Trans Abstract

Purpose

Idiopathic pulmonary fibrosis (IPF) is a chronic, progressive fibrosing interstitial lung disease of unknown etiology that primarily affects older adults. It is characterized by worsening dyspnea, declining lung function, and poor prognosis. This review summarizes current approaches to IPF diagnosis and treatment.

Current concepts

Diagnosis requires a multidisciplinary approach integrating clinical, radiological, and, when necessary, histopathological findings. A key feature is the usual interstitial pneumonia pattern on high-resolution computed tomography and/or lung biopsy. Pirfenidone and nintedanib appear to slow the decline in lung function, including forced vital capacity (FVC), and are recommended as first-line therapies. Clinical trials of novel agents are ongoing, with several recent trials yielding promising findings. Non-pharmacological management, including pulmonary rehabilitation, oxygen therapy, and symptom control, is essential for comprehensive care. Lung transplantation remains the only curative option and should be considered in eligible patients.

Discussion and conclusion

Early, accurate IPF diagnosis is essential and requires a multidisciplinary approach that integrates clinical, radiological, and histopathological findings, as outlined in the 2018 and 2022 American Thoracic Society/European Respiratory Society/Japanese Respiratory Society/Latin American Thoracic Association guidelines. Pirfenidone and nintedanib remain the cornerstone antifibrotic agents for pharmacological treatment, slowing FVC decline and reducing the risk of acute exacerbation. Emerging agents, including nerandomilast and inhaled treprostinil, have shown promise in recent phase 3 clinical trials. Non-pharmacological strategies, including pulmonary rehabilitation, oxygen therapy, and palliative care, are integral to comprehensive management. Despite these advances, IPF remains a progressive and fatal disease, underscoring the need for continued research into novel therapeutic strategies.

High-resolution computed tomography patterns in idiopathic pulmonary fibrosis according to the official ATS/ERS/JRS/ALAT clinical practice guideline [2,13]

Figure 1.

Diagnostic algorithm for idiopathic pulmonary fibrosis (IPF) according to the official American Thoracic Society/European Respiratory Society/Japanese Respiratory Society/Latin American Thoracic Association (ATS/ERS/JRS/ALAT) clinical practice guideline. a)Patients with a radiological pattern of probable usual interstitial pneumonia (UIP) may receive a diagnosis of IPF after multidisciplinary discussion (MDD), without confirmation by lung biopsy, in an appropriate clinical setting, such as a 60-year-old male smoker. Bronchoalveolar lavage (BAL) may be appropriate in some patients with a probable UIP pattern. b)BAL may be performed before MDD in some patients evaluated at experienced centers. c)Transbronchial lung cryobiopsy (TBLC) may be preferred to surgical lung biopsy (SLB) in centers with appropriate expertise and/or in selected patient populations. Subsequent SLB may be justified in some patients with nondiagnostic findings on TBLC. HRCT, high-resolution computed tomography. Adapted from Raghu G et al. Figure 2.

Diagnosis of idiopathic pulmonary fibrosis (IPF) based on high-resolution computed tomography (HRCT) and histopathology/biopsy patterns in patients with clinically suspected IPF, developed by consensus discussion in the official American Thoracic Society/European Respiratory Society/Japanese Respiratory Society/Latin American Thoracic Association (ATS/ERS/JRS/ALAT) clinical practice guideline. UIP, usual interstitial pneumonia. a)Clinically suspected IPF is defined as unexplained bilateral pulmonary fibrosis on chest radiography or chest computed tomography, bibasilar inspiratory crackles, and age >60 years. Middle-aged adults, defined as those aged 40 to 60 years, can rarely present with otherwise similar clinical features, especially those with features suggestive of familial pulmonary fibrosis. b)Diagnostic confidence may need to be downgraded if histopathological assessment is based on transbronchial lung cryobiopsy, due to the smaller biopsy size and greater potential for sampling error compared with surgical lung biopsy. c)IPF is the likely diagnosis when any of the following features are present: (1) moderate-to-severe traction bronchiectasis and/or bronchiolectasis (defined as mild traction bronchiectasis/bronchiolectasis in 4 or more lobes, with the lingula counted as a lobe, or moderate-to-severe traction bronchiectasis in 2 or more lobes) in a man older than 50 years or a woman older than 60 years; (2) extensive reticulation involving >30% of the lungs on HRCT and age >70 years; (3) increased neutrophils and/or absence of lymphocytosis in bronchoalveolar lavage fluid; or (4) multidisciplinary discussion that produces a confident diagnosis of IPF. d)Indeterminate for IPF remains indeterminate without an adequate biopsy, but it may be reclassified to a more specific diagnosis after multidisciplinary discussion and/or additional consultation when an adequate biopsy is available.

UIP pattern Distribution
- Subpleural and basal predominance
- Often heterogeneous, with areas of normal lung interspersed with fibrosis
- Occasionally diffuse
- May be asymmetric
CT features
- Honeycombing with or without traction bronchiectasis or bronchiolectasis
- Irregular thickening of interlobular septa
- Usually superimposed with a reticular pattern and mild GGO
- May be accompanied by pulmonary ossification
Probable UIP Distribution
- Subpleural and basal predominance
- Often heterogeneous, with areas of normal lung interspersed with reticulation and traction bronchiectasis or bronchiolectasis
CT features
- Reticular pattern with traction bronchiectasis or bronchiolectasis
- May include mild GGO
- Absence of subpleural sparing
Indeterminate for UIP Distribution
- Diffuse distribution without subpleural predominance
CT features
- Features of lung fibrosis not suggestive of a specific etiology
Alternative diagnosis Distribution
- Peribronchovascular predominance with subpleural sparing (consider NSIP)
- Perilymphatic distribution (consider sarcoidosis)
- Upper- or mid-lung predominance (consider fibrotic HP, CTD-ILD, and sarcoidosis)
- Subpleural sparing (consider NSIP or smoking-related IP)
CT features
Lung findings
- Cysts (consider LAM, PLCH, LIP, and DIP)
- Mosaic attenuation or three-density sign (consider HP)
- Predominant GGO (consider HP, smoking-related disease, drug toxicity, and acute exacerbation of fibrosis)
- Profuse centrilobular micronodules (consider HP or smoking-related disease)
- Nodules (consider sarcoidosis)
- Consolidation (consider organizing pneumonia and other conditions)
Mediastinal findings
- Pleural plaques (consider asbestosis)
- Dilated esophagus (consider CTD)

ATS/ERS/JRS/ALAT, American Thoracic Society/European Respiratory Society/Japanese Respiratory Society/Latin American Thoracic Association; UIP, usual interstitial pneumonia; IPF, idiopathic pulmonary fibrosis; IIP, idiopathic interstitial pneumonia; LAM, lymphangioleiomyomatosis.

a)

Includes granulomas, hyaline membranes (except when associated with acute exacerbation of IPF, which may be the presenting manifestation in some patients), prominent airway-centered changes, areas of interstitial inflammation without associated fibrosis, marked chronic fibrous pleuritis, and organizing pneumonia. These features may not be overt or readily apparent to an untrained observer and often require specific attention.

b)

Features that should raise concern for an alternative diagnosis include a cellular inflammatory infiltrate away from areas of honeycombing; prominent lymphoid hyperplasia, including secondary germinal centers; and a distinctly bronchiolocentric distribution, which may incorporate extensive peribronchiolar metaplasia.
