Abstract

Purpose: Metastatic bladder cancer has historically been treated with platinum-based chemotherapy as the standard first-line therapy. Since the introduction of cisplatin-based regimens, advancements in treatment strategies have been limited. However, the recent emergence of immune checkpoint inhibitors (ICIs) and antibody-drug conjugates (ADCs) has significantly transformed the treatment landscape. Notably, the combination of enfortumab vedotin (EV) and pembrolizumab has demonstrated considerable clinical benefits, challenging traditional chemotherapy. This review aims to provide an overview of recent advancements in the treatment of metastatic bladder cancer.
Current Concepts: Recent phase III clinical trials, notably EV-302/KEYNOTE-A39 and CheckMate 901, have reshaped the first-line management. EV combined with pembrolizumab has shown superior progression-free survival and overall survival compared to platinum-based chemotherapy, establishing it as a new standard of care. Patients ineligible for this regimen due to underlying comorbidities or toxicity may still benefit from alternative options. Such alternatives include platinum-based chemotherapy with ICI maintenance therapy, including avelumab or nivolumab. The selection of treatments should be individualized, taking into account specific patient factors, particularly platinum eligibility, renal function, and performance status.
Discussion and Conclusion: The incorporation of ICIs and ADCs into the therapeutic landscape for metastatic bladder cancer has significantly improved patient outcomes. EV plus pembrolizumab has demonstrated substantial survival benefits, establishing it as a preferred first-line regimen for eligible patients. However, not all patients are candidates for this combination, emphasizing the necessity of individualized treatment strategies. Future research should focus on managing treatment-related adverse events and developing personalized therapies to maximizing efficacy while minimizing toxicity.

서론

전이성 방광암의 치료는 20여 년 전 발표된 임상시험에서 시스플라틴(cisplatin) 기반 화학요법이 생존율 향상을 입증한 이후, 1차 치료의 표준으로 자리 잡았으며, 이후 오랫동안 큰 변화가 없었다[1–3]. 이러한 오랜 치료 패러다임은 최근 몇 년간 면역요법 및 면역관문억제제의 등장으로 변화의 도전을 받았으며[4], 2023년 10월 발표된 2개의 핵심적인 무작위 임상시험(EV-302/KEYNOTE A39 and Checkmate 901)의 결과로 완전히 바뀌었다[5,6]. 새롭게 떠오른 약물은 엔포투맙 베도틴(enfortumab vedotin, EV)과 펨브롤리주맙(pembrolizumab)이다. 이 논문에서는 전이성 방광암 치료에 있어 최근의 주요 변화들을 소개하고, 임상 현장에서의 적용 가능성에 중점을 두고자 한다. 이 연구는 사람을 대상으로 한 연구가 아니므로, 기관윤리위원회의 승인이나 연구 참여자의 동의는 필요하지 않다.

환자 상태 별 전이성 방광암의 초기 치료 접근법

1차 치료에 있어 전이성 방광암의 환자는 크게 2가지 범주로 나눌 수 있다. 병용요법이 가능한 환자와 병용요법이 불가능한 환자이다. 이는 앞서 언급한 두 가지 연구를 기반으로 분류되었다. 병용요법이 가능한 환자는 다시 백금 기반 항암제의 사용 가능 여부에 따라 세분화된다.

병용요법이 가능한 환자군은 (1) Eastern Cooperative Oncology Group 수행능력 상태(ECOG performance status, ECOG PS) 0–2점, (2) 사구체여과율(glomerular filtration rate, GFR) ≥30 mL/min, (3) EV+펨브롤리주맙에 적합한 장기 기능을 보유한 경우이다. 상기 조건을 모두 만족한다면 EV+펨브롤리주맙을 사용하며, 적합하지 않다면 백금 기반 항암제/젬시타빈(platinum/gemcitabine)+유지요법 아벨루맙(avelumab) 또는 시스플라틴/젬시타빈+니볼루맙(nivolumab)을 사용한다. 면역관문억제제의 투약이 제한된다면 platinum/젬시타빈을 단독으로 투약할 수 있다. 병용요법에 적합한 기준을 갖지 못한 환자군들은 programmed death-ligand 1 (PD-L1) positive라면 아테졸리주맙(atezolizumab), 펨브롤리주맙을 사용해 볼 수 있다(Table 1).

EV+펨브롤리주맙 병용요법

EV+펨브롤리주맙의 병용요법은 병용치료가 가능한 환자에게 새로운 표준 치료법으로 자리 잡았다. EV는 항체-약물 접합체(antibody-drug conjugate, ADC)로, nectin-4에 특이적으로 결합하는 항체와 세포 독성 약물인 monomethyl auristatin E (MMAE)가 결합한 형태이다. Nectin-4는 요로 상피암을 포함한 다양한 암세포에서 높은 발현율을 보이는 표면 단백질로, EV가 종양세포에 결합하면 내포작용(endocytosis)으로 MMAE가 세포 내로 유입되고 세포 사멸을 유도한다[7].

펨브롤리주맙은 면역관문억제제로 programmed cell death protein 1 (PD-1)과 결합하여 PD-1/PD-L1의 상호작용을 차단한다. 이로써 일부 암세포가 PD-L1을 과발현하여 T세포의 면역 감시를 회피하는 기전을 차단하여 T세포가 암세포를 효과적으로 인식하고 공격할 수 있는 면역반응을 회복시킨다.

2023년에 발표된 연구는 수술적 치료가 불가능 하거나 전이성 방광암 환자의 1차 치료로 EV+펨브롤리주맙과 platinum/젬시타빈을 비교하였다. EV+펨브롤리주맙은 무진행 생존율(progression-free survival, PFS)과 전체 생존율(overall survival, OS)에서 유의미한 우위를 보였다(median PFS of 12.5 대 6.3 months (위험비, 0.45; 95% 신뢰구간, 0.38–0.54), median OS of 31.5 대 16.1 months (위험비, 0.47; 95% 신뢰구간, 0.38–0.58). 객관적 반응률(objective response rate, ORR)에 있어서도 EV+펨브롤리주맙은 67.7% (완전 관해율, 29.1%), platinum+젬시타빈은 44.4% (완전 관해율, 12.5%)로 통계적으로 유의한 차이(P<0.00001)를 보였다[5].

치료 관련 이상반응에 있어서 3등급 이상의 치료 관련 이상반응은 EV+펨브롤리주맙에서 55.9%, platinum/젬시타빈군에서는 69.5% 확인되었다. EV+펨브롤리주맙에서 흔한 부작용은 반구진성 발진(7.7%), 고혈당(5.0%), 그리고 호중구 감소증(4.8%)이었으며, 3등급 이상으로 보고된 이상반응은 피부 발진, 말초신경병증, 고혈당이었다[8]. 이러한 연구결과를 바탕으로 EV+펨브롤리주맙은 전이성 방광암에서 1차 치료로 자리잡았다.

백금 기반 항암제 적합성 여부의 정의

방광암 전문가들의 설문조사를 바탕으로 시스플라틴 투약이 적합한 환자군, carboplatin 투약이 적합한 환자군, 백금 기반 항암제 투약이 어려운 환자군의 기준을 정하였다[9]. 시스플라틴 투약이 적합하지 않다고 분류되는 환자들은 다음 기준 중 최소 한 개 이상을 충족한다. (1) P>1, (2) GFR≤60 mL/min, (3) 2등급 이상 청각 손실, (4) 2등급 이상 말초신경병증, (5) 뉴욕심장학회(New York Heart Association) III급 심부전이다(Table 2) [10].

전이성 방광암 환자의 50% 정도는 상기 기준 중 하나 이상을 충족하여 시스플라틴 투약에 적합하지 않다[11]. 이중 신기능이 가장 중요하며, 결과가 불확실한 경우 방사성 동위원소(⁹⁹mTc-DTPA 또는 ⁵¹Cr-EDTA)를 이용한 GFR 측정을 권장한다.

시스플라틴 투여가 어려운 환자들은 대체 요법으로 carboplatin을 고려할 수 있다. 그러나 환자가 (1) PS>2, (2) GFR<30 mL/min, (3) PS 2면서 GFR<60 mL/min인 경우에는 백금 기반 항암제 치료 여부와 상관없이 예후가 좋지 않기 때문에 백금 기반 항암제 치료에 적합하지 않은 것으로 판단한다[12].

병용요법이 가능하지만 EV+펨브롤리주맙 투약이 불가능한 환자

EV+펨브롤리주맙의 우위성이 확인되었지만 조절되지 않는 당뇨병이 있거나 2등급 이상의 말초신경병증, 중증 피부질환이 있는 환자들은 치료 대상에서 제외된다. 또한 EV+펨브롤리주맙의 부작용에 대한 걱정으로 환자가 치료를 거부할 수 있다. 이러한 환자에서 백금 기반 항암제와 면역관문억제제를 병용하는 것이 선호되는 치료 방법이다.

시스플라틴은 1980년대 후반부터 전이성 방광암의 표준 치료법으로 자리를 잡았으며, 다양한 연구에서 12–14개월의 생존율을 보인다[13]. 대표적인 요법은 메토트렉세이트(methotrexate), 빈블라스틴(vinblastine), adriamycin, 시스플라틴(MVAC)과 젬시타빈/시스플라틴으로 각각 14.8개월, 13.8개월의 생존율을 보고하였다[14]. 치료에 대한 반응률은 MVAC이 46%, 젬시타빈/시스플라틴이 49%로 보고되었지만 젬시타빈/시스플라틴이 MVAC에 비해 낮은 독성을 보여 표준요법이 되었다[1]. 과립구집락자극인자(granulocyte-colony stimulating factor)를 병용하는 dose-dense MVAC의 경우 MVAC과 비교하여 독성이 낮고 완전 관해율과 2년 생존율에서 이점을 보였으나 생존율의 차이를 보이지는 않았다[15].

백금 기반 항암제를 이용한 치료로 최소한 질병이 안정적으로 유지되는 환자를 대상으로 면역항암제를 이용한 유지 치료를 진행한다. 펨브롤리주맙을 이용한 유지 치료에 대한 무작위 2상 연구에서, 펨브롤리주맙 유지 치료군에서 PFS가 5.4개월로, 대조군의 3.0개월보다 유의하게 증가(위험비, 0.65; P=0.04)하였다[16].

JAVELIN Bladder 100 연구에서는 PD-L1 억제제인 아벨루맙을 이용한 유지 치료의 효과를 평가하였다. Platinum/젬시타빈 화학요법(4–6주기) 후 최소한 안정병변 이상을 보인 환자를 대상으로, 아벨루맙 유지 치료군과 최선의 보존 치료(best supportive care, BSC)군으로 무작위 배정하였다. OS가 아벨루맙군에서 21.4개월로, BSC군의 14.3개월보다 유의하게 개선(위험비, 0.69; 95% 신뢰구간, 0.56–0.86; P<0.001)되었다[17]. 이 연구결과를 바탕으로, 아벨루맙 유지 치료는 최근까지 백금 기반 항암제 치료 후 질병이 안정적으로 유지된 모든 환자에서 표준 치료법으로 자리 잡았다.

또한 CheckMate 901 연구는 니볼루맙의 효과에 대한 평가를 제시하였다[6]. 이 연구에서는 니볼루맙+젬시타빈/시스플라틴의 병용요법을 평가하였으며, 최대 24개월까지 니볼루맙 유지 치료를 시행한 군과 젬시타빈/시스플라틴 단독 치료군을 비교하였다. 연구결과, PFS와 OS 모두 유의하게 개선되었으며, 각각 7.9개월 대 7.6개월(위험비, 0.72; 95% 신뢰구간, 0.59–0.88), 21.7개월 대 18.9개월(위험비, 0.78; 95% 신뢰구간, 0.63–0.96)로 보고되었다. 또한, 니볼루맙+젬시타빈/시스플라틴 병용군에서 ORR이 57.6%로 젬시타빈/시스플라틴 단독군의 43.1%보다 개선되었으며, 완전 관해율은 21.7%로, 반응 지속 기간은 37.1개월이었다. 약물 독성에 있어서 니볼루맙+젬시타빈/시스플라틴 병용군에서 3등급 이상의 치료 관련 독성 발생률이 62%로, 젬시타빈/시스플라틴 단독군(52%)보다 높게 보고되었다. 이 연구결과를 바탕으로, 니볼루맙+젬시타빈/시스플라틴 병용요법은 EV+펨브롤리주맙 치료가 불가능하거나 사용이 제한된 환자에서 젬시타빈/시스플라틴+아벨루맙 유지 치료의 대안이 될 수 있다.

병용요법이 적합하지 않은 환자의 치료

이러한 환자군(PS>2 또는 GFR<30 mL/min, 장기 기능 부전)의 최적 치료법에 대한 자료는 제한적이다. 과거에는 이 환자군의 예후가 좋지 않았으며, 전신 치료보다는 최선의 지지요법이 선택되는 경우가 많았다. 백금 기반 항암요법의 대안 치료를 평가한 대부분의 연구가 이 환자군에 초점을 맞추지 않아, 연구결과를 해석하는 데 어려움이 있다.

미국에서는 단일 실험군 2상 임상시험 결과를 바탕으로 PD-L1 유무와 상관없이 백금기반 화학요법을 받을 수 없는 환자에서 1차 치료제로 펨브롤리주맙을 승인하였다[18]. 유럽에서는 시스플라틴 사용이 불가능한 PD-L1 양성 환자에서 펨브롤리주맙과 아테졸리주맙을 1차 치료제로 승인하였다[19]. 펨브롤리주맙은 시스플라틴을 사용할 수 없는 진행성 또는 전이성 방광암 환자 370명을 대상으로 평가되었으며, ORR은 29%, 완전 관해율은 7%로 나타났다.

전이성 방광암의 후속 치료

앞서 이야기한 EV-302/KEYNOTE A39 임상시험의 결과로 EV+펨브롤리주맙 병용요법이 1차 치료로 자리잡았다. 그러나 치료에도 질병이 진행한 환자에 대해서는 아직 합의가 이루어진 치료법은 없으며, 후속 치료에 대해서는 많은 연구가 진행 중이다. 현재는 여러가지 치료 옵션이 존재한다(Table 3).

화학요법

EV+펨브롤리주맙 병용요법 후 platinum/젬시타빈 화학요법을 고려할 수 있다. 하지만 EV+펨브롤리주맙을 사용 후 환자의 임상 상태에 대한 자료는 아직 제한적이며, 치료에 대한 독성을 고려하여 치료 계획을 수립해야 한다. 1차 치료로 백금 기반 화학요법을 사용했다면, 치료 후 6–12개월 동안 질병이 진행하지 않았다면 백금 기반 화학요법을 재도전하는 것이 가능하다. Retrospective International Study of Cancers of the Urothelium 코호트에서 296명의 환자를 대상으로 수행된 후향적 분석 결과, 후속 백금 기반 병용 화학요법이 OS를 유의하게 개선하였다(OS, 7.9 대 5.5 months; P=0.035)[20]. 후속 치료로 단일 치료 약물로는 파크리탁셀(paclitaxel), 도세타셀(docetaxel), 젬시타빈, 나브-파크리탁셀(nab-paclitaxel), 옥살리플라틴(oxaliplatin), 이포스파미드(ifosfamide), 토포테칸(topotecan), 페메트렉시드(pemetrexed), 라파티닙(lapatinib), 게피티닙(gefitinib), 보르테조밉(bortezomib)을 사용해볼 수 있으나 반응률은 0–28%로 보고되며 아직 2상 연구 단계에 머물러 있다[21].

이전 면역요법 경험이 없는 백금 기반 화학요법 환자를 위한 면역요법

펨브롤리주맙, 니볼루맙, 아테졸리주맙, 아벨루맙, 두르발루맙(durvalumab)은 1차 백금 기반 화학요법 중 또는 이후에 질병이 진행된 환자들을 대상으로 시행한 임상시험에서 유사한 효과와 안전성을 보였다. 542명을 대상으로 펨브롤리주맙 단독 치료군과 화학요법(파크리탁셀, 도세타셀, 빈플루닌[vinflunine])으로 무작위 배정한 3상 연구에서 OS를 유의미하게 개선하였다(median OS of 10.3 months; 95% 신뢰구간, 8.0–11.8 대 7.4 months; 95% 신뢰구간, 6.1–8.3; 위험비, 0.73; 95% 신뢰구간, 0.59–0.91; P=0.002). 이는 PD-L1 발현 여부와 관계없이 나타났다[22]. 니볼루맙은 단일 2상 임상시험(CheckMate 275) 기반으로 식품의약국 승인을 받았으며, 이 연구에서는 백금 기반 항암 치료의 경험이 있는 270명의 환자를 평가하였다. ORR은 19.6%, 전체 생존율은 8.74 개월로 보고되었다[23].

이전 면역요법 및 백금 기반 화학요법을 받은 환자에서 EV 단독요법

EV-201 연구는 125명의 환자를 대상으로 진행한 2상 단일군 연구로, 백금 기반 화학요법과 면역관문억제제 치료를 받은 환자에서 후속 치료를 진행하였다[24]. ORR은 44%를 보였고, 완전 반응률은 12%였다. 이러한 데이터를 기반으로 EV는 PD-1 또는 PD-L1 억제제 및 백금 기반 화학요법을 받은 전이성 방광암 환자와, 시스플라틴 투여가 어려우며 이전 한가지 이상의 치료를 받은 환자를 대상으로 미국과 유럽에서 신속 승인을 받았다. 또한 EV 단독 추가 치료와 단일 화학요법을 비교한 3상 무작위 대조군 연구에서도 EV는 전체 생존율을 4개월 연장시키는 효과를 보였다(12.88 months 대 8.97 months; 위험비 0.7; 95% 신뢰구간, 0.56–0.89) [25].

Fibroblast Growth Factor Receptor 억제제

요로 상피암의 유전체 분석 결과에서 fibroblast growth factor receptor (FGFR) 변이가 치료 가능성이 있는 유전자 변이로 밝혀졌다[26]. 에르다피티닙(erdafitinib)은 pan-FGFR 티로신(tyrosine) 카이나제억제제로 백금 기반 화학요법 후 FGFR 변이를 보이는 전이성 방광암 환자를 대상으로 식품의약국 승인을 받은 최초 표적 치료제다. 최근 THOR cohort 연구에서 FGFR 변이가 있는 환자를 대상으로 1차 치료 후 질병이 진행된 환자에서 에르다피티닙과 화학요법(도세타셀 또는 빈플루닌)을 비교하였다. 연구결과 에르다피티닙 환자군이 전체 생존율과 무진행 생존기간이 유의미하게 향상되었다(OS, 12.1 months vs. 7.8 months; 위험비, 0.64; 95% 신뢰구간, 0.47–0.88), median PFS 5.6 대 2.7 months; 위험비, 0.58; 95% 신뢰구간, 0.44–0.78) [27]. 두 그룹 간의 3등급 이상 치료 독성 발생률은 유사하였고, 에르다피티닙의 가장 흔한 이상 부작용은 수족홍반증후군(9.6%), 구내염(8.1%), 조갑박리증(5.9%), 고인산혈증(5.2%) 등이었다. 에르다피티닙 이외에도 여러 FGFR 억제제들(인피그라티닙[infigratinib], 로가라티닙[rogaratinib])이 연구 중이다.

결론

최근 연구결과를 통해 수년간 큰 변화가 없었던 전이성 방광암의 치료 전략이 획기적으로 변화하였다. 오랫동안 표준 치료로 자리 잡았던 시스플라틴 기반 요법은 면역관문억제제와 ADC의 도입으로 새로운 치료법과 경쟁하게 되었다. EV+펨브롤리주맙 병용요법은 PFS와 OS에서 의미 있는 향상을 보여 새로운 표준 치료로 자리 잡았다. 그러나 모든 환자가 EV+펨브롤리주맙을 사용할 수 있는 것은 아니며, 백금 기반 항암제 적합성 여부, 면역관문억제제 병용 가능 여부에 따라 맞춤형 치료 접근이 필요하다. 앞으로도 전이성 방광암 치료의 효과를 극대화하고 부작용을 최소화하기 위한 연구가 지속될 것으로 기대된다.

최근 새롭게 각광받고 있는 방광암에 대한 약물치료법을 각각의 주어진 상황에 맞춰서 소개하고 있다. 즉 병용요법이 가능한 경우는 비교적 새로운 약제인 베도틴(enfortumab vedotin, EV)과 펨브롤리주맙(pembrolizumab)을 사용하며 적합하지 않은 경우는 백금 기반 항암제/젬시타빈+유지요법 아벨루맙(avelumab) 또는 cisplatin/gemcitabine+니볼루맙(nivolumab)을 사용한다. 이 논문에서 베도틴(enfortumab vedotin, EV)과 펨브롤리주맙(pembrolizumab)이 전이성 방광암에 대한 1차적 치료법으로 자리잡은 배경을 설명하고 있으며 이러한 치료법이 적절하지 않은 경우에 대해 고려할 수 있는 치료제들을 소개하고 있다. 결국은 모든 환자들에게 같은 치료제를 고려할 수 없고 백금 기반 항암제 적합성 여부와 면역 관문 억제제 병용 가능 여부에 따라 맞춤형 치료 접근이 필요하다는 것을 언급하고 있다.
