Trans Abstract

Background

Occipital neuralgia is defined as paroxysmal shooting, or stabbing pain in the posterior part of the scalp, in the distribution of the greater and lesser occipital nerves. Occipital neuralgia may present only as an intermittent stabbing pain, but different opinions exist on its cause and diagnostic criteria.

Current Concepts

According to the latest version of headache classification, only paroxysmal stabbing pain is included in the diagnostic criteria, and persistent aching pain is excluded. Pain intensity was also limited to severe cases. It has therefore become difficult to classify existing occipital neuralgia, whose main symptom is persistent pain rather than paroxysmal stabbing pain. Occipital neuralgia is classified as either idiopathic or secondary. Secondary occipital neuralgia is caused by structural lesions innervating the trigeminocervical complex (TCC) in the upper spinal cord, the dorsal root of second cervical cord, and the greater occipital nerve (GON).

Discussion and Conclusion

Although idiopathic occipital neuralgia has no cause, the entrapment of the GON in the tendinous aponeurotic attachment of the trapezius muscle at the superior nuchal line has recently been proposed as an etiology. Chronic, irritating afferent input of occipital neuralgia caused by entrapment of the GON seems to be associated with sensitization and hypersensitivity of the second-order neurons in the TCC receiving convergent input from trigeminal and occipital structures. TCC sensitization induces referred pain in the facial trigeminal area.

서론

후두신경통(occipital neuralgia)은 쑤시고 찌르는 통증이 뒤통수의 뒤통수신경(큰뒤통수신경, 작은뒤통수신경, 셋째뒤통수신경) 분포 영역에 발생하는 것을 의미한다. 후두신경통은 가끔 통증 부위의 감각저하와 이상감각이 동반되며 침범 신경의 압통이 흔히 동반된다[1-3]. 후두신경통은 뒤통수에 순간적으로 발생하는, 특징적인 찌르는 듯한 통증으로 인해 임상적으로 쉽게 의심할 수 있고, 국소마취제를 이용한 후두신경차단술로 쉽게 진단이 가능하다. 그럼에도 불구하고, 후두신경통의 빈도는 정확히 알려져 있지 않다[4].

두통과 신경통의 분류와 진단은 아직도 어렵다. 후두신경통 환자들을 보면, 뒤통수와 측후두부에 조이고 누르는 듯한 두통이 존재하면서, 동시에 간혹 찌르고 저리는 듯한 전기 흐르는 느낌의 간헐성 통증이 동반되는 경우가 대부분이기 때문이다. 이 경우, 현재 국제두통학회 분류에 의하면 머리 한쪽의 두통이 심한 경우 편두통, 양측성의 중등도 통증은 긴장형두통으로 분류되기 때문이다[3]. 안면의 신경통과 달리, 현재 두통은 두통의 위치에 따라 분류되지 않는다[3]. 만약 측후두 두통이 간헐적 후두의 찌르는 통증을 동반하면 서, 목과 어깨의 방사통을 동반하면 분류는 더욱 어렵다. 게다가 이 환자들이 눈과 턱의 불편함을 동시에 호소하는 경우, 두통과 후두신경통, 안면신경통의 가능성을 모두 염두에 두고 분류해야 하므로 진단은 더욱 어려워진다[5,6]. 이 논문에서는 후두신경통 관련 최신 문헌고찰을 통해, 후두신경통이 뒤통수에 찌릿찌릿한 단순한 통증이 아님을 고찰하고자 한다.

후두신경통 진단기준 변화와 문제점

후두신경통의 진단기준은 최근 매우 엄격해졌다. 후두신경통에 대한 국제두통학회 진단기준은 1998년 1판, 2004년 2판, 2018년 3판으로 개정되면서 많이 바뀌었다(Table 1) [1-3]. 국제두통학회에 의한 후두신경통의 정의를 추이를 살필 수 있도록, 연대별로 Table 1에 정리하였다[1-3]. 2판과 3판을 거치면서 안면의 신경통(cranial neuralgia)에 대해서는, 국제통증연구학회(International Association of Study for Pain)의 의견을 반영하여 작성되었다고 한다[3].

1998년 후두신경통의 정의는 간헐적인 뒤통수의 찌르는 통증이 주된 증상이고, 동시에, 순간적인 찌르는 통증 사이에 일반적인 욱신거리는 통증(aching pain)이 지속적으로 발생할 수 있음이 진단기준에서 제시되어 있다[1]. 지속적인 욱신거리는 통증은 2004년 2판 분류에서는, 동반될 수도 있고 없을 수도 있음으로 중요성이 강조되지 않았다. 2018년 세 번째 분류에서는 지속성인 통증이 완전히 제외되었다. 삼차신경통에서 나타나는 전형적인 간헐적인 찌르는 통증에만 국한되도록 진단기준이 변경되었다. 게다가 통증의 세기에 대해 통증이 심한 경우, 즉 수치통증척도(numerical rating scale) 6 이상에 한정되었다(Table 1) [1-3]. 통증과 동반되는 이학적 소견도 강조되었다. 그러므로 후두신경통은 심한 찌르는 통증에만 국한되는 것으로 정의된다[3]. 욱신거리고 조이는 듯한 지속통증은 진단기준에 제외되었기 때문에, 저자의 경우, 3판 진단기준에 의한 명확한 후두신경통의 빈도는 현저히 감소하였다. 또한 안면과 두개신경통 분류에는 시간 흐름에 따른 증상 변화에 대한 기준이 없다. 측후두부의 지속성인 통증이 반복되다가, 마침내 후두에 특징적인 찌르는 통증이 간헐적으로 발생할 경우 일차두통과 후두신경통의 진단이 중복된다.

뒤통수의 찌릿찌릿한 통증은 비교적 흔한 증상이다. 실제로, 반복적으로 찌릿찌릿한 뒤통수통증은 대부분 측후두의 지속적인 통증과 동반되는 경우가 흔하다. 그래서 후두신경통으로 진단하기 어렵다. 더욱이 정확한 후두신경통 진단기준은 통증 세기가 심한 경우에 국한되고, 압통과 이상감각을 동반해야 후두신경통의 기준에 부합하기 때문에 후두신경통으로 진단하기 곤란하다[3]. 그래서 대부분 일반적인 두통 분류를 사용해야 한다. 한편, 대표적인 안면신경통인 삼차신경통의 경우, 전형적으로 간헐적으로 찌르는 통증과 함께, 통증 분지의 지속성 통증이 동반될 수 있음이 명시되어 있다[3].

후두신경의 해부학

척추신경은 해당 척추의 신경공을 통해 나오며 큰 배쪽가지(ventral, anterior ramus)와 작은 등쪽가지(dorsal, posterior ramus)로 나뉜다[7]. 후두신경통과 관련되는 가지는 등쪽가지이며, 감각과 운동신경을 모두 포함하는 혼합형 신경이다. 경추신경근의 등쪽가지는 제1경추신경을 제외하고, 후근신경절을 지나 척추 후관절을 돌아 올라오면서 내측, 외측가지(medial and lateral branches)로 나뉜다. 이중 제2-5경추신경근의 등쪽가지의 내측가지(medial branches of the C2-C5 dorsal rami)만 피부감각을 담당한다[7].

제2경추신경근의 등쪽가지(the dorsal rami of the C2 nerve)는 C2 배쪽가지와 다른 경추 등쪽가지보다 크다[7]. 그만큼 감각을 담당하는 범위가 넓고 중요하기 때문이다. 이 제2경추신경근 등쪽가지는 목뼈를 지나 아래빗근(inferior oblique muscle) 아랫면을 돌아 올라오며, 여기서 내측가지와 외측가지로 나뉘어진다. 제2경추신경근의 등쪽 가지의 내측가지(medial branch of the dorsal rami of the C2 neve)는 큰뒤통수신경이 되면서 내측으로 진행하면서 머리반가시근(semispinalis capitis muscle) 사이를 통과한다. 머리반가시근 사이를 통과한 큰뒤통수신경은 다시 외측 상방으로 진행하면서 승모근 아래를 진행하고, 승모근과 흉쇄유돌근(sternocleidomastoid muscle)의 건막이 후두골의 위목덜미선(superior nuchal line)에 부착되는 부위에서, 피하지방과 피부로 이행하여 두피와 목뒤 부분의 감각을 지배한다[7-10]. 여기서 큰뒤통수신경이 승모근 건막 부착부를 통과하는 부분이 섬유성 터널을 형성하며 이를 승모근터널(trapezial tunnel)이라고 한다(Figure 1) [11,12]. 이 터널이 큰뒤통수신경이 흔히 포착되는 장소로 알려져 있다[11-14].

한편, 제2경추신경 등쪽가지의 내측가지는 제1경추신경가지와 교통하고 있고, 아래쪽 교통분지는 제2/3경추후관절에 분포하면서 제3경추신경의 등쪽가지와도 연결된다. 이렇게 제1, 2, 3 경추신경의 등쪽가지는 서로 연결되어 있다. 또한 승모근터널을 통과한 큰뒤통수신경은 작은뒤통수신경과 제3뒤통수신경과 피하에서 서로 교통한다. 제1-3경추신경근의 등쪽가지는 팔로 가는 신경이 아니라 머리 뒷부분 감각을 담당하는 하나의 단위로 생각해야 한다. 이중 제2경추신경 뒤쪽가지의 내측분지에서 기원하는 큰뒤통수신경이 가장 크고 후두신경통에서 가장 중요한 역할을 한다.

큰뒤통수신경은 제2경추신경근으로 직접 연결되며, 제2경추신경근은 경막 안의 제2경추신경의 후근(C2 dorsal root)로 바로 연결되며, 제2경추신경 후근은 제2경수 척수 안에 위치한 삼차신경목복합체(trigeminocervical complex)로 직접 연결된다(Figure 2). 삼차신경목복합체는 두통과 안면 후두통증의 병태생리에서 가장 중요한 구조로 알려져 있다. 그래서 큰뒤통수신경은 얼굴의 삼차신경처럼 후두부의 통증에 있어 가장 중요한 구조가 된다.

후두신경통의 분류와 원인

후두신경통의 원인은 정확히 알려져 있지 않다. 후두신경통은 특발성 및 이차성 후두신경통으로 구분된다. 원발성 후두신경통은 제2경추신경근이나 큰뒤통수신경을 침범하는 해부학적인 구조적 병변이 없는 경우다. 이차성 후두신경통은 두개기저부(skull base) 구조적 이상 병변으로 제 1, 2, 3경추신경근(upper C1-C3 cervical roots)과 큰뒤통수신경의 경로가 자극될 때 흔히 발생한다. 또한 신경근의 염증과 제2경수의 척수염에서도 보고되어 있다. 이차성 후두신경통은 제2경수, 제2-3척추신경 후근, 큰뒤통수신경을 자극하는 구조적인 병변에 의해 발생한다. 이차성 후두신경통을 유발하는 원인에 대해서는 Table 2에 정리하였다[15-27]. 특발성 후두신경통은 그동안 원인 없이 발생하는 것으로 알려져 왔다. 그러나 최근 큰뒤통수신경의 포착증후군(entrapment neuropathy of the greater occipital nerve)이 원인으로 제시되고 있다.

Reported etiology of occipital neuralgia

한편, 삼차신경통은 고전적(classic), 원발성(idiopathic), 이차성(secondary) 삼차신경통으로 분류된다[3]. 고전적 삼차신경통은 일반적인 삼차신경통으로 삼차신경 분지에 발생하는 특징적인 찌르는 통증이다. 고전적 삼차신경통은 실제로는 삼차신경에 대한 혈관압박으로 발생하는 것을 의미하며, 기본적으로는 이차성이라 할 수 있으나, 통상적으로 이차성과 구분하기 위해 사용한다. 이차성 삼차신경통은 수막종, 혈관기형과 같은 구조적 병변에 의해 삼차신경이 자극되어 나타나는 신경통이다. 특발성 삼차신경통은 삼차신경통의 증상이 있지만 혈관압박이 확인되지 않는 경우에 정의된다. 삼차신경통은 비교적 흔한 신경통이라 세부 분류가 있지만, 후두신경통은 세부 구분이 없다[3].

후두신경통의 원인에 대한 고찰은 매우 중요하다. 구조적인 원인이 확인되지 않는 원발성 후두신경통은 원인이 없으므로 증상조절만 하면서 지내야 한다. 그러나 원발성 후두신경통이 큰뒤통수신경의 포착증이라는 원인이 있다고 생각하면 치료는 매우 달라진다. 고전적 삼차신경통은 혈관압박에 의해 발생하므로, 삼차신경을 압박하는 혈관을 감압하는 미세혈관감압술이 원칙적으로 가장 효과적인 치료가 된다. 만약 원발성 후두신경통이 큰뒤통수신경의 포착에 의해 발생한다면, 이론적으로 큰뒤통수신경 감압술이 근치요법으로 추천될 수 있기 때문이다.

후두신경통의 진단

후두신경통의 진단은 임상진단이다. Table 1에 의하면 통증은 한쪽 또는 양쪽에 발생할 수 있으며, 통증의 위치가 큰 뒤통수신경과 작은뒤통수신경의 영역이라는 점이 중요하다[1-3]. 통증은 간헐적이어야 하며, 통증의 세기는 심한 정도, 특징적으로 찌르면서 날카롭다. 두피의 예민함이 동반될 수 있으며 통증부부의 압통이나 유발점이 있을 수 있다. 통증이 국소마취제를 이용한 후두신경차단술에 일시적으로 호전되는 것이 중요하다. 진단기준은 임상적인 특징과 이학적 소견에 의하며, 후두신경차단술에 일시적인 효과를 확인하게 되므로, 후두신경통은 전형적인 임상진단이다.

후두신경통을 유발할 수 있는 구조적인 기질적인 질환들을 감별하는 것은 중요하다. 큰뒤통수신경은 제2경추신경근에서 기원하기 때문에, 척수에서 기원하는 제2경추 척추신경의 후근(c2 dorsal root)과 큰뒤통수신경의 경로에서 신경을 자극하는 기질적인 병변이 없는지 확인해야 한다. 경추 단순방사선 검사는 반드시 굴곡, 신전을 확인해서 제1/2경추의 불안정성과 두개기저부 함입(basilar invagination)과 같은 질환을 확인해야 한다. 단순방사선검사에서 특이한 점이 없으면 후두신경통 진단을 위해서는 전산화단층촬영이나 자기공명영상은 일반적으로 필요하지 않다. 전산화단층촬영은 두개기저부 병변이나 제1-2경추의 심한 골관절염, 신경공 협착과 같은 뼈와 인대의 구조적인 이상이 의심될 때 필요하며, 자기공명영상은 뼈가 아닌 신경 자체의 이상여부를 확인하기 위해 필요할 경우가 있다. 3개월 이상 지속되는 심한 후두신경통이 목과 머리의 자세에 의해 유발되거나, 심해질 때는 전산화단층촬영이나 자기공명영상이 필요하다.

후두신경통의 진단을 위해서는 경추 단순방사선검사로 충분하다. 후두신경통은 임상진단이기 때문이다. 단순방사선 검사를 제외한 영상검사는 3개월 이상 치료해도 호전되지 않는 후두신경통에서 필요할 수도 있다. 즉 전산화단층촬영과 자기공명영상은 이차성 후두신경통의 감별진단에 필요한 검사이고, 특발성 후두신경통의 진단에는 필요하지 않다.

후두신경통의 진단에는 영상진단은 보조적이며, 국소마취제를 이용한 후두신경차단술은 반드시 필요하다. 큰뒤통수신경의 신경차단술은 간단하다. 또한 치료효과도 있어서 만성 후두신경통이 아닌 급성 후두신경통의 경우 신경차단술과 약물치료를 통해 대부분 2주 내 증상을 호전시킬 수 있다. 후두신경차단술은 큰뒤통수신경의 해부학적 경로를 이해하면, 후두신경은 비교적 얕게 위치하기 때문에 간단히 시행할 수 있다. 최근에는 초음파를 이용하며 후두신경차단술을 시행하는 경우도 많아지고 있다.

영상을 이용하지 않는 후두신경차단술의 기본 출발점은 뒤통수점(inion, external occipital protuberance)이다. 큰뒤통수신경은 뒤통수점 아래 3 cm, 측방 1 cm 부근에서 머리반가시근 사이를 뚫고 나와서, 측상방으로 이행한다. 이후 위목덜미선에 부착되는 승모근과 흉쇄유돌근의 건막 사이를 통과한다. 이 건막사이에서 큰후두신경이 통과하는 부위는, 성인의 경우 뒤통수점에서 측방 3-4 cm에 위치한다. 그러므로 큰뒤통수신경의 차단은 이 경로에서 쉽게 시행할 수 있다(Figure 1). 1-2% 리도카인 2 mL 정도를 사용하면 신경차단은 쉽게 이루어진다[28-30]. 국소마취제를 이용한 큰뒤통수신경의 신경차단술 후 한, 두 시간 정도 후두두피의 감각저하가 발생해야 정확한 신경차단술이라 할 수 있다. 만약 일시적인 감각저하가 두피에 발생하지 않았다면, 국소마취제 투여가 부정확했다고 다시 시행해 보아야 한다. 후두신경차단술은 후두신경통뿐 아니라, 편두통, 긴장성두통, 경추성 두통과 같은 다양한 두통과 목통증에 효과가 있음은 잘 알려져 있다.

후두신경통의 치료

만성적으로 재발하는 후두신경통이 아닌, 1-2주 정도 지속되며 간헐적으로 찌르는 듯한 통증인 후두신경통은 보전적 약물치료와 신경차단술로 대부분 치료가 가능하다. 약물 치료는 세 가지 기전의 약물이 사용된다. 비스테로이드 소염제, 아세트아미노펜, 근이완제, 가바펜틴과 프레가발린과 같은 항경련제, 삼환계 항우울제가 사용된다[28,29]. 증상의 중등도와 부작용 유무에 따라 종류와 용량을 조절해서 사용할 수 있으며, 후두신경차단술을 병행하면서 치료한다. 후두의 통증과 함께 목의 통증이 동반되는 경우 물리치료를 병행해서 치료할 수 있다.

만약, 후두신경통의 통증이 3개월 이상의 보전적 요법으로 호전되지 않고 계속 재발하거나, 통증의 세기가 줄어들지 않으면, 추가적인 영상검사가 반드시 필요하다. 만약 추가적인 영상에서 기질적인 원인이 확인되지 않는다면 중재적 시술이 필요할 수도 있다.

1. 중재적 시술

1) 후두신경차단술

약물치료로 2주 내 증상이 빨리 조절되지 않거나, 증상이 심해서 일상생활에 불편할 경우 후두신경차단술은 간단하고 효과적인 방법이다. 또한 국소마취제를 이용한 후두신경차단에 의한 통증완화는 후두신경통 진단에도 필수적인 요소이다. 후두신경통에서 후두신경차단술의 단기적인 증상 호전에 대해서는 이견이 없다[28-30]. 국소마취제는 리도카인이나 부피바카인과 같은 종류에 따른 차단술의 효과는 차이는 없다[30]. 스테로이드는 덱사메타손이 통상적으로 많이 사용되고 있다.

후두신경차단술의 이슈는 영상유도를 사용할 것인지이다. 기존 큰뒤통수신경 차단술은 랜드마크를 이용하여 시행되어 왔다. 뒤통수점에서 측방 2 cm, 아래 2 cm가 통상적인 랜드마크이나, 뒤통수점 3 cm 아래, 1 cm 측방 지점도 추천된다[8,9]. 큰뒤통수신경에 대한 차단술에서 랜드마크를 이용한 차단술은 효과가 입증된 방법이지만, 뒤통수점과 뒷목의 근육은 사람마다 차이가 있어 부정확한 경우도 많다[28,30]. 초음파를 이용한 후두신경차단술은 뒤통수점에서 근위부에서 보다 정확하게 큰뒤통수신경을 확인하고 약물을 주입할 수 있다[31,32]. 전산화단층촬영과 자기공명영상을 이용한 영상유도 차단술도 제시되었다[33]. 그러나 고식적인 랜드마크를 이용한 차단술과 영상-유도하 차단술의 효과에 대한 차이는 아직 입증되어 있지 않다[28].

2) 보툴리눔 독소, 고주파응고술, 펄스형 고주파응고술

보툴리눔 독소(onabotulinum toxin A)는 이미 편두통, 긴장형두통, 경추성 두통에 시도해 볼만한, 효과적인 치료로 제시되었다. 한 보고에서는 후두신경통의 발작성 통증에 효과가 있으나 지속성 통증에는 효과가 없었다는 보고가 있었다[34]. 그러나 진단기준에 언급되었듯이 현재 후두신경통 진단기준에 지속성 통증이 없어 명백한 후두신경통에 대한 효과로 받아들이기 힘들다. 긍정적인 사례군 연구들이 있지만, 아직 체계적인 비교연구는 없다.

고주파응고술(radiofrequency ablation)은 신경에 열응고 병변을 만들며, 특히 통증전달 섬유인 A-delta 섬유와 C 섬유에 이론적으로는 선택적인 열병변을 만들 수 있는 방법이다[28]. 고주파응고술은 만성 두통에 시도되었으며 긍정적인 효과도 보고되어 있으나[35], 말초신경에 대한 열 병변은 그 자체로 신경손상을 유발할 수 있고, 신경손상으로 인한 이차적인 신병병성 통증의 위험도 있다[35]. 펄스형 고주파응고술(pulsed radiofrequency ablation)은 일반 고주파응고술의 위험을 줄이는 고주파 자기장을 신경에 가하는 방법으로 신경손상의 위험이 없다[36]. 펄스형 고주파응고술도 아직 적은 수의 사레들을 대상으로 한 사례군 연구들이나, 보고 대상이 후두신경통뿐 아니라 편두통도 포함된 보고여서 앞으로 장기적인 결과를 확인해 보아야 한다[37]. 냉동고주파응고술(cooled radiofrequency ablation, cryoneurolysis)도 펄스형 고주파응고술의 연장선에서 시도되고 있다.

2. 후두신경통의 수술치료

후두신경통의 수술치료는 후두신경통의 원인에 따라 결정된다. 큰뒤통수신경 경로의 구조적인 병변에 의한 이차성 후두신경통은 원인에 대한 수술치료가 필요하다. 그러므로 후두신경통에 대한 수술치료는 원발성 후두신경통에 대한 수술을 의미한다. 약물치료와 후두신경차단술을 포함하는 중재적 시술로 호전되지 않는 만성 후두신경통은 아직도 엄연히 존재한다. 원발성 후두신경통에 대한 수술치료는 신경절제술(neurectomy), 신경감압술(neurolysis), 후두신경자극술(occipital nerve stimulation)로 나뉜다. 신경절제술은 절제 부위에 따라, 경막 안에 위치하는 제2-3경추 척추신경 후근 절제술(dorsal root rhizotomy), 제2경추 후근신경절 절제술(dorsal root ganglionectomy), 큰뒤통수신경절제술(peripheral nerve neurectomy)이 있다. 신경감압술은 큰 뒤통수신경의 경로를 감압하는 큰뒤통수신경 감압술(greater occipital nerve decompression)을 의미한다. 후두신경자극술은 뒤통수 피하에, 척수신경자극술에 사용되는 선형 신경자극 전극(cylindrical lead)을 삽입하여 뒤통수의 피하와 신경분지에서 뒤통수신경을 자극하는 통증 조절 임플란트를 의미한다.

신경절제술은 신경병성 통증에 대한 약물치료가 보편화되기 이전인, 2000년대 이전에 나온 보고들이 대부분이다. 또한 신경통에 대한 신경차단술과 고주파치료 등이 보편화되기 전 보고들이다. 당시에는 다른 치료방법이 없었기 때문에 최후의 방법으로 시도되었음을 감안하고 해석하여야 한다. 신경절제술은 신경절제로 인한 신경종과 이차성 신경병성통증의 위험성으로 인해 더 이상 추천되지 않는다[38].

원발성 후두신경통에 대한 큰뒤통수신경의 후두감압술은 후두신경통의 병태생리와 관련이 있다. 원발성 후두신경통에 대한 신경절제술과 신경자극술은 신경통을 유발하는 구조적인 병변이 없다는 전제에서 다른 방법이 없으므로 시행되었다. 그러나 최근 원발성 후두신경통과 후두부 통증이 심한 편두통에서 큰뒤통수신경의 포착이 가장 흔한 원인으로 확인되었다[8,11-14]. 후두신경통이 그동안 인지하지 못했던 큰뒤통수신경 포착으로 발생한다면, 큰뒤통수신경의 신경감압술이 이론적으로 병인에 정확하게 부합하는 치료방법이다[14]. 큰뒤통수신경의 신경감압술에 대한 보고들이 최근 증가하고 있다[5,6,13,14,39-43].

후두신경통의 얼굴 삼차신경 연관통, 큰뒤통수신경 포착증후군

현행 국제두통학회 3판 분류에 의한 후두신경통의 분류에 의한 문제점은 앞에서 기술하였다. 가장 큰 문제는 간헐성, 찌르는 통증만 포함하고, 흔히 동반되는 지속성 후두부 통증은 제외되었다는 점이다[3]. 또한 국제두통학회 후두신경통 2판과 3판의 진단기준에서 눈여겨보아야 할 점은 삼차신경목복합체와의 관련성을 설명한 주석이다[2,3]. 주석은 후두신경통은 제2경수에 위치한 삼차신경목복합체에서 뒤통수신경과 삼차신경과의 연결로 인해 삼차신경 영역이 전두부와 눈 부위 통증을 유발할 수 있음을 지적하고 있다. 대표적인 일차두통인 편두통에서 후두와 목의 통증이 흔히 동반됨은 잘 알려져 있다. 편두통에 동반되는 후두, 목통증의 병태생리는 삼차신경목복합체에서 일어나는, 삼차신경과 뒤통수신경을 통한 유해성 통증 구심성 섬유의 신경자극모음(convergence)을 통해 발생한다. 편두통의 구심성자극을 전달하는 삼차신경의 통증으로 인해 뒤통수에 발생하는 연관통(referred pain)은 잘 알려져 있지만, 큰뒤통수신경의 유해자극으로 인한 얼굴 삼차영역의 연관통은 잘 알려져 있지 않다.

Kuhn 등[44]은 이미 1997년 이명, 구역, 어지러움, 시야 장애과 같은 다양한 증상들이 30-60%의 빈도로 후두신경통에 동반되며, 이런 증상으로 인해 진단의 복잡함을 기술하였다. Son [14]에 의하면, 후두신경통으로 진단받고 큰뒤통수신경 감암술로 호전되었던 환자들 중 많은 수가 발작성 통증과 함께 지속성 뒤통수, 목통증을 호소하였다. 후향적 분석 결과, 기존 후두신경통 환자들의 22%만 엄격한 진단기준에 의한, 전형적인 후두신경통으로 진단되었다. 그리하여, 뒤통수신경 영역의 지속성인 조이고 누르는 듯한 통증을 주 증상으로 신경감압술을 시행받고 호전된 환자군을 큰뒤통수신경 포착증후군(chronic occipital headache due to greater occipital nerve entrapment, COHGONE)으로 잠정 분류하였다[14]. 이는 앞에서 기술한 것처럼, 지속성 통증이 포함된 삼차신경통 분류가 후두신경통에서는 진단기준에 포함되지 않았기 때문이다[3].

큰뒤통수신경 포착에 의한 후두신경통과 COHGONE는 동측 삼차신경영역의 연관통을 흔히 유발한다[13,14]. 후두신경통 진단기준에는 삼차신경 제1분지인 안신경(ophthalmic nerve) 영역의 연관통만 강조되었지만[2,3], 후두신경통에 동반하여 제2, 3분지 영역에도 연관통이 발생함이 보고되었고[45], 심지어 얼굴 반쪽에 안면 감각저하와 신경통이 연관통으로 발생하였다[46,47]. 또한 신경 포착은 귀 속과 귀 주위에 연관통을 유발할 수도 있고, 전형적인 후두신경통 증상보다 선행해서 발생하는 경우도 있다[48]. 큰뒤통수신경 포착에 의한 연관통은 얼굴 삼차신경 영역뿐 아니라 팔다리로도 전이될 수 있다[49,50]. 이런 큰뒤통수신경 포착에 의한 삼차신경과 전신 연관통은 큰뒤통수신경의 신경감압술로 바로 호전되었다[45-49].

흥미로운 점은 전형적인 후두신경통 뿐 아니라 COHGONE에서도 안면 연관통이 흔하게 발생한다는 점이다[14]. 큰뒤통수신경 포착에 의한 후두통은 전형적인 찌르는 통증이 없는 경우, 대부분 편두통, 긴장형 두통, 경추성 두통으로 진단되었다. 만성 편두통에서 동반된 심한 안면 통증이 30년이 지나, 결국 큰뒤통수신경통의 신경감압술로 편두통과 심한 안면 통증이 호전된 경우도 보고되었다[6]. 큰뒤통수신경의 포착이 편두통으로 진단된 경우로 해석된다.

후두신경통은 단순한 후두의 찌르는 이유 없이 발생하는 통증이란 범주를 벗어나서, 큰뒤통수신경의 포착으로 인한, 만성적인 유해자극(noxious stimuli)으로 발생하는 여러 통증 중 하나의 증상으로 해석해야, 이 현상들을 설명할 수 있다. 큰뒤통수신경의 포착은 전형적인 찌르는 통증과 함께 지속성 통증을 유발한다. 또한, 큰뒤통수신경 포착에 의한 만성적인 유해자극은 상부 척수의 삼차신경목복합체에 직접 연결되며 신경자극모음과 중추 감작(central sensitization)의 과정을 통해, 얼굴 삼차신경 영역뿐 아니라 체간과 팔다리에도 다양한 연관통을 유발할 수 있다. 앞으로 큰뒤통수신경 포착의 병태생리와 다양한 증상에 대해 많은 관심이 필요하다.

결론

후두신경통은 뒤통수신경 영역에서 간헐적인 찌르는 통증이 발생하는 질환이다. 현재 진단기준에 의하면, 두피 이상감각이나 압통이 동반되며, 통증 세기는 심하고, 국소마취제를 이용한 신경차단술에 일시적으로 호전됨이 확인되어야 한다. 간헐적 통증과 별도로 발생하는 지속성 통증은 진단기준에서 제외되어, 지속성 통증과 함께 간헐적 찌름이 발생하는 기존 기준에 의한 후두신경통은 진단과 분류에 어려움이 있다. 후두신경통은 제2경추신경 후근과 큰뒤통수신경 경로에서 신경을 자극하는 원인이 확인되는 이차성 후두신경통과 구조적인 원인이 없는 원발성 후두신경통으로 구분된다. 원발성 후두신경통이 3개월 이상 지속되면 세밀한 영상검사가 필요하다. 후두신경통은 약물치료과 보전적 치료에 대부분 잘 조절된다. 후두신경차단술은 진단과 치료에 중요한 술기이다. 증상이 빨리 조절되지 않거나, 3개월 이상 지속되는 후두신경통은 중재 치료가 필요하다. 후두신경통의 수술치료는 원발성 후두신경통에 대한 수술치료를 의미한다. 신경절제술, 신경감압술, 후두신경자극술 중에서 신경감압술이 추천된다.

원발성 후두신경통에서는 최근 큰뒤통수신경 포착증후군이 병인으로 제시되고 있다. 고전적 삼차신경통이 삼차신경동맥압박에 의해 발생하듯이, 원발성 후두신경통도 큰뒤통수신경 포착에 의한 증상으로 해석된다. 큰뒤통수신경은 뒤통수와 목의 통증 전달에 가장 중요한 역할을 하며, 제2경추 척추신경의 후근으로 직접 연결되어, 얼굴과 두개강내 통증 감각핵인 삼차신경핵의 아래핵이 위치하는 삼차신경목복합체로 직접 연결된다. 삼차신경과 큰뒤통수신경의 유해성 통증자극이 신경자극모음과 과흥분을 통해, 얼굴의 삼차신경 영역으로 다양한 연관통이 발생한다. 이로 인해 측후두통증과 후두신경통 환자들에게서 눈, 뺨, 잇몸, 턱에 설명하기 어려운 통증이 발생한다.

이 논문은 임상에서 드물지 않게 경험하는 후두신경통에 대한 자세한 고찰과 신경학적 증상을 해부학적 관점에서 이해하기 쉽게 설명하고, 후두신경통의 보존적 치료와 최근 국내에서 시행되고 있는 큰뒤통수신경감압술의 효과에 관하여 자세히 소개해 주고 있다. 후두신경통은 삼차신경-목-복합체에 의한 전두부와 눈 부위 통증과 큰뒤통수신경의 유해 자극에 의한 안면부 연관통과 같은 증상들이 흔히 동반된다. 이런 증상들은 일차성 두통에서도 흔히 동반되므로 후두신경통을 처음부터 진단하기 어렵게 만든다. 따라서, 일차성 두통으로 장기간 약물치료를 하였으나 호전되지 않는 통증의 경우에는 후두신경통의 가능성을 고려하고 진단 목적의 신경차단술을 시행하는 것이 필요하다. 후두신경통은 일반적으로 약물치료를 포함한 보존적 치료를 주로하고 있으나 이 논 문에서는 중재적 시술과 수술적 치료에 대하여 자세하게 기술하면서 병태생리의 근본적인 원인 해결이 치료의 근본적인 해법임을 강조하고 있다. 이 논문은 후두신경통 및 큰뒤통수신경 포착 증후군의 병태생리를 이해하고 임상 현장에서 치료하는 데 많은 도움이 될 것으로 판단된다.

Pathogenesis of referred pain from occipital neuralgia caused by greater occipital nerve (GON) entrapment. Nociceptive afferent input of the face is transmitted through the trigeminal nerve to the nucleus of the spinal trigeminal tract in the brainstem and upper cervical cord (C2). Nociceptive afferents from the occipital and neck area are transmitted to the trigeminocervical complex (TCC) located in the C2 dorsal horn through the high cervical C1-C3 roots. Convergence of trigeminal and occipital nociceptive afferent inputs occurs in the TCC.
