Trans Abstract

Purpose

The advent of targeted therapies has revolutionized the treatment landscape of atopic dermatitis (AD), particularly through the introduction of biologics and small-molecule agents. This review summarizes current evidence regarding the efficacy, safety, and clinical applications of biologics such as dupilumab, tralokinumab, lebrikizumab, and nemolizumab, as well as small-molecule inhibitors including Janus kinase (JAK) inhibitors, phosphodiesterase-4 (PDE4) inhibitors, and aryl hydrocarbon receptor (AhR) modulators.

Current concepts

AD is a chronic, relapsing inflammatory skin disorder characterized by a complex interplay between skin barrier dysfunction and immune dysregulation. Biologic therapies have targeted key cytokines involved in AD pathogenesis: dupilumab inhibits interleukin (IL)-4 and IL-13; tralokinumab and lebrikizumab selectively target IL-13; and nemolizumab blocks IL-31. In addition, small-molecule JAK inhibitors such as upadacitinib, baricitinib, and abrocitinib are now used for moderate-to-severe AD. Topical JAK inhibitors (e.g., ruxolitinib, delgocitinib), PDE4 inhibitors, and AhR modulators have also shown promise in clinical trials, offering new therapeutic options. Clinical studies have consistently demonstrated marked improvements in disease severity, pruritus, and quality of life, with generally acceptable safety profiles. Nonetheless, questions remain regarding long-term safety, patient selection, cost-effectiveness, and optimal sequencing of these therapies.

Discussion and conclusion

Targeted biologics and small-molecule therapies have significantly improved quality of life and long-term disease control for patients with AD. Ongoing and future studies are essential to refine personalized treatment approaches based on individual disease phenotypes, biomarkers, and patient-specific factors.

서론

배경

아토피피부염(atopic dermatitis)은 비교적 흔한 만성 염증성 피부질환으로 소아의 15–30%, 성인의 2–10%에서 발생한다. 아토피피부염은 유전적인 소인과 피부장벽이상, 면역학적 요인, 환경적 요인 등이 복합적으로 작용하여 발생한다. 경증의 아토피피부염은 국소 스테로이드제나 국소 타크로리무스, 국소 피메크로리무스 도포제에 비교적 반응이 좋다. 중등증 내지 중증의 아토피피부염의 경우에는 전신치료로 사이클로스포린(cyclosporine)이나 메토트렉세이트(methotrexate), 아자싸이오프린(azathioprine), mycophenolate mofetil, interferon-gamma, 광선치료, 항원특이면역치료 등을 시도할 수 있으나, 반응이 제한적이고 장기간 사용시 부작용이 우려되며, 임상시험 자료가 적어 소아나 성인에서 객관적인 근거수준이 낮다. 아토피피부염은 임상적으로 다양한 양상을 보이며 만성적으로 진행하는 경과로 인해 질환을 장기적으로 조절하고 삶의 질을 개선하기 위해 보다 안전하고 효과적인 치료가 필요하다. 기존의 일반적인 치료에 반응하지 않는 난치성 아토피피부염 환자의 경우에는 사회, 경제적, 건강상의 부담이 심각해질 수 있다.

최근 아토피피부염의 분자면역학적 발생기전에 대한 연구가 활발히 진척되면서, 면역학적 표적에 특이적으로 작용하는 다양한 생물학제제(biologics)와 소분자 물질 억제제(small-molecule inhibitor)들이 개발되어 국제 임상시험들을 거쳐 시판되고 있다. 새로운 표적치료제들은 기존의 고식적인 치료에 반응 불응성의 아토피피부염에서 T helper (Th) 2와 이와 연관된 신호전달경로를 선택적으로 차단하여 우수한 치료효과를 증명하였다. 생물학제제로는 인터루킨(interleukin, IL)-4와 IL-13을 표적으로 하는 dupilumab과 IL-13을 선택적으로 억제하는 tralokinumab과 lebrikizumab이 type 2 사이토카인(cytokine)들을 억제하여 중등증 내지 중증의 아토피피부염에서 좋은 효과를 보이고 있다[1]. 이외에도 아직 우리나라에는 시판되고 있지 않지만 IL-31 수용체를 억제하여 아토피피부염의 가려움증 개선효과가 뛰어난 nemolizumab이 있다. 면역학적 경로를 선택적으로 차단하는 소분자 물질 억제제로는 현재 Janus kinase (JAK) inhibitor 중 경구 약제로 baricitinib, upadacitinib, abrocitinib이 국내에서 처방되고 있다. 국소 도포제로 나온 소분자 물질 중 JAK inhibitor로는 ruxolitinib과 delgocitinib이 사용되고 있고, phosphodiesterase (PDE)-4 inhibitor로 crisaborole과 difamilast, aryl hydrocarbon receptor (AhR) modulators가 경증 내지 중등증의 아토피피부염에서 좋은 효과를 보이고 있으나[2], 새로운 국소 도포제들은 아직 우리나라에서는 승인, 시판되지 않고 있다.

목적

본 논문은 생물학적 제제와 소분자 억제제를 포함한 다양한 아토피 피부염 표적 치료제의 작용 기전과 사용법을 소개하고자 한다.

생물학제제

Dupilumab

Dupilumab은 IL-4와 IL-13이 공통으로 결합하는 IL-4 수용체 alpha-subunit에 결합하여 Th2 사이토카인을 차단하는 인간 유전자재조합 단클론 항체이다.

중등증에서 중증의 성인 아토피피부염을 대상으로 한 3상 임상시험(SOLO 1, SOLO 2)에서 dupilumab 300 mg을 2주에 1회, 또는 위약을 피하 주사로 16주간 치료하였다. 16주에 위약 대조군에 비해, dupilumab 2주에 1회 주사 군에서 investigator global assessment (IGA) 점수 0/1 (병변이 완전히 소실되거나 거의 없는 상태) 달성률 37% vs. 9% (위약)를 보였다. Eczema Area and Severity Index(EASI) 점수가 75% 감소한 EASI-75의 달성률도 치료군 47.7%, 위약군 13.3%로, 치료군에서 현저한 개선 효과를 보였다[3]. Dupilumab은 또한 가려움증과 불안, 우울증세, 삶의 질 등의 통계 변수에서 유의한 호전을 보였다. Dupilumab에서 흔히 나타난 부작용은 주사 부위의 반응, 결막염이었다. 이 임상시험은 아토피피부염 치료의 이정표가 되는 획기적인 결과로 아토피피부염 치료의 새로운 장을 열었다. Dupilumab의 장기간 효능과 안전성을 입증하기 위한 임상시험에서는 중등도 내지 중증의 아토피피부염 성인 환자에서 필요 시 국소 스테로이드제 도포를 병행하면서, dupilumab 300 mg (매주 또는 격주 치료)과 위약 대조군에서 52주간 진행하였다. 16주에 EASI-75에 도달한 환자 비율(매주 치료군 63%, 격주 치료군 69%, 위약 대조군 23%)이 52주에서도 지속적으로 동등한 효과를 보였다(Table 1) [4]. 치료 중 발생한 부작용으로는 결막염이 14–19%에서 나타났으며, 이에 대한 확실한 원인은 아직 밝혀진 바 없다. Dupilumab은 자가로 맞을 수도 있는 피하주사제로 성인의 경우 초기 용량 600 mg을 주사 후, 2주마다 300 mg을 투여하도록 되어있다. 아직까지 용량을 감소시키거나 약제를 중단하는 시점에 대한 자료는 분석 중이며, 명확한 가이드라인은 없다. 우리나라에서도 현재 6개월 이상의 소아 및 성인에서 기존의 전신 면역억제치료에 반응하지 않는 중증 아토피피부염에서 보험급여가 가능한 제제로 사용되고 있다. 실제 임상에서 dupilumab의 사용 경험과 자료가 축적되면서, 뛰어난 효능과 안전성을 고려할 때 국소치료제에 반응하지 않는 중등도 이상의 아토피피부염 환자에서 광선치료나 고식적인 면역억제제 보다 dupilumab을 1차 전신치료제로 사용할 것을 권고하는 전문가 의견과 치료 가이드라인이 제시되었다[5,6].

Efficacy outcomes of biologics

Tralokinumab

Tralokinumab은 IL-13을 중화시키는 면역글로불린(immunoglobulin, Ig) G4 인간항체로 IL-13Rα의 활성화를 억제한다. 아토피피부염 환자의 피부에서 IL-4보다는 IL-13이 아토피피부염의 병리학적 기전에서 더 중심적인 역할을 하는 사이토카인으로 피부장벽의 파괴, 염증반응의 증폭과 신경기원의 가려움증에 관여한다. 중등증에서 중증의 성인 아토피피부염을 대상으로 한 3상 임상시험(ECZTRA 1, ECZTRA 2)에서 tralokinumab 300 mg을 2주마다 투여한 군과 위약군에서 16주에 IGA 0/1 달성률을 달성한 비율이 15.8% /22.2% (ECZTRA 1/2) vs. 7.1% /10.9% (위약군)였고, EASI‑75 달성률은 위약군 대비 25% /33.2% (ECZTRA 1/2) vs. 12.7%/11.4% (위약군)였다(Table 1). 신속한 가려움증 감소와 수면, 삶의 질 개선효과도 1년 이상 장기간 치료 시 국소 스테로이드제나 추가 약물치료 치료 없이도 꾸준히 유지되었다[7].

Lebrikizumab

Lebrikizumab은 IL-13에 대한 IgG4 단클론 항체로 IL-4Rα와 IL-13Rα1 heterodimer 형성을 방해하여 신호전달체계를 억제하는 기전을 가지고 있다. Tralokinumab과는 달리 lebrikizumab은 decoy receptor로 알려진 IL-13Rα2 receptor와는 결합하지 않는데, 이 수용체의 생물학적 기능은 아직 명확히 밝혀진 바는 없다. 3상 임상시험 lebrikizumab 단독요법(ADvocate 1/2)에서 250 mg을 2주 간격으로 투여한 군은 위약 대비, 16주에 IGA 0/1은 43.1%/33.2% (위약 12.7%/10.8%), EASI‑75는 58.8%/52.1% (위약 16.2%/18.1%)로 유효성을 보였다. 국소 스테로이드제 도포를 병행한 경우에는 EASI-75 69.5% vs. 42.2% (위약) 달성률을 보였고, 가려움과 삶의 질이 현저히 개선되었다(Table 1) [8].

Nemolizumab

Nemolizumab은 IL-31 receptor α 길항체로 IL-31 경로를 차단하여 가려움증뿐 아니라 피부 염증반응과 표피증식, 피부장벽 기능 개선도 기대할 수 있다. Nemolizumab과 국소 스테로이제/국소 칼시뉴린(calcineurin) 억제제를 병행한 3상 임상시험(ARCADIA 1/2)에서 16주에 IGA 0/1 달성률 36%/38% (ARCADIA 1/2) (위약 25%/26%)와 EASI-75 44%/42% (위약 29%/30%) 유효성을 보였다(Table 1). Nemolizumab의 강점은 주사 후 1주 이내에 가려움증이 신속히 사라지고, 수면 개선효과가 뛰어나다[9]. 아직 국내에는 시판되고 있지 않다.

JAK 억제제

JAK-STAT 신호전달체계(특히 JAK1)는 type 2 사이토카인(IL-4, IL-13, IL-31)이 활성화된 이후의 염증반응을 조절한다. JAK 억제제는 IL-4, IL-13과 연관된 Th2 세포의 분화를 억제하고, 피부장벽 기능의 개선과 소양증을 감소시키는 효과가 있다. JAK 억제제를 비롯한 소분자 물질들은 경구용 또는 국소 도포제로 개발되어 편의성과 접근성이 향상되었다. 2세대 JAK 억제제는 JAK의 기능을 선택적으로 억제하여(특히 JAK1) 1세대에 비해 안전성과 효과가 높아졌다.

Baricitinib

JAK1/2 억제제로 아토피피부염의 전신치료제로 유럽에서 처음 승인된 약제이다. Baricitinib 단독요법에서는 16주 평가 시 IGA 0/1 16.8% (4 mg), 11.4% (2 mg), 4.8% (위약); EASI‑75 24.8% (4 mg), 18.7% (2 mg), 8.8% (위약)로 보고되었다[10]. Baricitinib 4 mg을 국소 도포제와 병합하여 사용했을 때 16주 평가시점에서 EASI‑75 달성률 48% (4 mg) vs. 23% (위약) 등 주요 지표가 유의하게 개선되었다(Table 2). 가려움증은 복용 후 1주만에 사라지는 빠른 효과를 보였으며, 발생할 수 있는 부작용으로는 인두염, 상기도 감염, 일시적인 creatinine phosphokinase 증가, 두통이 보고되었으나 대부분 경미한 수준이었다. 52주간 투여한 경우에도 2 mg, 4 mg으로 지속적인 치료효과를 보였다[11,12].

Efficacy outcomes of small molecules

Abrocitinib

JAK1의 선택적 억제제로 12세 이상의 중등증 내지 중증의 아토피피부염 환자에서 100 mg, 200 mg을 사용할 수 있다. Abrocitinib 단독 치료 12주 평가에서 IGA 0/1이 44% (200 mg)/24% (100 mg)/8% (위약), EASI‑75 63% (200 mg)/40% (100 mg)/12% (위약)로 보고되었다(Table 2) [13]. Abrocitinib 200 mg과 dupilumab 300 mg으로 26주간 국소 도포제를 병행하면서 시행한 직접 비교 임상시험에서 2주차에 가려움증 개선효과는 abrocitinib vs. dupilumab (48% vs. 26%), 4주차에 EASI-90은 abrocitinib vs. dupilumab (29% vs. 15%)으로 dupilumab보다 좋은 치료효과를 보였다[14]. 가장 흔한 부작용으로는 오심(nausea)이었으며, 구토, 두통, 어지럼증, 비 인후염, 상기도 감염, 여드름, 모낭염, 대상포진 등으로 비교적 경미한 반응이었다[15].

Upadacitinib

JAK1 선택적 억제제로 12세 이상의 아토피피부염 환자에게 사용할 수 있으며, 15 mg, 30 mg으로 출시되었다. Upadacitinib 단독 치료 시 16주 평가시점에서 IGA 0/1 달성률이 48% (15 mg), 62% (30 mg)와 EASI-75를 달성한 비율은 70% (15 mg), 80% (30 mg)에 달해 매우 뛰어난 치료효과를 보였다(Table 2) [16]. Upadacitinib 30 mg과 Dupilumab 300 mg을 직접 비교한 임상시험에서 16주 평가 시, EASI-75 달성률은 upadacitinib vs. dupilumab (71% vs. 61.1%), 치료 2주 후 가려움증 효과 척도는 upadacitinib vs. dupilumab (43.7% vs. 17.4%)으로 우월한 치료효과를 보였다[17]. 가장 흔히 발생하는 부작용은 여드름이며, 그 외 다른 JAK 억제제들과 비슷한 부작용들이 발생하였다[18].

Ruxolitinib

Ruxolitinib 국소 JAK1/2 억제제로 피부 내 염증성 사이토카인 신호를 억제한다. 두 건의 3상 임상시험(TRuE-AD1, TRuE-AD2)에서 8주차에 IGA 0/1 달성률이 50%/39% (0.75% RUX cream), 53.8%/51.3% (1.5% RUX cream), 15.1%/7.6% (위약)를 보였고, 가려움증 개선효과는 1.5% RUX 도포 후 12시간 이내에 나타났다. 국소 도포에 부작용은 1% 이하로 낮았다[19]. 국소 제형으로 개발되어 피부 안전성이 높고, 장기간 유지 치료로 질환을 조절할 수 있는 가능성이 제기되었다.

Delgocitinib

Delgocitinib: pan-JAK 억제제의 국소 제형으로, 일본에서 성인 및 소아(2세 이상) 아토피피부염 치료제로 승인되었다. 3상 임상시험에서 delgocitinib 0.5% 연고제를 4주간 도포 후 mEASI 점수는 44.3% (위약군 1.7% 증가) 감소했으며, 가려움증 개선효과가 위약군 대비 유의하게 감소하였다. 24주까지 연장시험에서 mEASI-50 달성률은 51.9% (위약군 11.5%)까지 증가하였다. 부작용은 대부분 경미하였으나, 1.9%에서 카포시 수두양발진이 발생하였다[20,21].

PDE4 억제제

PDE4는 cyclic adenosine monophosphate (cAMP)를 분해하여 염증반응을 유발한다. PDE4를 억제함으로써 세포 내 cAMP가 증가하고, nuclear factor-κB와 T세포 활성화 신호전달체계를 차단하여 사이토카인의 분비를 억제한다.

Crisaborole은 2세 이상의 소아 및 성인의 경도 내지 중등도 아토피피부염 환자에서 4주간 도포 후 위약 대비 IGA 및 pruritus 개선효과를 입증하였고, 국소 자극증상 외에 부작용은 경미하였다[22].

Difamilast는 일본에서 2세 이상의 소아 및 성인의 아토피피부염에서 승인되었으며, 3상 임상시험에서, 0.3%, 1%, 위약 국소 제형제를 4주간 도포 후 IGA (44.6%/47.1%/18.1%)와 EASI-50, EASI-75, EASI-90 반응률이 유의하게 높았고, EASI 점수는 치료 1주 후부터 현저히 감소하였다[23,24]. Roflumilast는 6세 이상의 소아 및 성인의 경증 내지 중등도의 아토피피부염 환자에 0.15% 제형제를 4주간 도포한 경우 IGA 0/1이 28–32% (위약 12–15%) 달성률을 보였고, 국소 자극반응은 미미하였다[25]. Roflumilast 0.1% 국소 제제는 건선 및 지루 피부염에서도 승인을 받았다.

AhR 작용제

AhR 작용제(aryl hydrocarbon receptor agonist)는 피부장벽과 면역 조절에 관여하는 전사인자를 표적으로 하며, 새로운 기전의 소분자 물질로 개발되었다. Tapinarof는 대표적인 AhR 작용제로 피부장벽 회복과 항산화 효과, Th2 사이토카인 감소를 통해 염증을 억제한다. 1% tapinarof 국소 제형제를 2세 이상의 소아 및 성인의 아토피피부염에 도포한 ADORING 1/2 3상 임상시험에서 8주차에 IGA 0/1 달성률이 45.4–46.4% (위약 13.9–18%)였고, EASI-75 반응률도 55.8–59.1% (위약 21.2–22.9%)로 유의하게 개선되었다[26]. 흔한 부작용은 모낭염(>5%)과 두통, 인후염이었고, 대부분 경미하였다.

결론

새로운 표적치료제들은 치료 불응성의 아토피피부염에서 Th2와 이와 연관된 신호전달경로를 선택적으로 차단하여 우수한 치료효과를 보여 치료 불응성의 난치성 아토피피부염 환자들의 치료에서 기대를 모으고 있다. 치료제를 선택하는 기준에는 작용기전과 약제 효과의 발현시기, 장단기적 유효성과 안전성, 위험요인, 동반질환, 환자의 연령과 선호도, 경제적 요인, 제한 규정 등 다양한 요인을 고려하여 의사와 환자가 함께 결정을 하는 것이 바람직하다.

하지만 장기간 안전성과 지속적인 효과, 비용 대비 효과 측면을 고려하여 체계적이고 장기적인 자료 분석이 필요하다. 면역학적 기전에 기반한 새로운 표적치료제들은 향후 아토피피부염의 개인적인 다양한 특성들을 고려한 맞춤 치료에 새로운 패러다임을 제시할 것으로 기대한다.
