다. 예를 들어 하나의 공약에 대하여 여러 목표를 제시하거나 하나의 목표에 대하여 여러 가지 이행 방법을 제시하는 경우가 있었다. 따라서 공약을 분류하기 위해서 ‘분석 단위(unit of analysis)’의 결정이 필요하였는바 이 연구에서는 이행 방법을 분석 단위로 정하였다. 왜냐하면 ‘모음집’에 수록된 공약의 구성 항목인 정책 분야, 목표, 이행 방법, 이행 기간, 재원 조달 방법 중 이행 방법에 대한 서술이 가장 구체적이어서 정책 내용을 파악하기에 용이하였고, 정책 수단이 이행 방법에 대한 서술에서 잘 드러났기 때문이다. 더불어민주당의 일부 공약을 예로 들어 보자. 해당 공약에는 여러 정책 목표가 있고 각 정책 목표에는 여러 가지 구체적인 이행 방법이 제시되어 있다(Table 1) [9]. 따라서 이 예에서는 1개의 공약 대신 9개의 이행 방법이 분석 대상 의료 공약이 된다. 다만 공약에 이행 방법 항목 자체는 없으나 이행 방법에 관한 서술이 공약의 정책 목표 항목에 제시된 경우에는 이를 이행 방법으로 취급하였다.

4. 공약 분류 기준 선정과 분류

의료 공약을 분류하기 위한 기준은 크게 내용적 분류 기준과 이론적 분류 기준으로 나눌 수 있다. 내용을 바탕으로 공약을 분류하는 것은 독자의 직관적 이해를 도울 수 있으나 공약 분석의 도구로서는 다소 평면적이라는 한계가 있다. 따라서 이 연구에서는 공약 내용에서 키워드를 뽑아 공약을 분류하는 것 외에도, 기존 연구 성과를 바탕으로 의료 시스템의 구성 요소, 의료 개선의 5중 목표, 의료 개혁을 위한 조종 손잡이(control knobs)라는 세 가지 이론적 분류 기준을 적용하였다.

1) 의료 시스템의 구성 요소

2007년, 세계보건기구(World Health Organization, WHO)는 각국의 의료 개혁에 도움을 주고자 가이드라인이 될 만한 보고서를 발간하였다[11]. 이 보고서는 각국 의료 시스템의 성과(performance)를 평가한 2000년 세계보건보고서[12]의 연장선에서 의료 시스템에 대한 공통 정의를 제시하고 의료 시스템 강화의 필요 조건으로 의료 시스템의 여섯 가지 구성 요소(building blocks), 즉 서비스 제공, 의료 인력, 의료 정보, 생산품·백신·기술, 의료 재정, 리더십/거버넌스를 제시하였다(Table 2) [11]. 이들 구성 요소는 의료 시스템이 필수적으로 갖추어야 할 여러 분야를 포괄하여 개선이 필요한 부분을 쉽게 파악할 수 있다. 따라서 이를 기준으로 삼아 공약을 분류한다면 주요 정당의 총선 의료 공약이 의료 시스템의 구성 요소를 고루 다루고 있는지 판단할 수 있을 것이다.

2) 의료 개선의 5중 목표

Berwick 등은 미국 학술원 의료연구소(Institute of Medicine, 현재의 National Academy of Medicine)가 2001년에 제시한 6대 의료 개선 목표인 안전, 효과, 환자 중심성, 적시성, 효율, 형평이[13] 독립적으로 추구된 탓에 의료의 발전이 파편화되었다고 주장하였다. 그리하여 그들은 기존의 6대 목표를 통합하고 건강 결과 향상을 최우선으로 하는 3중 목표(triple aim)를 제안하였는데[14], 이는 학계의 큰 지지를 받아 이후 의료 정책 분석에 광범위하게 인용되었다. 그 후 2014년에는 Bodenheimer와 Sinsky [15]가 의료진의 소진(burnout)을 3중 목표 달성의 주요 위협 요인으로 지적하면서 4중 목표(quadruple aim)을 제안하였고, 2022년에는 Nundy 등[16]이 건강 형평성을 추가한 5중 목표(quintuple aim)를 제창하였다(Table 3) [14-16]. 이러한 5중 목표는 환자의 진료 경험 개선, 인구 집단의 건강 수준 향상, 일인당 의료비 절감이라는 기존 3중 목표에 의료진의 안녕과 건강 형평성을 추가한 것으로서 최근까지의 논의를 반영한 의료 정책의 지향점으로 간주할 수 있다[16]. 2024년, Bréchat 등[17]은 건강 형평성을 건강 민주주의로 확장하고, 환경의 지속 가능성을 여섯 번째 목표로 추가한 ‘6중 목표(hexagonal aim)’를 제창하였으나 아직 학술적 검토가 충분하지 않고 한국의 여건상 적용이 시기상조라고 판단하여 이 연구의 검토 대상에서는 제외하였다. 요컨대 총선 의료 공약을 5중 목표를 기준으로 분류한다면 분류 결과를 바탕으로 각 정당 의료 정책의 지향점을 가늠할 수 있을 것으로 판단하였다.

3) 의료 개혁을 위한 조종 손잡이

오랫동안 각국의 의료 개혁 과정에 관여해 온 Roberts 등[18]은 ‘Getting Health Reform Right: A Guide to Improving Performance and Equity’에서 의료 정책 담당자가 의료 개혁에 활용할 수 있는 다섯 가지 조종 손잡이로 재정(의료 시스템의 구성 요소로서의 의료 재정과 구별) (Table 2), 지불, 조직, 규제, 행동을 제안하고 구체적인 사례를 제시하였다(Table 4) [18]. 다섯 가지 조종 손잡이는 Roberts 등[18]이 의료 개혁 자문에 응한 다년간의 경험을 바탕으로 의료 시스템 성과 개선을 이끌어 낼 수 있는 정부 개입의 방식으로 특정한 것이다. 즉, 이들 조종 손잡이는 오랜 경험을 통하여 검증된 대표적 의료 개혁 수단으로 간주할 수 있다. 따라서 조종 손잡이를 기준으로 의료 공약을 분류한다면 각 정당의 의료 개혁 수단을 가늠할 수 있을 것으로 판단하였다.

4) 공약 분류

먼저 내용 기준 공약 분류의 경우 제일 저자가 연구 대상 의료 공약을 읽고 추출한 각 공약의 키워드를 기준 삼아 공약을 분류하였다. 그런 다음 책임 저자가 제일 저자의 분류 결과를 검토하였고 검토 결과 두 저자의 의견이 다른 경우 토의를 하여 합의를 도출하였다. 의료 시스템의 구성 요소, 의료 개선의 5중 목표, 의료 개혁을 위한 조종 손잡이 등 이론적 기준에 따른 공약 분류도 같은 방식으로 하였다.

결과

연구 결과, 제22대 국회 보건복지 상임 위원 소속 정당의 의료 공약은 더불어민주당 12개(더불어민주연합 4개 포함), 국민의힘 16개(국민의미래 5개 포함), 개혁신당 3개, 조국혁신당 2개 등 총 33개였다(Table 5) [9]. 이때 국민의힘의 공약 중 ‘간병인 등록 및 자격 관리제 및 간병 비용 연말 정산 세액 공제’는 성격이 다른 공약이 결합되어 있다고 보고, ‘간병인 등록 및 자격 관리제’와 ‘간병 비용 연말 정산 세액 공제’라는 2개의 공약으로 분리하였다. 공약 내용 기준 키워드 빈도는 의료 인력(7개), 지역 의료(7개), 간병(5개) 순이었다. 자세한 분류 결과는 부록으로 수록하였다(Data Set 1).

1. 의료 시스템의 구성 요소에 따른 공약 분류 결과

WHO 의료 시스템의 구성 요소를 기준으로 공약을 분류한 결과는 Figure 1과 같다. 분류 결과, 서비스 제공 공약이 17개로 가장 많았는데, 여기에는 간병비 건강 보험 적용 등이 포함되었다. 의료 인력 공약(9개)으로 지역 의사제, 공공 의대 및 지역 의대 신설 등이 있었고, 리더십/거버넌스 공약(5개)에는 필수 의료 분야 의료 사고에 대한 국가 책임 강화 등이 포함되었으며, 생산품·백신·기술 공약(2개)으로 특정 질환에 대한 첨단 로봇 수술 건강 보험 급여화 등이 있었다. 그러나 의료 정보나 재정 공약은 없었다. 자세한 분류 결과는 부록으로 수록하였다(Data Set 1).

2. 의료 개선의 5중 목표에 따른 공약 분류 결과

Figure 2는 5중 목표에 따라 공약을 분류한 결과다. 분류 결과, 환자 진료 경험 개선 공약이 18개로 가장 많았는데, 여기에는 간병인 양성 체계 마련 등이 포함되었다. 그다음이 건강 형평성 공약(9개)으로 지역 의료 격차 해소 특별법 제정 등이 있었고, 의료진의 안녕 증진 공약(4개)에는 전공의 수련 환경 및 간호 인력 처우 획기적 개선 등이 포함되었으며, 인구 집단 건강 향상 공약(2개)으로는 군 장병의 정신 건강 증진 서비스 체계 확대 등이 있었다. 그러나 일인당 의료비 절감 공약은 없었다. 자세한 분류 결과는 부록으로 수록하였다(Data Set 1).

3. 의료 개혁을 위한 조종 손잡이에 따른 공약 분류 결과

조종 손잡이를 기준으로 공약을 분류한 결과는 Figure 3과 같다. 분류 결과, 규제 공약이 14개로 가장 많았는데 여기에는 합리적 의료인 증원 계획 마련 등이 포함되었다. 11개 재정 공약에 간병 비용 연말 정산 세액 공제 등이 있었고, 7개 조직 공약에 소아 전문 응급 의료 센터 전국 확대 등이 포함되었다. 지역에서도 마음 놓고 출산할 수 있도록 하는 환경 마련과 양질의 지역 사회 의료 인프라 확충은 다섯 가지 조종 손잡이 중 어느 하나로 보기 어려워 기타로 분류하였다. 행동 및 지불 공약은 없었다. 자세한 분류 결과는 부록으로 수록하였다(Data Set 1).

고찰

이 연구는 제22대 국회 보건복지 위원회 배정 국회의원의 소속 정당이 제시한 총선 의료 공약을 살펴보았다. 그 결과, 이번 총선 의료 공약 중 키워드가 의료 인력과 지역 의료인 공약이 각각 7개로 가장 많았고 간병(5개) 공약이 뒤를 이었다. 이는 최근 몇 년간 제기된 의사 인력 확충[19]과 지역 의료 격차 해소[20], 간병 부담 완화[21] 등에 대한 사회적 요구를 각 정당이 공약에 반영한 결과로 보인다. 다만 공약의 구체성이나 적절성은 별도로 따져볼 필요가 있다.

의료 공약은 개수와 내용 면에서 정당 간에 차이가 있었으나 유사점도 발견되었다(Table 1). 공약 개수 면에서는 원내 1당인 더불어민주당(12개)과 원내 2당인 국민의힘(16개)이 제시한 공약이 대부분을 차지하였다. 그러나 두 정당이 총선 후 각자의 비례 위성 정당과 통합하여 공약 개수가 늘어났다는 점을 고려해야 한다. 공약 내용 면에서 국민의힘은 공약 대상 집단을 특정(예: 노인, 경찰 공무원)하는 경향이 있었고 더불어민주당은 사회 구성원 전반을 대상으로 한 공약이 부각되었다. 두 정당 모두 요양 병원 간병비 급여화 등 간병 관련 공약을 제시하였고 지역 의대 신설과 지역 의료 강화 등도 약속하였다. 과거 양당은 여야로 나뉘어 정치적으로 대립해 왔으나 실상 보수 정당[22], 실용 정당[23]이라는 유사한 정체성을 지니고 있다고 평가되어 왔다. 그러나 박근혜 정부 이후 양당은 보수/진보라는 양극을 형성하면서 새로운 정치적 대립이 심화되고 있다는 평가도 있다[24,25]. 사정이 이러하므로 양당 의료 공약에 유사점과 차이점이 공존한다는 점을 설명하기 위해서는 추가적인 연구가 필요하다.

다음으로 이 연구는 의료 시스템의 구성 요소, 5중 목표, 조종 손잡이 등 세 가지 이론적 기준으로 공약을 분류하였다. 먼저, WHO 의료 시스템 구성 요소에 따라 공약을 분류한 결과, 절반 이상(17개, 52%)이 서비스 제공 영역에 집중되어 있었고 대표적인 공약은 특정 의료 서비스에 대한 건강 보험 적용 또는 급여화였다. 이는 정당이 선거에서 승리하기 위하여 유권자의 선호를 공약에 반영한 결과로 볼 수 있다[26]. 다만 이러한 공약이 부정적인 의미의 ‘포퓰리즘’에 빠지지 않도록 실현 가능성이나 재정 대책 등 공약의 건전성 여부나 정도를 체계적으로 판단하는 절차가 필요하다[26]. 의료 전문가나 전문가 단체는 바로 이 지점에서 의미 있는 기여를 할 수 있다.

한편, 의료 시스템의 구성 요소 중 재정에 상응하는 공약의 부재는 5중 목표에 따른 공약 분류 결과와 일치한다. 즉, 5중 목표 중 일인당 의료비 절감이라는 목표에 부합하는 공약 역시 찾을 수 없었다. 이는 그간 국민건강보험 재정 관리의 중요성이 일부 강조되어 왔으나[27,28] 경상 의료비 자체는 경제협력개발기구(Organisation for Economic Co-operation and Development, OECD) 회원국 평균에 미치지 못하였다는 점과 관계가 있어 보인다. 그러나 한국의 경상 의료비는 2022년, 국내 총생산 대비 9.4%에 이름으로써 OECD 회원국 평균(9.2%)을 처음으로 넘어섰고 2023년 잠정치도 9.9%로 나타나[29], 앞으로 몇 년 내에 경상 의료비 규모가 국내 총생산 대비 두 자릿수에 달할 것으로 전망된다. 나아가 오는 2033년이면 경상 의료비 규모가 국내 총생산의 12.1-15.9%에 이를 것이라는 예측도 있다[30]. 이와 같은 상황은 의료비 절감에 대한 정부와 언론의 관심과 압력이 전례 없이 커질 수 있음을 시사한다. 이때 필요한 것은, 의료비 절감에 대한 일방적 강조보다는 의료비 적정 관리와 지출 구조 개선이라는 균형 잡힌 시각이다. 이러한 시각을 적절한 재정 정책으로 구체화하고 그 실행을 보장하는 정치적 책임을 부과하려면 정책의 공약화가 요구된다[31].

조종 손잡이에 따른 공약 분류 결과, 규제와 재정, 조직 공약이 대부분을 차지하였고 그중 규제 공약이 14개로 가장 많았다. 그러나 행동 및 지불 공약은 전무하였다(Figure 3). 규제는 대표적인 강제(hard) 정책 수단으로 법률 등 규정 제정과 그 집행을 통하여 정책 목표를 달성하는 것이 보통이다. 이는 법적 강제력이 보장되는 경우 실천력이 크고 명확한 결과를 얻을 수 있으나 이해관계자의 저항을 야기하거나 집행 비용이 많이 들 수 있다[32]. 의료 분야에는 자율성을 특징으로 하는 여러 의료 전문가가 관여하므로 의료 공약이 규제 같은 강제 정책 수단 위주로 구성되는 것은 문제일 수 있다.

이 연구의 제한점은 다음과 같다. 첫째, 공약 선별의 내용 기준으로서의 ‘의료’ 범위에 대하여 논란이 있을 수 있다. 이 연구에서의 의료는 질병 진단과 치료 중심의 의료, 즉 좁은 의미의 의료로 정의하였다. 그러나 넓은 의미의 의료는 진단과 치료뿐 아니라 건강 증진에서 예방, 재활, 호스피스 및 완화 의료에 이르기까지 그 범위가 대단히 넓다[10]. 또 고령화 추세에 따라 복지 서비스와 통합되는 의료 서비스 요소도 있다. 문재인 정부 때부터 강조되기 시작한 지역 돌봄(community care)이 대표적인데[33], 2026년부터는 관련법이 시행될 예정이기도 하다[34]. 다만 이 연구에서는 넓은 의미의 의료, 복지 서비스와 통합되는 의료의 범위와 그 안에서의 의료인의 역할에 관해서는 아직 공감대가 부족하다는 현실을 고려하였다. 둘째, 일부 공약의 경우 구체성이 부족하여 분류 기준을 적용할 때 하나의 공약을 여러 범주로 분류할 수 있는 경우가 있는가 하면, 어느 한 범주로도 분류하기 어려운 경우도 있었다. 예를 들어 ‘특정 질환에 대한 첨단 로봇 수술 건강 보험 급여화’라는 공약을 의료 시스템의 구성 요소에 따라 분류하는 경우, 보험 급여 확대라는 관점에서는 ‘서비스 제공’ 공약으로, 기술 발전 촉진이라는 관점에서는 ‘생산품·백신·기술’ 공약으로 분류할 수 있다. 다만 이 연구에서는 생산품·백신·기술 공약으로 분류하였다. 또 조종 손잡이에 따라 지역 사회 의료 인프라 확충 공약을 분류할 때는 다섯 가지 조종 손잡이 중 어느 하나로 분류하기 어려워 ‘기타’로 분류하였다. 이는 향후 연구에서 ‘모음집’ 외에 각 당의 정책 공약집을 대조함으로써 보완할 수 있을 것이다. 다만 이 연구가 첫 번째 의료 공약 분류 작업으로서 탐색적 성격이 강하다는 점, 각 당의 역량에 따라 정책 공약집의 구체성에 차이가 있다는 점을 고려할 필요가 있다.

실현 가능성 면에서 선거 공약이 가치가 있는가에 관해서는 논란이 있다[35,36]. 이는 의료 공약도 예외가 아니다. 그러나 선거는 대의 민주정의 핵심 구성 요소이고 공약은 유권자가 선거 후보자를 선택할 때 중요한 고려 사항이다[37]. 이 점에 비추어 볼 때 의료 공약에 대한 연구 노력은 조금 더 격려될 필요가 있다. 무엇보다 이 연구는 의료 시스템의 구성 요소, 의료 개선의 5중 목표, 의료 개혁을 위한 조종 손잡이를 이용하여 의료 공약을 분류함으로써 한국 주요 정당의 최근 의료 정책을 개관하였다는 점에서 검토의 가치가 있을 것이다.

Five control knobs for health sector reform

Control knob	Description
Financing	Determines how much money is available, who bears the financial burden, who controls the funds, how risks are pooled, and whether healthcare costs can be controlled. It also considers the question of how to allocate the funds that the financing system generates.
Payment	Discusses which methods should be selected to pay physicians, hospitals, and other healthcare providers. It also examines the incentives that payment schemes create for both buyers and sellers and how these incentives can be adjusted to further the goals of health-sector reform.
Organization	Considers how government can influence the organization of the healthcare delivery system, in 3 kinds of interventions: changes in who-does-what, changes in incentives, and managerial changes. It refers to the structure of the health system and the individual institutions that provide health services.
Regulation	The use of coercive power of the state to change the behavior of individuals and organizations in the health sector. Only includes the full range of legal instruments issued by government or non-governmental bodies (which the government has delegated regulatory power).
Behavior	Involves population-based interventions such as the design, implementation, and evaluation of programs intended to change individual behavior. Its main concern is how behavior can be used to improve health system performance and promote public health goals.

이 연구는 22대 국회 보건복지위원회 소속 정당의 의료 공약을 분석한 결과, 지역 보건 서비스와 의료 인력 관련 공약이 가장 많았음을 확인했다. 주요 초점은 서비스 제공과 환자 경험 개선이었으며, 규제가 가장 자주 사용된 개혁 도구였다. 공약은 의료 인력 부족, 지역 의료 불평등, 간병 부담 완화에 대한 사회적 요구를 반영하고 있으며, 이를 해결하기 위한 균형 잡힌 자금 조달과 다양한 정책 개혁 도구의 필요성을 제안한다. 저자들은 이러한 국민적 요구를 기반으로 정부, 사회, 그리고 의료계가 대응할 수 있는 근거 자료를 제공하고 있다.
