Abstract

Purpose: Systemic therapy remains the cornerstone of managing advanced gastric cancer (AGC). For patients with unresectable or metastatic AGC, it is essential for extending survival and alleviating symptoms.
Current Concepts: Cytotoxic chemotherapy, historically centered on fluoropyrimidines, platinum agents, taxanes, and irinotecan, continues to form the treatment foundation. More recently, targeted agents and immune checkpoint inhibitors (ICIs) have reshaped therapeutic strategies. Combination regimens incorporating nivolumab, pembrolizumab, or tislelizumab with chemotherapy have improved outcomes in HER2-negative AGC, particularly among patients with programmed cell death-ligand 1–positive or microsatellite instability-high/deficient mismatch repair tumors. Zolbetuximab, which targets CLDN18.2, has also shown clinically meaningful benefit in CLDN18.2-positive disease. In HER2-positive AGC, trastuzumab remains the therapeutic backbone, with findings from the KEYNOTE-811 trial supporting the addition of pembrolizumab to trastuzumab-based chemotherapy. In the second-line setting, ramucirumab plus paclitaxel is preferred, and trastuzumab deruxtecan provides benefit in previously treated HER2-positive patients. Later-line options include trifluridine/tipiracil, nivolumab, and trastuzumab deruxtecan.
Discussion and Conclusion: The treatment landscape for AGC is rapidly evolving toward biomarker-driven precision oncology. Incorporating ICIs, targeted agents, and optimized treatment strategies has transformed management and enabled more individualized, effective, and less invasive therapeutic approaches.

서론

배경

위암은 전 세계적으로 발병률과 사망률이 높은 악성종양으로, 우리나라를 포함한 동아시아에서 질병 부담이 특히 크다. 조기 병기에서는 내시경 절제나 근치 수술로 완치를 기대할 수 있으나, 국소 진행성•절제 불가능•전이성 단계에 이르면 전신 항암치료가 치료 성과를 좌우한다. 지난 수십 년간 플루오로피리미딘(fluoropyrimidine), 백금계, 탁센(taxane), 이리노테칸(irinotecan)을 중심으로 한 세포독성 항암제가 치료의 근간을 이루었고, 이후 HER2 표적치료제와 면역관문억제제(immune checkpoint inhibitor)의 도입으로 치료 패러다임이 빠르게 변화했다. 최근에는 programmed cell death-1 (PD-1) 억제제와 화학요법의 병용, CLDN18.2 표적치료, 항체-약물 접합체(antibody-drug conjugate, ADC) 등 바이오마커 기반 치료가 실제 임상에 적용되며, 분자 아형에 따른 정밀의료 전략의 중요성이 커지고 있다. 그럼에도 치료 반응의 이질성, 내약성 문제, 최적 치료 순서와 병용 전략에 대한 근거 부족 등 해결 과제가 여전히 남아 있다.

목적

이 논문의 목적은 진행성 위암의 전신치료를 최신 근거 중심으로 정리하고, 세포독성 항암요법, 표적치료, 면역항암치료의 역할과 한계를 균형 있게 고찰하는 데 있다. 또한, 대한위암학회에서 발표한 위암치료 가이드라인을 참고하여, 환자 선별과 약제 선택에 있어 실제 진료에서 활용 가능한 실천적 지침을 제시하고자 한다[1].

HER2 음성 질환에서의 1차 항암치료

니볼루맙(nivolumab)은 PD-1을 억제하는 면역관문억제제로, HER2 음성 국소 진행성, 절제 불가 또는 전이성 위암 환자에서 세포독성 항암제와 병용하는 1차 치료로 사용되어 성공을 거둔 첫 번째 면역관문억제제이다. 전향적 3상 임상시험인 CheckMate-649에서, 캡시타빈(capecitabine) 또는 5-fluorouracil (5-FU)/옥살리플라틴(oxaliplatin)에 니볼루맙을 추가한 군이 화학요법 단독군에 비해 전체생존(overall survival, OS)을 유의하게 개선함을 보여주었다(중앙 OS, 13.8개월 vs. 11.6개월; 위험비[hazard ratio, HR], 0.80; 99.3% 신뢰구간[confidence interval, CI], 0.68–0.94; P=0.0002) [2]. 특히 programmed cell death-ligand 1 (PD-L1) combined positive score (CPS) ≥5 환자에서는 효능이 증대되었다(중앙 OS, 14.4개월 vs. 11.1개월; HR, 0.71; 98.4% CI, 0.59–0.86; P<0.01) [2]. 일본, 한국, 대만에서 수행된 ATTRACTION-4 3상 임상시험에서도 HER2 음성 위암에서 1차 완화요법으로 니볼루맙과 화학요법 병용을 평가하였는데, 니볼루맙+캡시타빈 또는 S-1/옥살리플라틴 병용군에서 무진행생존(progression-free survival, PFS)이 유의하게 개선되었다(중앙 PFS, 10.5개월 vs. 8.3개월; HR, 0.68; 95% CI, 0.51–0.90; P<0.01). 그러나 OS 개선은 관찰되지 않았다[3].

펨브롤리주맙(pembrolizumab) 또한 1차 치료로 항암제 병용요법으로 평가되었다. KEYNOTE-062, 3상 시험에서, 펨브롤리주맙+화학요법(5-FU 또는 캡시타빈+시스플라틴[cisplatin])은 HER2 음성이며, PD-L1 CPS ≥1인 위암 환자에서 OS에 대해 화학요법 단독 대비 우월성을 입증하지 못했다(중앙 OS, 12.5개월 vs. 11.1개월; HR, 0.85; 95% CI, 0.70–1.03; P=0.05) [4]. 그러나 KEYNOTE-859, 3상 시험에서는 펨브롤리주맙을 5-FU/시스플라틴(FP) 또는 CAPOX에 병용한 1차 완화 전신치료가 OS를 유의하게 개선하였다. 전체 환자군(intention-to-treat, ITT)에서 펨브롤리주맙+화학요법군의 중앙 OS는 12.9개월로, 화학요법 단독군의 11.5개월보다 길었다(HR, 0.78; 95% CI, 0.70–0.87; P<0.01). 또한 PD-L1 CPS ≥1 환자에서는 중앙 OS 13.0개월 vs. 11.4개월(HR, 0.74; 95% CI, 0.65–0.84), CPS ≥10 환자에서는 15.7개월 vs. 11.8개월(HR, 0.65; 95% CI, 0.53–0.79)로 더 큰 이득이 관찰되었다[5]. 또 다른 PD-1억제제인 티슬렐리주맙(tislelizumab)은 글로벌 3상 연구인 RATIONALE-305 시험에서 항암제 단독 대비 티슬렐리주맙을 병용하였을 때 OS 개선을 보였으며(중앙 OS, 15.0 vs. 12.9개월; HR, 0.80; P=0.001), 특히 PD-L1 tumor area positivity ≥5 환자에서 개선 폭이 더 컸다[6].

또한 메타분석 결과, 항-PD-1 항체와 화학요법 병용은 화학요법 단독 대비 OS (HR, 0.81; 95% CI, 0.76–0.86), PFS (HR, 0.78; 95% CI, 0.73–0.83), 객관적 반응률(objective response rate, ORR), 질병조절률(disease control rate, DCR)에서 모두 우월했다. 이 근거를 바탕으로, 한국에서는 현재 니볼루맙+화학요법(CAPOX 또는 FOLFOX), 펨브롤리주맙+화학요법(FP 또는 CAPOX), 또는 티슬렐리주맙+화학요법(FP 또는 CAPOX)이 1차 완화 전신치료로 승인되어 있다. 또한 microsatellite instability-high (MSI-H)/ deficient mismatch repair (dMMR) 종양은 드물지만, 하위군 분석에서 항-PD-1 항체는 일관되게 유의미한 OS 이득을 보였다[7].

졸베투주맙(zolbetuximab)은 CLDN18.2를 표적으로 하는 단클론 항체로, 2개의 글로벌 3상 연구인 SPOTLIGHT와 GLOW 시험에서 mFOLFOX6 또는 CAPOX와 병용 시 OS와 PFS를 유의하게 개선하였다(OS, 18.2 vs. 15.6개월; HR, 0.78; P<0.01/OS, 14.3 vs. 12.2개월; HR, 0.77; PFS, 8.2 vs. 6.8 months; HR, 0.69; P<0.001) [8,9]. 이 2개의 연구를 통해 현재 한국에서는 CLDN18.2 양성, HER2 음성인 진행성 위암 환자에서 졸베툭시맙+항암요법(mFOLFOX6 또는 CAPOX)이 승인되어 사용되고 있다.

HER2 양성 질환에서의 1차 항암치료

HER2를 표적으로 하는 단클론 항체인 트라스투주맙(trastuzumab)의 임상적 유효성은 글로벌 3상 연구인 ToGA 임상시험에서 처음 입증되었다. HER2 양성 위암 환자에서, 트라스투주맙을 플루오로피리미딘/시스플라틴 기반 1차 화학요법에 병용했을 때 OS를 유의하게 개선하였다[10]. 세부적으로 캡시타빈 또는 FP에 트라스투주맙을 병용한 군은 화학요법 단독군에 비해 OS가 개선되었다(중앙값, 13.8개월 vs. 11.1개월; HR, 0.74; 95% CI, 0.60–0.91; P=0.0046). 특히 HER2 단백 발현이 높은 군(immunohistochemistry [IHC] 3+ 또는 IHC 2+ & fluorescence in situ hybridization [FISH] 양성)에서 생존 이득이 더욱 뚜렷하였다(중앙 OS, 16.0개월 vs. 11.8개월; HR, 0.65; 95% CI, 0.51–0.83; P<0.01). PFS 역시 트라스투주맙 병용군에서 의미 있게 연장되었다(중앙, 6.7개월 vs. 5.5개월; HR, 0.71; 95% CI, 0.59–0.85; P=0.036). ORR은 트라스투주맙 병용군에서 더 높았으며(47% vs. 35%; 교차비[odds ratio, OR], 1.70; 95% CI, 1.22–2.38; P=0.002), DCR 또한 우월하였다(75% vs. 70%; OR, 1.66; 95% CI, 1.14–2.41; P=0.008).

한편, 티로신키나제 억제제(tyrosine kinase inhibitor, TKI) 라파티닙(lapatinib, epidermal growth factor receptor [EGFR]/HER2 TKI)을 이용한 LOGiC 3상 시험에서 CAPOX와 병용하여 평가되었으나, HER2 증폭 위암 환자에서 OS를 개선하지 못했다(중앙, 12.2개월 vs. 10.5개월; HR, 0.91; 95% CI, 0.73–1.12; P=0.349) [11]. HER2 이중 차단 전략(dual HER2 blockade)의 유효성을 검증하기 위해, JACOB 3상 임상시험에서는 HER2와 HER4 수용체 간 이형이합체화(heterodimerization)를 억제하는 단클론 항체 퍼투주맙(pertuzumab)을 트라스투주맙+화학요법에 추가하였다. 퍼투주맙 추가 시 PFS는 개선되었으나(중앙, 8.5개월 vs. 7.2개월; HR, 0.73; 95% CI, 0.62–0.85; P<0.01), 1차 평가변수였던 OS는 통계적 유의성을 달성하지 못했다(OS, 17.5개월 vs. 14.2개월; HR, 0.84; 95% CI, 0.71–1.00; P=0.057) [12].

HER2 음성 위암에서 면역관문억제제를 표준 화학요법에 병용하는 전략이 생존 개선을 입증한 바 있으며, 이러한 결과는 HER2 양성 위암에서도 확장되었다. 무작위배정 3상 KEYNOTE-811 임상시험에서는 펨브롤리주맙+트라스투주맙+표준 화학요법(플루오로피리미딘/백금계)과 트라스투주맙+표준 화학요법을 비교하였다[13]. 의도한 ITT에서 펨브롤리주맙 병용군의 PFS가 위약군 대비 개선되었다(중앙, 10.0개월 vs. 8.1개월; HR, 0.73; 95% CI, 0.61–0.87; P=0.0005). OS 역시 개선 경향을 보였는데(중앙, 20.0개월 vs. 16.8개월; HR, 0.84; 95% CI, 0.70–1.01; P=0.06), PD-L1 CPS ≥1인 환자군에서는 개선 폭이 더 컸다. 이 아형 환자군에서 PFS는 10.9개월 vs. 7.3개월(HR, 0.72; 95% CI, 0.60–0.87; P<0.01), OS는 20.1개월 vs. 15.7개월(HR, 0.79; 95% CI, 0.66–0.95; P=0.006)로 확인되었다. ORR 또한 펨브롤리주맙 병용군이 더 높았다(72.6% vs. 60.1%; OR, 1.76; 95% CI, 1.28–2.42; P<0.001). DCR 역시 우수한 경향을 보였다(91.7% vs. 87.4%; OR, 1.60; 95% CI, 0.98–2.63; P=0.06). 이러한 결과를 바탕으로, HER2 양성이면서 PD-L1 CPS ≥1인 환자에서 트라스투주맙+화학요법에 펨브롤리주맙을 추가하는 전략이 권고된다.

2차 항암치료

무작위 대조시험(randomized controlled trial)과 기존 메타분석들은 국소 진행성 불가절제 또는 전이성 위암 환자에서 2차 완화 화학요법(이리노테칸 또는 탁센계)이 최선지지요법(best supportive care) 단독 대비 생존 이득을 제공함을 명확히 입증해왔다[14–19]. 이전의 3상 시험들에 따르면, 주 1회 파클리탁셀(paclitaxel weekly regimen)은 격주 이리노테칸(biweekly irinotecan)과 유사한 생존 결과를 보였다[18,19]. 한편, 혈관내피성장인자수용체-2 (VEGFR-2)를 표적으로 하는 단클론 항체 라무시루맙(ramucirumab) 단독요법은 3상 REGARD 시험에서 위약 대비 OS와 PFS를 유의하게 개선하였다[20]. 또한, 3상 RAINBOW 시험에서는 주 1회 파클리탁셀에 라무시루맙을 추가했을 때 파클리탁셀+위약 대비 OS (중앙, 9.6개월 vs. 7.4개월; HR, 0.807; 95% CI, 0.68–0.97; P=0.017)와 PFS (중앙, 4.4개월 vs. 2.9개월; HR, 0.635; 95% CI, 0.53–0.76; P<0.0001)가 모두 유의하게 연장되었다[21]. 이러한 근거를 종합하면, 라무시루맙+파클리탁셀이 현재 가장 선호되는 2차 치료로 권고된다.

면역관문억제제 중 펨브롤리주맙은 일반 위암 환자에서 파클리탁셀 대비 유의한 생존 이득을 보여주지 못했으나[22–24], MSI-H, dMMR, tumor mutational burden-high (≥10 mutations/mega base) 등 특정 분자 아형 환자에서는 효과가 입증되었다[25,26]. 한국에서는, 절제 불가능 또는 전이성 고형암(위암 포함) 환자 중 MSI-H 또는 dMMR이면서 이전 치료 후 질병이 진행하였고, 대체 치료 옵션이 없는 경우에 한해 펨브롤리주맙 사용이 허가되어 있다.

또한, 트라스투주맙 데룩스테칸(trastuzumab deruxtecan, T-DXd)은 HER2 표적 ADC (payload: topoisomerase I inhibitor)로, 이전에 트라스투주맙 기반 치료를 받은 HER2 양성 위암 환자에서 미국 식품의약국 승인을 받았다. 2차 치료로서 T-DXd의 근거는 주로 서구 환자 집단에서 도출되었으며[27], 최근 발표된 글로벌 3상 DESTINY-Gastric04 시험(아시아 환자 포함)이 HER2 양성 위암 환자에서 T-DXd와 라무시루맙+파클리탁셀(표준 2차 치료) 간 생존 결과를 비교한 결과를 발표하였다. 일차 변수인 OS (14.7개월 vs. 11.4개월; HR, 0.7; 95% CI, 0.55–0.90, P=0.004)과 PFS (HR, 0.74; 95% CI, 0.59-0.92)를 향상시킨 결과를 발표하였다[28]. 이 결과를 통해 HER2양성 위암 환자의 2차 치료제로 T-DXd가 사용될 것을 기대하고 있다.

3차 항암치료

2차 치료 후에도 수행능력(performance status)이 유지된 환자에서는 3차 완화 전신치료가 권고된다. 세포독성 항암제 중에서는 도세탁셀(docetaxel) 또는 이리노테칸이 3차 치료로 권고된다. 무작위 3상 시험에서 도세탁셀 혹은 이리노테칸은 OS를 개선하였다(중앙, 5.3개월 vs. 3.8개월; HR, 0.66; 95% CI, 0.49–0.89; P=0.007) [14]. 탁센 또는 이리노테칸 기반 3차 화학요법을 평가한 다수의 2상 및 후향적 연구들 역시 일관된 결과를 보여주었다[29–31]. 트리플루리딘/티피라실(trifluridine/tipiracil, TAS-102)의 무작위 3상 시험에서는, 최소 2개 이상의 전신치료를 받은 위암 환자에서 위약 대비 OS가 개선되었다(중앙, 5.7개월 vs. 3.6개월; HR, 0.69; 95% CI, 0.56–0.85; P=0.00058) [32]. 니볼루맙은 일본-한국-대만의 3개 아시아국 다기관 연구인, ATTRACTION-2 3상 임상시험에서, 2가지 이상의 전신치료에 실패한 환자에서 위약 대비 OS 개선을 보였다(중앙, OS 5.3개월 vs. 4.1개월; HR, 0.63; 95% CI, 0.51–0.78; P<0.0001) [33]. 2년 추적 분석에서도 PD-L1 발현과 무관하게 니볼루맙의 장기 생존 이득이 유지되었다[34]. 따라서 면역관문억제제 치료 경험이 없는 환자에서 니볼루맙은 3차 치료로 권고된다.

한편, DESTINY-Gastric01 2상 임상시험(한국 및 일본)에서는, HER2 양성이며, ≥2차 전신치료(트라스투주맙 포함)를 받은 환자에서 T-DXd와 의사선택군(이리노테칸 또는 파클리탁셀)을 비교하였다[35]. 이 시험에서 T-DXd는 ORR (51% vs. 14%, P<0.001)과 OS (중앙, 12.5개월 vs. 8.4개월; HR, 0.59; 95% CI, 0.39–0.88; P=0.01)를 모두 개선하였다. 이러한 결과를 근거로, T-DXd는 미국에서는 2차 이상, 한국에서는 3차 이상 치료로 승인되었다.

결론

진행성 위암은 세포독성 항암제 단독치료에서 HER2 양성인 위암에서는 1차요법으로 HER2 억제제인 트라스투주맙과 항암제 병용치료 또는 anti-PD-1 억제제인 펨브롤리주맙의 포함한 4제 요법의 치료로 높은 반응률과 생존율의 향상을 가져왔고, ADC인 T-DXd가 3차 및 2차 치료제로 효능을 입증하여 다양한 치료가 가능하게 되었다. HER2 음성인 위암에서는 CLDN18.2 양성에서는 졸베투주맙과 세포독성 항암제 병용치료가 가능하게 되었고, PD-L1 양성인 위암의 경우 면역관문억제제인 니볼루맙, 펨브롤리주맙 및 티슬렐리주맙을 사용할 수 있게 되었으며, PD-L1 발현이 높은 종양에서는 생존율도 고무적으로 향상되었다. 현재 진행성 위암에서 CLDN18.2 표적치료, ADC 등을 포함한 새로운 약제가 개발 중이며, 이에 따라 생존율의 지속적인 향상이 기대된다.
