당뇨망막병증: 선별검사부터 치료까지

Abstract

Purpose: Diabetic retinopathy is one of the most common complications of diabetes mellitus and remains a leading cause of visual impairment worldwide. Because early-stage diabetic retinopathy is often asymptomatic, diagnosis is frequently delayed, resulting in progression to vision-threatening stages. This education column reviews the importance of screening, summarizes current diagnostic approaches, and outlines contemporary treatment strategies for diabetic retinopathy in routine clinical practice.
Current concepts: Regular screening and timely ophthalmic referral are essential for the early detection of diabetic retinopathy. Fundus examination remains the cornerstone of diagnosis, while advances in retinal imaging—including ultra-widefield fundus photography, optical coherence tomography (OCT), fluorescein angiography, and OCT angiography—enable more precise assessment of disease severity, retinal ischemia, and macular involvement. Management strategies are determined according to disease stage and include systemic risk factor control, intravitreal injections, laser photocoagulation, and surgical intervention. Intravitreal anti–vascular endothelial growth factor therapy is the first-line treatment for diabetic macular edema, whereas panretinal photocoagulation remains the standard treatment for proliferative diabetic retinopathy.
Discussion and conclusion: Diabetic retinopathy is a progressive disease; however, vision-threatening complications can be substantially reduced through early detection and appropriate management. Regular screening, accurate staging, and timely treatment are key elements in preventing severe visual loss. Close collaboration among primary care physicians, endocrinologists, and ophthalmologists, together with continuous patient education, plays a critical role in improving screening adherence and clinical outcomes. A comprehensive, multidisciplinary approach is essential to reduce the burden of visual impairment caused by diabetic retinopathy.

서론

배경

망막은 눈으로 들어오는 빛을 전기 신호로 변환하여 시신경을 통해 뇌로 전달하는 역할을 하는 조직으로, 흔히 카메라의 필름에 비유된다. 망막은 발생학적으로 뇌와 동일한 구조물에서 기원하여 뇌의 일부로 간주되며, 해부학적 및 기능적으로도 뇌와 유사한 특성을 지닌다[1]. 망막에는 다양한 신경세포층과 함께 동맥 및 정맥으로 구성된 미세혈관망이 존재하고, 혈액-망막 장벽(blood-retinal barrier)이 형성되어 있어 신경조직을 보호한다[2]. 이러한 구조적 특성으로 인해 망막 혈관에 이상이 발생하면 혈액-망막 장벽이 손상되고, 이로 인해 망막 신경의 기능 장애 및 시력 저하가 초래될 수 있다.
당뇨망막병증(diabetic retinopathy)은 당뇨병의 대표적인 합병증으로, 전 세계적으로 실명의 주요 원인 중 하나이다. 지속적인 고혈당 상태로 인한 산화 스트레스는 망막 모세혈관의 손상을 유발하고, 이로 인해 망막 허혈성 변화가 진행된다. 그 결과 망막 출혈, 당뇨황반부종(diabetic macular edema), 유리체 출혈(vitreous hemorrhage), 섬유화 증식에 의한 견인 망막박리(tractional retinal detachment) 등 다양한 합병증이 발생할 수 있다[3].
고령화와 함께 당뇨병의 유병률이 전세계적으로 증가하고 있으며, 국내에서도 당뇨망막병증 환자수는 지난 10년간 약 두 배 증가한 것으로 보고되었다[4]. 국민건강영양조사를 이용한 국내 연구에 따르면 당뇨병 환자 중 11.0–20.0%에서 당뇨망막병증이 동반되며, 이 중 실명 위험 당뇨망막병증은 약 5%를 차지하는 것으로 나타났다[46].
당뇨망막병증은 진행될 경우 단순한 시력 저하를 넘어 실명까지 이어질 수 있으나, 적절한 시기에 진단하고 단계에 맞는 치료를 시행할 경우 상당 부분 예방이 가능하다.

목적

이에 이번 연구에서는 당뇨망막병증의 선별검사, 진단 방법 및 치료 전략에 대해 정리하고자 한다.

선별검사 및 정기검사의 중요성

당뇨병의 유병 기간이 길수록, 그리고 혈당 조절이 불량할수록 당뇨망막병증의 발생 위험은 증가한다. 당뇨병 유병 기간이 6–10년인 환자에서는 10명 중 2명, 15년 이상인 환자에서는 3명 중 2명 정도꼴로 당뇨망막병증이 발생하는 것으로 보고되었다. 또한 당화혈색소(hemoglobin A1c) 수치가 1% 증가할 때마다 당뇨망막병증의 위험은 약 1.4배 증가한다. 당뇨망막병증은 일단 발생하면 혈당이 잘 조절되더라도 계속 진행할 수 있으므로 조기 발견이 매우 중요하며, 적절한 시기에 치료를 할 경우 심각한 시력 손실을 50–60%까지 감소시킬 수 있다.
그럼에도 불구하고 국내 2022년 건강보험심사평가원 자료에 따르면, 당뇨병 환자 중 당뇨망막병증 실명 예방에 필수적인 안저 검사를 받은 비율은 약 46%에 불과하였다[7]. 국민건강영양조사를 이용한 연구에서도 한해 당뇨망막병증 선별검사를 시행한 환자는 29.5%에 그쳤다[8].
정기적인 안과 검진이 이루어지지 않는 가장 큰 이유는 당뇨망막병증이 상당히 진행될 때까지 뚜렷한 시각적 자각 증상이 없는 경우가 많기 때문이다. 증상이 없어 검진을 미루다가 시력 저하 등의 증상을 느껴 안과에 방문하는 경우에는 이미 당뇨망막병증이 상당히 진행된 경우가 많다.
대한당뇨병학회에서는 1형 당뇨병 환자의 경우 당뇨 진단 후 5년 이내, 2형 당뇨병 환자의 경우 당뇨 진단 시점에서 즉시 안저 검사를 포함한 안과 검진을 받을 것을 권고하고 있다. 1형 또는 2형 당뇨병이 있는 임산부인 경우에는 임신 확인 시점 또는 임신 1분기 내에 첫 안과 검진을 시행하는 것이 권장된다. 이후 정기 검진 주기는 당뇨망막병증의 단계에 따라 조정되며, 당뇨망막병증 소견이 없고 혈당 조절이 잘되면 1–2년, 경증–중등증 비증식성 당뇨망막병증(non-proliferative diabetic retinopathy)에서는 6–12개월, 중증 비증식성 당뇨망막병증에서는 3–4개월, 증식성 당뇨망막병증(proliferative diabetic retinopathy)에서는 3개월 이내의 추적관찰이 필요하다. 당뇨황반부종이 동반된 경우에는 1–4개월 간격의 면밀한 추적관찰이 요구된다.

당뇨망막병증의 진단

당뇨망막병증 진단의 기본은 안저 검사이지만, 최근에는 다양한 영상검사 기법의 발전으로 초기 병변까지 보다 정밀하게 평가할 수 있게 되었다.

안저 검사

산동 후 검안경을 이용한 안저 검사(fundus examination)는 당뇨망막병증 진단의 가장 기본적인 방법으로, 직접 눈 속의 망막을 들여다보는 검사이다. 이외에도 산동 없이 촬영 가능한 안저 사진을 통해 망막 후극부의 변화를 평가할 수 있다. 최근에는 약 200도 범위의 주변부 망막까지 관찰 가능 초광각안저촬영(ultra-wide field fundus photography)이 널리 사용되고 있으며, 진단 민감도가 높고 산동이 필요 없다는 장점이 있다[6,9].
안저 검사에서 관찰되는 대표적인 소견으로는 미세혈관류(microaneurysm), 망막 출혈, 경성 삼출물(hard exudate), 면화반(soft exudate), 염주정맥(venous beading), 망막 내 미세혈관 이상(intraretinal microvascular abnormalities), 신생혈관(neovascularization), 유리체 출혈 또는 망막 앞 출혈, 섬유화 증식 등이 있다. 특히 모세혈관의 부분적인 원형확장을 보이는 미세혈관류의 분포와 정도는 비증식성 당뇨망막병증의 중등도를 판단하는 중요한 지표이다[10]. 또한 신생혈관의 존재 여부는 증식성 당뇨망막병증 진단의 핵심 기준이 된다. Figure 1은 당뇨병성 망막병증 소견의 사진이다.

형광안저혈관조영술

형광안저혈관조영술(fluorescein angiography)은 망막 허혈 범위 및 신생혈관을 평가하는 데 유용한 검사로, 형광 조영제를 정맥 주사한 후 시간에 따라 연속적인 안저 영상을 촬영한다. 초광각 형광안저혈관조영술의 도입으로 주변부 망막의 허혈성 변화에 대한 평가가 더욱 용이해졌다[9]. Figure 2는 초광각 형광안저혈관조영술 소견이다.

빛간섭단층촬영 및 빛간섭단층혈관조영술

빛간섭단층촬영(optical coherence tomography)은 현재 안과 영역에서 진단과 치료에 있어 광범위하게 사용 중인 검사 장비로, 황반부 구조를 고해상도로 관찰할 수 있는 비침습적 검사여서 당뇨황반부종 및 견인성 병변 평가에 필수적이다. 치료 전후 황반 두께 변화를 정량적으로 평가할 수 있다는 장점이 있다[9].
형광안저혈관조영술이 조영제 사용으로 인한 알레르기 반응, 신장 독성, 쇼크 반응 등의 부작용 위험성이 있는 한계를 보완하기 위해, 최근에는 빛간섭단층혈관조영술(optical coherence tomography angiography)이 널리 활용되고 있다. 빛간섭단층혈관조영술은 조영제를 사용하지 않고도 망막 및 맥락막 혈관 구조를 비침습적으로 시각화할 수 있어, 초기 미세혈관 변화 및 비관류 영역 평가에 유용하다[9]. 최근 기술 발전으로 보다 넓은 범위의 망막을 촬영할 수 있게 되어, 신생혈관의 존재 여부를 평가하는 데에도 도움이 되고 있다. Figure 3은 빛간섭단층촬영 사진이며, Figure 4는 빛간섭단층혈관조영술 소견이다.

당뇨망막병증의 단계

당뇨망막병증은 크게 비증식성 당뇨망막병증과 증식성 당뇨망막병증으로 분류된다. 비증식성 당뇨망막병증은 신생혈관이 발생하기 전 단계로, 안저 소견에 따라 경증(mild), 중등증(moderate), 중증(severe) 단계로 세분화된다. 또한 비증식성 당뇨망막병증 단계에서 관찰되는 심한 망막 출혈과 미세혈관류, 망막 내 미세혈관 이상, 염주정맥은 증식성 당뇨망막병증으로 진행하는 위험인자로 알려져 있으며, 이러한 소견의 분포와 정도에 따라 심한(severe) 비증식성 당뇨망막병증과 매우 심한(very severe) 비증식성망막병증으로 세분화될 수 있다.
증식성 당뇨망막병증은 망막 혈관 폐쇄로 인한 저산소증이 지속되면서, 허혈이 심한 부위에서 혈관형성 인자가 과도하게 생성되어 비정상적인 신생혈관이 발생하는 단계이다. 이러한 신생혈관은 시신경유두 표면이나 망막 내측 표면에부터 발생하여 유리체강 내로 증식할 수 있다. 신생혈관은 구조적으로 취약하기 때문에, 쉽게 출혈을 유발할 수 있다. 신생혈관에서 출혈이 발생하는 경우 유리체 출혈 또는 망막 앞 출혈이 나타나며, 환자는 비문증과 함께 급격한 시력 저하를 경험할 수 있다. 또한 신생혈관이 존재하는 부위에서 섬유화 증식이 동반되면 망막이 견인되어 견인성 망막박리가 발생할 수 있으며, 이는 심각한 시력 손실이나 실명으로 이어질 수 있다.
당뇨황반부종은 비정상적인 망막 혈관의 투과성 증가로 인해 혈관 외 누출이 발생하고, 이로 인해 망막 중심부인 황반 부위에 망막 내 부종, 망막하액, 삼출물 등이 형성되면서 황반부 두께가 증가하는 상태를 의미한다[11]. 당뇨황반부종은 당뇨망막병증의 어느 단계에서도 발생할 수 있으며, 주로 점진적인 중심 시력 저하의 원인이 된다.

당뇨망막병증의 치료

당뇨망막병증의 치료는 질환의 단계, 황반부종의 동반여부, 시력상태 및 전신상태를 종합적으로 고려하여 결정한다. 치료 전략은 크게 (1) 전신 치료, (2) 유리체강 내 주사 치료, (3) 레이저 치료, (4) 수술적 치료로 나눌 수 있다.

전신 치료

현재까지 당뇨망막병증의 발생이나 진행을 직접적으로 억제하는 경구 약물 치료에 대해서는 명확한 근거가 제한적이다. 따라서 당뇨망막병증 치료의 기본은 위험인자의 철저한 조절이며, 이는 모든 단계의 환자에서 필수적이다.
가장 중요한 위험인자는 당뇨병의 유병 기간과 혈당 조절 상태이다. 당화혈색소 수치를 7.0% 이하로 유지할 경우 당뇨망막병증의 발생 및 진행 위험을 유의하게 감소시킬 수 있으며, 당화혈색소 수치가 10% 감소할 때마다 당뇨망막병증의 진행 위험이 약 40% 감소하는 것으로 보고되었다[12,13]. 다만 고령 환자나 심혈관 위험이 높은 환자에서 지나치게 엄격한 혈당 조절은 저혈당 및 사망률 증가와 연관될 수 있어, 개별 환자의 상태를 고려한 목표 설명이 필요하다[14,15].
혈압 조절 또한 중요한 요소로, 수축기 혈압이 10 mmHg 감소할 때마다 당뇨망막병증의 진행 위험이 약 35% 감소하는 것으로 알려져 있다[12]. 또한 고지혈증은 황반부종 및 경성 삼출물의 형성과 관련이 있으므로, 총 콜레스테롤 및 중성지방 수치를 적절히 관리하는 것이 중요하다. 이와 같이 내과적 전신 치료는 당뇨망막병증 치료의 근간을 이루며, 안과 치료의 효과를 극대화하는 데 필수적이다.

유리체강 내 주사 치료

당뇨황반부종이 동반된 경우, 유리체강 내 항혈관내피성장인자(anti-vascular endothelial growth factor) 주사 치료가 현재 1차 치료로 자리 잡고 있다. 당뇨망막병증에서는 혈관내피성장인자 발현 증가가 혈관 투과성 증가와 신생혈관 형성을 유도하여 황반 부종 및 시력 저하를 초래한다[16]. 여러 대규모 임상 연구를 통해 항혈관내피성장인자 주사 치료는 당뇨황반부종 환자에서 황반부 두께를 감소시키고 시력을 유의하게 개선하는 것으로 입증되었다[1720]. 치료는 일반적으로 초기 집중 주사 후 반응에 따라 유지 치료를 시행하며, 환자 상태에 따라 주사 간격을 조절한다[18,19].
일부 환자에서는 항혈관내피성장인자 주사에 대한 반응이 불충분하거나, 약효 지속 기간이 짧아 잦은 주사가 필요한 경우가 있다. 이러한 경우 유리체강 내 스테로이드 제제를 고려할 수 있다. 사용 가능한 약제로는 트리암시놀론 아세토나이드(triamcinolone acetonide)와 덱사메타손 임플란트가 있다. 스테로이드 제제는 혈관 투과성을 억제하고 염증 반응을 감소시켜 황반부종을 완화시키는 효과가 있으나, 안압 상승 및 백내장 진행 등의 부작용 가능성이 있어 환자 선택과 추적관찰이 중요하다[21].
증식성 당뇨망막병증에서도 유리체강 내 항혈관내피성장인자 주사 치료를 시행할 수 있으며, 신생혈관의 퇴행 및 유리체 출혈의 진행을 지연시키는 데 효과가 있다. 다만 범망막레이저광응고술을 완전히 대체할 수 있는지에 대해서는 아직 논란이 있으며, 현재로서는 레이저 치료와 병행 요법으로 사용되는 경우가 많다[22,23].

레이저 치료

범망막레이저광응고술(panretinal photocoagulation)은 증식성 당뇨망막병증의 표준 치료법으로, 중심 시력과 직접적으로 연관된 망막의 후극부를 제외한 주변부 망막에 레이저를 조사하여 저산소 상태의 망막 조직을 파괴하는 치료이다. 이를 통해 혈관내피성장인자의 생성을 감소시켜 신생혈관을 퇴행시킬 수 있으며, 범망막레이저광응고술을 시행한 증식성 당뇨망막병증 환자에서 심각한 시력 손실의 위험을 약 50% 감소시킬 수 있다[24]. 다만 시야 협착 등의 부작용이 발생할 수 있어 치료 적응증과 범위를 신중히 결정해야 한다.
황반부종이 국소적인 미세혈관류의 누출에 의해 발생한 경우, 국소 레이저 치료(focal laser)를 통해 누출 부위를 광응고하여 부종 감소를 유도할 수 있다. 현재는 항혈관내피성장인자 주사 치료가 주된 치료법으로 사용되지만, 일부 환자에서는 보조적 치료로 국소 레이저 치료가 고려될 수 있다.

수술적 치료

당뇨망막병증에서 수술적 치료의 가장 흔한 적응증은 지속적인 유리체 출혈과 견인성 망막박리이다. 유리체 출혈이 자연 흡수되지 않거나, 섬유혈관 증식으로 인해 망막이 견인되는 경우에는 유리체절제술(vitrectomy)이 필요하다.
유리체절제술은 안구의 앞쪽 각막 윤부로부터 약 3.5–4 mm 정도 떨어진 공막을 통해 모양체 평면부(pars plana)에 3개의 작은 절개를 가한 뒤, 해당 절개창을 통해 관류관, 눈 속 조명기와 유리체 제거침을 삽입하여 동공을 통해 수술 현미경으로 망막을 관찰하면서 시행하는 수술이다. 이 수술을 통해 출혈로 혼탁해진 유리체를 제거하거나, 섬유혈관 증식막을 망막으로부터 분리하고, 출혈을 유발하는 혈관을 지혈하여 망막을 안정화할 수 있다. 필요에 따라 수술 중 안구 내 레이저 광응고술 혹은 유리체강 내 항혈관내피성장인자 주사를 병행하기도 한다.
최근에는 25게이지 또는 27게이지 시스템을 이용한 미세절개 유리체절제술의 발전으로 수술 시간이 단축되고 수술 관련 합병증이 감소하여, 전반적인 수술 성공률과 환자 만족도가 크게 향상되었다[25].

결론

당뇨병 환자에서는 자각 증상이 없더라도 정기적인 안과 검진을 통해 당뇨망막병증을 조기에 발견하고, 병의 단계에 맞는 적절한 치료를 시행하는 것이 실명 예방의 핵심이다. 안저 검사를 통해 병의 진행 정도를 정확히 파악하고, 이에 따라 추적관찰 간격과 치료 전략을 결정해야 한다.
내과와 안과 간의 긴밀한 협진, 그리고 환자에 대한 지속적인 교육은 당뇨망막병증으로 인한 시력 손실과 실명률을 낮추는 데 중요한 역할을 할 것이다.
