중추신경계 전이암 역학 및 치료의 의미

Abstract

Purpose: Brain metastases are the most common intracranial tumors in adults and represent an increasingly important clinical challenge. This review summarizes the epidemiology of brain metastases and the clinical value of their early detection and treatment.
Current concepts: Brain metastases develop in a substantial proportion of patients with cancer and most commonly arise from lung cancer, breast cancer, and melanoma. However, their true incidence remains difficult to determine.
Discussion and conclusion: Although the overall prognosis of patients with brain metastases remains limited, advances in neuroimaging, stereotactic radiosurgery, and targeted systemic therapies have expanded treatment options and improved intracranial disease control. Early detection and active treatment are important for preventing neurological deterioration, preserving functional status, and enabling continued systemic cancer treatment.

서론

배경

전이성 뇌종양(brain metastases)은 전신 암에서 기원하여 두개골, 뇌실질, 경막, 연수막 등 다양한 두개강 구조로 전이되는 이차성 종양이다[1]. 전이성 뇌종양은 신경학적 증상 유발, 기능 저하, 치료 전략 변화 등 암 환자의 임상 경과에 중대한 영향을 미친다[2]. 또한 전신 치료의 발전과 영상 기술의 향상으로 암 환자의 생존 기간이 연장되면서 전이성 뇌종양의 임상적 중요성은 지속적으로 증가하고 있다. 이에 따라 전이성 뇌종양의 정확하고 포괄적인 역학 자료를 규명하기 위한 연구가 활발히 이루어지고 있다.

목적

이 논문은 전이성 뇌종양의 최신 역학을 기술하고 조기 진단과 치료의 임상적 가치를 평가하고자 한다. 구체적으로 전이성 뇌종양의 발생률, 원발암 부위, 발생 추이 및 기존 역학 자료의 한계를 정리한다. 아울러 신경영상, 수술, 정위방사선수술, 전뇌방사선치료, 최신 표적치료 및 면역치료를 포함한 생존 결과와 최신 치료 전략을 논의한다.

전이성 뇌종양의 역학

전이성 뇌종양은 성인에서 가장 흔하게 진단되는 두개강 내 종양이다[3,4]. 발생 빈도는 원발성 악성 뇌종양보다 최대 10배 높은 것으로 보고된다[5]. 고형암 환자의 약 10–20%는 질병 경과 중 중추신경계 전이를 경험하는 것으로 알려져 있다[6,7]. 인구 기반 연구와 부검 연구에서는 다양한 추정치를 제시하고 있으나, 전이성 뇌종양의 유병률은 일반적으로 인구 10만 명당 8.3–14.3명으로 보고된다[1]. 가장 흔한 원발암은 폐암, 유방암, 흑색종이며[8,9] 발생 부위는 대뇌 반구가 가장 흔하고 이후 소뇌와 뇌간 순으로 나타난다[10,11].
전이성 뇌종양의 발생률은 전반적으로 증가하는 추세로 여겨지며 일부 인구 기반 연구에서도 이러한 경향이 확인된다[1215]. 이러한 증가는 인구 고령화에 따른 암 유병 증가[16], 전신 치료 발전에 따른 생존 기간 연장[1], 그리고 발전된 신경영상 기술로 인한 진단율 증가[17] 등이 복합적으로 작용한 결과로 해석된다.

전이성 뇌종양 역학 연구들의 제한점

전이성 뇌종양의 실제 발생률은 아직 정확히 규명되지 않았다. 기존 역학 연구의 상당수는 현대적 영상기법이나 최신 전신 치료가 보편화되기 이전 자료에 기반하며, 전국 단위 인구 기반 데이터가 충분하지 않은 경우가 많다. 이러한 한계를 보완하기 위해 일부 연구에서는 보험 청구 데이터를 활용하고 있다[18,19].
미국 Surveillance, Epidemiology, and End Results (SEER) 자료는 중추신경계 종양 역학 연구에 널리 활용되며, 미국 인구의 약 26.5%를 대표하는 것으로 보고된다[8]. 그러나 SEER는 원발암 진단 당시 존재하는 동시성 전이성 뇌종양(synchronous brain metastases)만 포함하며, 원발암 진단 이후 발생하는 시차성 전이성 뇌종양(metachronous brain metastases)은 포함하지 않는다[9]. 이로 인해 상당수의 전이성 뇌종양 환자가 분석에서 누락될 수 있음이 지적되고 있다. 특히 원발암의 종류에 따라 시차성 전이성 뇌종양이 발생하는 시기가 상이하며, 이는 보고되는 원발암별 전이성 뇌종양의 발생률과 생존 결과 해석에 영향을 미칠 수 있다. 예를 들어 폐암의 경우 원발암 진단 후 비교적 이른 시기에 뇌전이가 발견되는 반면, 유방암과 대장암에서는 수년의 잠복기를 거친 후에 뇌전이가 진단되는 경우가 많다[20,21].
또한 원발암 종류에 따라 뇌 영상 선별검사 지침이 달라 전이성 뇌종양 발생률에 영향을 미칠 수 있다. 예를 들어 소세포폐암에서는 자기공명영상(magnetic resonance imaging, MRI) 기반 뇌 선별검사가 권고되지만[22], 유방암 등에서는 모든 환자에서 권고되지 않는다[23]. 영상기법 역시 중요한 변수로, 컴퓨터단층촬영(computed tomography)은 작은 병변에 대한 민감도가 낮으며, 정확한 진단을 위해서는 1.5 mm 이하의 얇은 절편의 조영증강 MRI가 필요하다[24]. 국가별 의료 정책과 영상 접근성의 차이 또한 보고되는 전이성 뇌종양 발생률에 영향을 미칠 수 있다.

전이성 뇌종양 관련 역학 연구

Parker 등[8]의 SEER 기반 연구에서는 2015–2019년 미국에서 동시성 전이성 뇌종양 발생률이 인구 10만 명당 7.1명으로 보고되었다. 연령별 발생률은 소아(0–14세) 0.14명, 청소년·젊은 성인(15–39세) 0.51명, 40세 이상 성인 16.03명이었다. 원발암별로는 폐암이 가장 흔했고 이후 흑색종과 유방암 순이었다. 소아에서 가장 흔한 원발암은 신장암이었다.
Jiang 등[25]은 미국 의료기관의 비식별화된 전자의무기록과 행정 청구 자료를 포함한 TriNetX Research Network를 이용하여 시차성 전이성 뇌종양을 포함한 대규모 분석을 수행하였다. 총 11,497,663명의 환자 중 300,863명(2.6%)에서 전이성 뇌종양이 발생했으며 이 중 62.2%가 시차성 전이였다.
대한민국에서는 Kim 등[26]이 국민건강보험공단의 샘플 코호트 자료를 이용하여 전이성 뇌종양의 역학을 분석하였다. 9년의 관찰 기간(2005–2013; 8,725,438인년) 동안 총 776명에서 전이성 뇌종양이 발생하였다. 전이성 뇌종양의 연령 표준화 발생률(age-standardized incidence)은 10만 인년(person-years)당 8.2명으로 보고되었으며, 연간 약 4,540명의 신규 환자가 발생하는 것으로 추정되었다. 주요 원발암은 폐암이었으며, 폐암 환자에서 연간 약 2,440건의 전이성 뇌종양이 발생하는 것으로 추정되었다. 특히, 간암 기원의 전이성 뇌종양이 6.2%로 비교적 높은 비중을 차지하였는데, 이는 국내 간세포암의 높은 발생률과 생존 기간 연장 추세가 반영된 결과로 해석된다[27].
2010–2017년 캐나다의 Canadian Cancer Registry 자료를 이용한 연구[28]에서는 원발암 진단 당시 전이성 뇌종양이 동반된 환자 수가 연평균 약 3,227명으로 보고되었다. 원발암 중에서는 폐암에서 전이성 뇌종양 동반 비율이 가장 높았으며 그 비율은 9.42%였다.
한편 단일 미국 3차 의료기관 데이터를 분석한 연구[29]에서는 유방암과 흑색종 유래 전이성 뇌종양 비율은 감소하는 반면 전립선암, 난소암, 방광암 등 과거 비교적 드물었던 원발암에서 기원한 전이성 뇌종양 비율이 증가하는 경향이 보고되었다. 이러한 변화는 향후 전이성 뇌종양 선별검사, 예후 예측, 치료 전략에 새로운 접근이 필요할 수 있음을 시사하였다.

전이성 뇌종양의 생존 결과

기존 전신 치료는 혈액뇌장벽(blood–brain barrier)을 충분히 통과하지 못하는 경우가 많아 방사선 치료와 수술적 절제와 같은 국소 치료가 전이성 뇌종양 치료의 중심을 이루어 왔다[30]. 그러나 최근 표적 치료와 면역 치료의 발전으로 일부 전이성 뇌종양 환자, 특히 폐암 환자에서는 전신 치료가 초기 치료 옵션으로 고려되고 있다[6,31]. 또한 5–20개의 다발성 전이성 뇌종양 환자를 대상으로 한 연구에서 정위방사선수술이 기존 해마 회피 전뇌방사선치료와 비교하여 전체 생존율은 유사하면서도 기능적 독립성과 수행 상태가 더 잘 보존된다는 연구들이 보고되며[32], 보다 적극적으로 적용되는 추세이다. 그러나 이러한 치료의 발전에도 불구하고 전이성 뇌종양 환자의 예후는 전반적으로 여전히 불량하다. SEER 기반 연구에서는 대부분의 원발암에서 동시성 전이성 뇌종양 환자의 중앙 생존 기간이 12개월 이하로 보고되었다[33]. 특히 65세 이상 시차성 전이성 뇌종양 환자에서는 중앙 생존이 4개월 이하로 더욱 불량한 것으로 보고되었다[34]. 반면 임상시험 기반 Graded Prognostic Assessment 연구에서는 중앙 생존이 8–16개월로, 조금 더 좋은 것으로 보고되었는데[35], 이는 환자 선별 및 선택 편향이 반영된 결과일 가능성이 있다.

연수막 전이의 역학 및 생존 결과

연수막 전이(leptomeningeal disease)는 종양 세포가 연수막과 뇌척수액 공간을 따라 확산되는 형태의 전이를 뜻한다[36]. 약 2–12%에서 초기 전이성 뇌종양 진단 시 확인되며, 질환 경과 중 1–37%에서 새롭게 발생할 수 있다고 보고되었다[9]. 수술 후 경막 전파(postsurgical pachymeningeal seeding)는 연수막 전이와 감별이 필요하며 이는 전이성 뇌종양 절제술 후 약 8%에서 발생하는 것으로 보고된다[37]. 연수막 전이 치료에는 비정위 방사선 치료, 뇌척수액 내 항암치료, 뇌척수액 배액 등이 포함된다. 그러나 예후는 매우 불량하며 치료 시 중앙 생존 기간은 약 4–6개월, 치료하지 않을 경우 약 4–6주로 보고된다[38].

전이성 뇌종양의 조기 발견 및 치료의 임상적 의의

전이성 뇌종양의 치료는 두개강 내 국소 조절에 중점을 두며, 이러한 치료는 중요한 임상적 의의를 가진다. 치료하지 않은 전이성 뇌종양은 진행성 신경학적 결손, 두개내압 상승, 그리고 신경학적 합병증으로 인한 사망을 초래할 수 있다[13,39,40]. 적극적인 치료를 통해 신경 기능을 보존하는 것은 전이성 뇌종양 환자가 추가적인 전신 치료를 받을 수 있는 상태를 유지하는 데 중요하다. 또한 이러한 전신 치료는 두개강 내 병변의 국소 조절에도 추가적인 치료 효과를 보일 수 있다[41]. 최근 연구에서는 신경학적 증상 부담(neurological symptom burden)의 중요성이 강조되고 있다[42]. Steindl 등[43]은 유방암 전이성 뇌종양 환자에서 신경학적 증상 부담이 생존의 독립적인 예측 인자임을 보고하였다. 또한 적절한 신경인지 기능의 유지는 암 환자의 삶의 질과 밀접하게 관련되는 것으로 알려져 있다[44].
치료받지 않은 전이성 뇌종양의 예후는 중앙 생존 기간이 1–2개월에 불과할 정도로 극히 불량한 것으로 보고된다[45]. 전뇌방사선치료를 시행할 경우 약 2–4개월의 생존 연장을 기대할 수 있으며, 적극적인 수술적 절제와 다학제 치료를 병행할 경우 환자군에 따라 중앙 생존 기간이 7.5개월에서 20개월 이상까지 뚜렷하게 향상되는 것으로 보고된다[46]. 또한, 현대 치료 전략 중 하나인 정위방사선수술은 특히 다른 치료와 병합될 경우 높은 국소 종양 조절률을 보이는 것으로 보고되고 있다[47,48]. 또한 표적 전신 치료의 발전은 비소세포폐암, 유방암, 흑색종 등 전이성 뇌종양 발생 위험이 높은 특정 암종에서 치료 성적을 향상시키고 있다[30]. 따라서 전이성 뇌종양의 조기 발견은 환자의 이환율과 사망률 감소뿐 아니라 이차적으로 치료 관련 독성을 줄이는 데에도 기여할 수 있다[33]. 이러한 이유로 전이성 뇌종양의 조기 발견과 적극적인 다학제 치료는 여전히 중요한 암 치료 전략으로 강조되고 있다.

결론

암 환자의 생존 연장과 영상 기술의 발전으로 전이성 뇌종양의 임상적 중요성은 지속적으로 증가하고 있다. 전이성 뇌종양 치료는 신경학적 악화를 예방하고 기능적 상태를 유지하며 전신 치료의 지속을 가능하게 한다. 또한 신경외과적 치료, 방사선 치료, 표적 전신 치료의 발전으로 치료 선택지가 확대되면서 두개강 내 질환 조절 또한 향상되고 있다. 따라서 전이성 뇌종양의 조기 발견과 다학제적 접근은 앞으로도 중요한 임상적 요소로 강조될 것이다.
