Trans Abstract

Background

In contrast to type 2 diabetes, type 1 diabetes mellitus (T1DM) requires insulin treatment to control blood glucose. As the incidence and prevalence of T1DM have steadily increased; therefore, T1DM is increasingly being diagnosed not only in children and adolescents, but also in adults. Therefore, the importance of accurate diagnosis and optimal management of T1DM is being recognized in clinical practice.

Current Concepts

T1DM is caused by insulin deficiency, following the destruction of insulin-producing pancreatic β-cells. Diagnosis of diabetes is based on the following criteria: fasting blood glucose levels ≥126 mg/dL, random blood glucose levels ≥200 mg/dL accompanied by symptoms of hyperglycemia, an abnormal 2-hour oral glucose tolerance test, or glycated hemoglobin ≥6.5%. Accurate diagnosis of T1DM based on patients’ clinical characteristics, serum C-peptide levels, and detection of autoantibodies against β-cell autoantigens is important for optimum care and to avoid complications. A target glycated hemoglobin level is recommended in children, adolescents, and young adults with access to comprehensive care. The availability of insulin analogues and mechanical technologies (insulin pumps and continuous glucose monitors) has improved the management of T1DM, and these are useful for the prevention of microvascular complications. Screening for microvascular complications should commence at puberty or 5 years after diagnosis of T1DM.

Discussion and Conclusion

Effective cooperation and coordination between patient, parents, and healthcare providers are necessary to achieve a successful transition from pediatric to adult care in patients with T1DM. Diabetic management for T1DM should be individualized based on patients’ lifestyle, as well as psychosocial, and medical circumstances.

서론

제1형 당뇨병은 유전적 감수성이 있는 환자에서 자가면역 기전에 의해 췌장 베타세포가 파괴되어, 이로 인한 인슐린 결핍으로 발생하는 질환이다[1]. 소아청소년 연령에서 발병하는 가장 흔한 당뇨병으로 전체 당뇨병 환자 중 5-10%를 차지한다고 알려져 있다[2]. 북유럽 특히 스칸디나비아 국가들과 미국에서 발병률 및 유병률이 높으며 우리나라를 포함한 아시아에서는 상대적으로 발병률 및 유병률이 낮다. 그러나 소아청소년 연령에서는 전 세계적으로 연간 3% 정도의 발병률 증가를 보이고 있고[3], 우리나라에서도 제1형 당뇨병의 발병률이 꾸준히 증가하고 있다[4]. 가장 최근의 연구에 따르면 2007-2017년 기간 동안 소아청소년 제1형 당뇨병 발병률은 매년 3-4%씩 꾸준히 증가하여 2008년 10만명당 3.70명에서 2016년 4.77명이었다[4]. 제1형 당뇨병 발병률의 증가와 함께 유병률도 증가하고 있는데, 이는 소아청소년 연령에 진단받은 환자들의 수명이 길어졌기 때문이다. 우리나라 제1형 당뇨병의 유병률은 2017년 인구 10만 명당 41.03명으로 2007년 32.85명보다 증가하였다[4].

제1형 당뇨병은 제2형 당뇨병과는 달리 췌장 베타세포의 파괴로 체내 인슐린이 분비되지 않기 때문에 혈당조절을 위해서는 인슐린 치료가 필수적이다. 제1형 당뇨병은 아직까지 완치할 수 있는 방법은 없으나 관리가 가능하며, 혈당조절을 통하여 합병증의 발생을 예방하는 것이 중요한 질환이다. 1921-1922년 인슐린이 발견된 이후로 100년 동안 제1형 당뇨병의 치료는 지속적으로 발전하였다. 그 결과 현재는 여러 종류의 인슐린과 함께 인슐린펌프, 연속혈당측정기와 같은 기계들이 제1형 당뇨병 환자에서 혈당을 정상 범위로 관리하는 데 사용되고 있다.

제1형 당뇨병의 적절한 관리를 위해서는 우선 제1형 당뇨병의 정확한 진단이 중요하다. 제1형 당뇨병은 제2형 당뇨병과는 달리 현재로서는 인슐린 치료가 필수적이기 때문에, 정확한 진단을 통해 제2형 당뇨병과 감별하고, 인슐린 치료를 통해 혈당을 잘 관리하여 최대한 합병증이 발생하지 않도록 하는 것이 중요하다. 소아청소년의 경우 제1형 당뇨병의 진행이 빠른 경우가 많고 진단 당시 전형적인 증상이 있는 경우가 많기 때문에 발병부터 진단까지 소요되는 시간이 길지 않을 수 있으나, 성인의 경우에는 소아청소년보다는 진행이 늦은 경우가 많다고 알려져 있다[5,6]. 따라서 이 논문에서는 소아청소년 제1형 당뇨병의 진단과 관리에 대해 2018년 국제소아청소년당뇨병협회(International Society for Pediatric and Adolescent Diabetes)에서 발표한 소아청소년 당뇨병 가이드라인[7,8], 2021년 미국당뇨병협회(American Diabetes Association, ADA)의 당뇨병 치료 가이드라인[6,9,10]을 토대로 살펴보고자 한다.

당뇨병의 진단

제1형 당뇨병은 여러 종류의 당뇨병 중 하나이다. 먼저 당뇨병임을 진단하고, 그 다음으로 환자의 임상양상과 검사 결과를 바탕으로 다른 원인의 당뇨병과 감별하여 제1형 당뇨병임을 진단하게 된다. 당뇨병의 진단기준은 혈당 측정과 증상의 유무를 기반으로 한다. 당뇨병을 진단하기 위한 검사 방법으로 과거에는 공복혈당과 75 g 경구 당부하검사 2시간 후의 혈당이 사용되었다[5]. 1997년 당뇨병 진단기준이 재개정 될 때, 당뇨병성 망막병증의 유무와 혈당과의 관련성을 중심으로 기준을 정하였고, 이 결과 장시간 지속되는 식후 2시간 혈당이 200 mg/dL 이상일 경우 당뇨병으로 진단하기로 결정하였다[5,11]. 이 수치는 당뇨병성 막막병증의 유병률을 증가시키는 변곡점에 해당하며, 공복혈당 및 경구 당부하검사 2시간 후의 혈당, 당화혈색소와 유의한 관련이 있다[5].

당뇨병을 진단할 때, 당화혈색소를 사용하는 방법은 공복혈당을 사용하는 것보다 몇 가지 장점이 있다. 검사를 위해 금식할 필요가 없으므로 검사 전의 안정성이 좋다고 할 수 있으며, 스트레스나 질병에 의한 일중, 일간의 변동도 심하지 않은 편이다. 그러나 혈색소증이나 특정 빈혈이 있는 경우에는 당화혈색소의 결과가 실제 혈당 수준과는 다를 수 있으므로 판정에 주의해야 한다. 당화혈색소는 National Glycohemoglobin Standardization Program에서 공인된 방법으로 시행하고 표준화되어야 한다. 당화혈색소 6.5% 이상이라는 당뇨병 진단기준이 성인 및 소아청소년 연령에서 사용되고 있으나, 소아청소년 연령에서 진단기준으로 당화 혈색소 6.5% 이상이라는 진단기준을 단독으로 사용할 수 있는지에 대해서는 아직 불명확한 상태로 추가적인 연구가 필요하며[12], 특히 소아청소년의 제1형 당뇨병을 진단하는데 당화혈색소 6.5% 이상이라는 기준을 단독으로 사용하는 것은 적절하지 않다[8]. 당뇨병의 진단기준을 종합하면 Table 1과 같다[8].

제2형 당뇨병과의 다른 제1형 당뇨병의 특성

당뇨병의 진단기준은 제1형과 제2형 당뇨병이 동일하다. 하지만 제1형 당뇨병은 치료와 교육에 있어 제2형 당뇨병과 큰 차이가 있기 때문에 제2형 당뇨병과 감별하는 것이 임상적으로 매우 중요하다[8]. 그러나 제1형 당뇨병은 소아청소년이나 성인 모두에서 발병할 수 있고, 소아청소년 연령에서는 급성으로 심한 증상을 동반하는 경우가 흔하지만, 성인에서 발병하는 경우는 서서히 진행하는 양상을 보여 초기 증상이 제2형 당뇨병과 뚜렷하게 구분되지 않는 경우가 많아서 제1형 당뇨병을 정확히 진단하는 것이 쉽지 않다[5].

제1형과 제2형 당뇨병의 감별은 임상양상, 췌장 베타세포에 대한 자가항체 존재 여부, 가족력 등을 참고로 한다. 일반적으로 마른 체형의 사람이 전형적인 고혈당의 증상을 보이면서 당뇨병의 가족력이 없는 경우에는 제1형 당뇨병을 더 시사한다. 반대로 과체중 이상의 사람이 대사증후군이 동반되면서 당뇨병의 가족력이 있을 경우에는 제2형 당뇨병을 더 시사한다. 하지만, 최근 들어 비만이 증가하는 것과 제2형 당뇨병 환자 일부에서 당뇨병케톤산증이 발생하는 것이 제1형과 제2형 당뇨병의 감별을 어렵게 한다[13,14]. 제1형 당뇨병은 자가면역성 질환의 일종이기 때문에, 췌장 베타세포와 관련된 자가항체의 양성 여부를 확인하는 것이 진단에 중요하다. 제1형 당뇨병으로 진단되는 환자의 90% 정도에서 진단 시에 인슐린 자가항체, glutamic acid decarboxylase 항체, protein tyrosine phosphatase 항체, zinc transporter protein 8 항체 중 한 가지 이상이 발현하는 것으로 알려져 있다[15-18]. 그러나 아시아인들의 경우 제1형 당뇨병 환자에서 자가항체 양성률이 낮은 것으로 알려져 주의가 필요하다[19-21]. 췌장의 인슐린 분비 능력을 반영하는 공복 혈청 C-peptide 농도가 제1형 당뇨병과 제2형 당뇨병을 감별하는 데 사용될 수 있는데[22], 소아청소년 연령에서는 혈청 C-peptide 농도가 당뇨병 진단 후 첫 1년 동안은 제1형 당뇨병에서도 감소되지 않아 제2형 당뇨병과의 감별이 어려운 경향이 있으며, 진단 시 연령과 무관하게 혈청 C-peptide가 진단 후 40년 후에도 발견되었다는 보고도 있어[23], 제1형 당뇨병의 진단에 있어 절대적이지는 않다. 따라서 제1형 당뇨병의 진단은 당뇨병의 진단기준을 만족하면서, 임상양상과 자가항체 양성 여부, 혈청 C-peptide의 감소 등의 소견을 종합해서 이루어져야 한다[21].

제1형 당뇨병의 관리

고혈당은 당뇨병을 진단하는 기준이 되며 당뇨병 합병증의 발생과도 직접 관련이 있다[21]. 따라서 혈당을 조절하는 것이 당뇨병 관리의 가장 기본이다[21]. 제1형 당뇨병은 아직까지 완치할 수 있는 방법은 없으나, 관리가 가능하며, 적절한 혈당조절을 통하여 합병증의 발생을 예방하는 것이 중요한 질환이다. 100년 전 인슐린의 개발로 치료방법이 없는 절망적인 질환에서 관리 가능한 질환이 되었으며, 이후 의학과 과학기술의 발전으로 여러 종류의 인슐린과 인슐린 펌프 및 연속혈당측정기기를 사용하여 혈당을 정상에 가까운 상태로 조절할 수 있게 되었다. 이상적인 혈당 관리를 위해서는 생활습관, 정신사회적, 의학적인 상황들이 종합적으로 고려되어야 하며, 인슐린 투여, 식단 관리, 꾸준한 운동의 세 요소가 함께 해야 한다. 또한 제1형 당뇨병 환자는 물론이고, 환자의 가족, 더 나아가 사회가 함께 참여해야 한다.

2005년 ADA에서 발표한 권고사항에서는 혈당조절의 목표를 소아청소년 연령에서는 연령별로 차이를 두어 당화혈색소 기준으로 6세 미만에서는 <8.5%, 6-12세에서는 <8.0%, 13-19세에는 <7.5%였다[24]. 이는 당화혈색소를 낮게 유지하려 할 때, 저혈당의 빈도가 증가하는 경향을 보인다는 연구에 근거하였다[25,26]. 반복되는 심한 저혈당은 소아청소년에서 신경인지기능의 장애와 관련이 있을 것이라는 연구가 있었다[27,28]. 그러나 이후 연구에서 반복되는 저혈당과 신경인지기능 저하 사이의 연관성이 없다고 발표하였고[29,30], 사춘기 전부터 혈당을 잘 조절하는 것이 추후 미세혈관 및 대혈관 합병증의 발생을 감소시킬 것이라고 생각되며, 고혈당 및 혈당의 변동 폭이 큰 것이 단기간의 신경인지기능과 중추신경계에 부정적인 영향을 초래한다는 연구가 있다[31,32]. 인슐린 다회주사요법이나 인슐린 펌프를 사용하여 인슐린 집중치료를 할 경우 저혈당의 위험을 높이지 않는다고 밝혀졌다[33-35]. 이러한 결과들을 바탕으로 현재는 제1형 당뇨병의 혈당조절 목표를 연령에 상관없이 당화혈색소 <7.0%로 권고하고 있다[7,10]. 혈당 목표치는 식전 70-130 mg/dL, 식후 90-180 mg/dL, 자기 전 80-140 mg/dL로 유지하는 것을 추천하고 있다[7]. 최근 연속혈당측정기의 사용으로 혈당 관리에서 당화혈색소와 자가혈당측정기로 측정한 혈당 수치만큼 중요하게 생각되고 있는 것이 time in range (TIR), time below range (TBR), time above range (TAR)이다. 연속혈당측정기를 사용한 혈당조절의 목표는 TIR >70%, TBR <5%, TAR<5%이다[21,36-38]. 자가혈당측정기로 측정한 혈당은 혈당을 측정한 그 시점의 상태만 확인할 수 있으며, 당화혈색소는 약 3개월의 평균 혈당을 반영하는 수치이지만, 같은 수치의 당화혈색소에서도 혈당의 변동 폭은 다양할 수 있어 평소 혈당의 변동 상태를 파악하기에는 한계가 있다. 연속혈당측정기는 기기를 부착하고 있는 상태에서는 실시간으로 혈당의 변화를 확인하는 것이 가능하므로, 제1형 당뇨병 환자의 혈당 관리에서 혈당을 목표혈당범위에 맞추는 데 큰 도움이 되고, 장기적으로 합병증의 예방에 도움이 된다[21].

1921-1922년 인슐린이 발견된 이래, 다양한 종류의 인슐린이 개발되어 당뇨병 환자의 혈당조절에 사용되고 있다. 제 1형 당뇨병 환자는 혈당조절을 위해 인슐린 투여가 필수적이며, 체내 인슐린 분비와 가장 유사한 형태로 인슐린을 투여하는 것이 이상적인데, 크게 인슐린 다회주사요법과 인슐린펌프를 사용하여 인슐린을 투여하는 방법 두 가지가 있다. 인슐린 다회주사요법은 지속형 인슐린 주사로 기저 인슐린 농도를 유지하고, 탄수화물 섭취를 기준으로 매 식사 전 초속효성 인슐린을 주사하여 혈당을 조절하는 방법이다[39]. 인슐린펌프(continuous subcutaneous insulin infusion)는 생리적인 인슐린 분비와 가장 유사하게 인슐린을 피하에 공급할 수 있는 기기이다. 인슐린펌프의 사용은 인슐린 주사에 비해 심한 저혈당 및 당뇨병성케톤산증 발생위험을 낮추고 혈당조절에 도움을 준다[40]. 인슐린펌프와 연속혈당측정기를 연결하여 사용하게 되는 센서 연동형 인슐린펌프(sensor augmented pump)의 사용은 성인과 소아 모두에서 인슐린 다회주사요법과 비교하여 저혈당의 위험을 증가시키지 않으면서 당화혈색소 수치를 크게 개선시켰다[41,42]. 또한 당뇨병 환자의 삶의 질 측면에서도 환자 및 보호자 모두 인슐린펌프 치료가 긍정적 영향을 준다고 보고하였다[43,44]. 최근 우리나라 소아청소년 제1형 당뇨병 환자들을 대상으로한 연구에서도 인슐린펌프 치료를 일찍 시작하고 지속적으로 교육할수록 혈당조절 및 당뇨병 관리에 효과적이라고 보고하였다[35].

이와 같이 혈당조절을 잘 하는 것은 당뇨병 합병증의 발생을 예방하기 위해서이다. 일반적으로 유병기간이 길수록 합병증의 발생률이 증가한다고 알려져 있지만, 혈당을 잘 관리하면, 수십 년간 합병증이 발생하지 않고 지낼 수 있다는 보고도 있다[45]. 제1형 당뇨병 환자에서 소아청소년 시기에 당뇨병 합병증의 유병률은 낮다. 그러나 유병기간이 긴 경우, 나이가 많은 경우, 사춘기인 경우가 합병증 발생의 위험 요소이다[46]. 제1형 당뇨병의 만성 합병증은 당뇨병성 신증, 당뇨병성 망막증 및 백내장, 당뇨병성 신경병증(말초 및 자율신경계)으로 대표되는 미세혈관 합병증과 심혈관계질환으로 대표되는 대혈관 합병증, 그 외 제1형 당뇨병 환자에서 발생할 수 있는 기타 합병증으로 나눌 수 있다. 합병증에 대한 선별검사의 시작은 만 11세부터 시작하는 것을 추천하며, 어린 나이에 발병한 제1형 당뇨병 환자의 경우, 발병 후 5년째부터는 사춘기 이전의 연령이라도 만성 합병증에 대한 선별검사를 시작하여 합병증 발생 여부를 확인하고 필요한 관리를 하도록 한다[46].

제1형 당뇨병은 주로 소아청소년기에 발병하여 생애 발달 단계를 거쳐 성인기, 노년기에 이르기까지 평생에 걸쳐 지속적인 관리가 필요한 질환이다. 최근 소아청소년의 제1형 당뇨병 발병률 및 유병률이 증가함에 따라 소아청소년에서 성인으로 이행하는 환자도 증가하고 있다. 적절한 이행을 위해서는 환자, 보호자, 의료진 모두가 이를 위한 여러 가지 준비를 해야 한다. 소아청소년 제1형 당뇨병 환자들의 성인기로의 적절한 이행에 대하여 2011년 ADA와 전문가 패널이 권장사항을 발표하였고[47], 2018년 국제소아청소년당뇨병협회의 청소년 당뇨병 가이드라인[48]과 2021년 발표된 ADA 가이드라인의 소아청소년 파트[9]에서도 모두 적절한 이행에 대한 내용을 포함하고 있다.

결론

제1형 당뇨병을 진단하기 위해서는 당뇨병의 진단기준에 부합해야 하며, 이것은 공복혈당, 당부하검사 후 혈당, 당화혈색소 등의 기준에 의한다. 당뇨병으로 진단된 후 제2형과 감별하여 제1형 당뇨병으로 진단하는 것은 임상양상과 자가항체 양성 여부, 혈청 C-peptide 농도 등을 종합해서 결정해야 한다. 당뇨병 관리의 목표는 혈당조절을 잘하여 합병증을 예방하는 것이다. 혈당조절의 목표로 당화혈색소 <7.0%가 추천되며, 연속혈당측정기를 사용할 경우 TIR >70%, TBR <5%, TAR <5%가 추천된다. 혈당조절을 위해서는 식단조절, 인슐린 투여, 규칙적인 운동 세 가지가 모두 함께 지켜져야 하며, 사춘기부터는 만성 합병증에 대한 선별검사를 시작하여 합병증 발생 여부를 확인하고 필요한 관리를 하도록 한다. 또한 제1형 당뇨병은 대부분 소아청소년 시기에 발병하여 평생에 걸쳐 지속적인 관리가 필요한 질환으로, 청소년기에서 성인기로 이행하는 시기에 환자, 보호자, 의료진이 적절한 이행이 이루어질 수 있도록 노력해야 한다. 제1형 당뇨병은 정확하게 진단하는 것이 매우 중요하며, 진단 후에는 혈당을 잘 관리하여 환자가 삶의 질을 유지하면서, 최대한 합병증의 발병을 예방할 수 있도록 해야 한다.

제1형 당뇨병은 인슐린의 결핍에 의한 당뇨병으로 주로 소아청소년기에 발병한다. 혈당 관리를 위해서는 인슐린 투여가 필수적이며 이 때문에 제2형 당뇨병과의 감별이 중요하다. 혈당을 목표 범위 내로 관리하여 합병증을 예방하는 것이 제1형 당뇨병 치료의 목표가 된다. 이 논문에서는 소아청소년 제1형 당뇨병의 진단 및 치료에 대해 최신 논문을 체계적으로 정리하여 설명해 주고 있다. 제1형 당뇨병의 진단, 제2형 당뇨병과의 감별점, 관리의 목표와 방법, 연관 합병증 및 성인기로의 이행 등을 포괄적으로 기술하고 있다. 2018년 국제 소아청소년당뇨병협회 및 2021년 미국 당뇨병협회의 당뇨병 진료지침이 기존의 진료지침들과 달라진 점을 제시하고, 지침 변화의 근거가 된 주요 연구 결과도 적절하게 제시하고 있다. 이 논문은 소아청소년 제1형 당뇨병의 진단 및 관리에 대한 좋은 지침이 될 것으로 판단된다.

1. Classic symptoms of diabetes or hyperglycemic crisis, with plasma glucose concentration ≥200 mg/dL (11.1 mmol/L).

2. Fasting plasma glucose ≥126 mg/dL (7.0 mmol/L). Fasting is defined as no caloric intake for at least 8 hours.a)

3. Two-hour postload glucose ≥200 mg/dL (11.1 mmol/L) during an OGTT.a)
The test should be performed using a glucose load containing the equivalent of 75 g anhydrous glucose dissolved in water or 1.75 g/kg of body weight to a maximum of 75 g.

4. HbA1c ≥6.5%b)
The test should be performed in a laboratory using a method that is NGSP certified and standardized to the DCCT assay.

In the absence of unequivocal hyperglycemia, the diagnosis of diabetes based on these criteria should be confirmed by repeat testing.

A value of less than 6.5% doses not exclude diabetes diagnosed using glucose tests. The role of HbA1c alone in diagnosing type 1 diabetes in children is unclear.
