주 and Ju: 난소암 치료 이행성 연구의 최신 동향

Abstract

Ovarian cancer is the most lethal gynecologic cancer. Countless efforts have been made to improve the survival of ovarian cancer patients by clinicians as well as basic researchers so far. As a result of those extensive translational researches some of promising molecular targets and its proper therapies are introduced into practice for treating intractable ovarian cancer, which includes anti-angiogenic therapy, tyrosine kinase inhibitor, and other modulators of cancer-related signaling pathways. Current trend of translational researches in ovarian cancer treatment are mainly focused on discovering companion diagnostics which can categorize the subgroup of patient having specific biomarker that may serve as a target of new treatment because the consequent outcome of new therapy varies depending on the molecular profile of each patient. It is important for clinician to consider each ovarian cancer patients to have a possible therapeutic target and to examine the tissue biopsies with available companion diagnostics. In addition more interest and endeavor should be given to the clinical trials to apply the result of up-to-date translational researches.

서론

상피성 난소암은 부인암 중 치사율이 가장 높은 암으로 발생률은 세계적으로 연간 200,000만 건이며 사망률은 연간 125,000건에 이른다[1]. 최근 몇십 년간 최초의 종양감축술 술기의 발전과 새로운 항암제제, 복강 내 투여법 및 항암제 조합의 개발로 단기적 진행은 다소 개선이 있으나 일차 치료 후의 재발, 항암제 내성은 아직 완전히 극복되지 못하고 있어 장기생존율은 답보상태에 있는 실정이다.
다른 암종과 마찬 가지로 난소암 연구에도 최신 분자생물학적 기술들이 도입되면서 게놈 차원의 분석들이 진행되어 DNA 복제수 변이, 메틸화, 돌연변이 등에 대한 정보들이 축적되고 있다. 이러한 최신 기술을 이용한 이행성 연구를 통해 난소암의 암화 과정을 밝히고 환자마다의 예후의 차이를 설명하고 종국적으로 이를 치료표적으로 발굴하고자 하는 노력들이 지속적으로 행해지고 있다. 그 결과 몇몇 표적치료제들은 실제 임상에 도입되어 사용되고 있으며 무병생존율을 개선시키고 특정 환자군에서 전체생존율의 향상까지도 보이고 있다.
최근의 난소암의 이행성 연구는 주로 표적치료에 이용되는 표적을 발굴하고 검증하는 단계로 이어지는 경향을 보이고 있는데, 검증이 완료되어 치료 가능한 환자군을 선별할 수 있는 진단은 동반진단이라고 명명할 수 있게 된다. 여기서는 상피성 난소암의 치료효과 및 생존율 개선을 위한 이행성 연구와 이를 검증하는 임상시험의 최신 동향을 살펴보고자 한다.

Low-grade Serous Ovarian Carcinoma

현재까지의 난소암 이행성 연구는 대부분 고등급 장액성 난소암(high-grade serous carcinoma, HGSC)을 대상으로 한 것들이었다. HGSC의 발생률이 높고 임상양상이 급격하여 최우선의 연구과제가 되었던 것인데, 최근에는 저등급 장액성 난소암(low-grade serous carcinoma, LGSC)에 대한 이해성 연구도 활발히 이루어 지고 있다. LGSC는 양성 낭선종에서 시작하여 경계성 종양 단계를 거쳐 난소암으로 발전하는 과정을 거치는데 진행 속도가 비교적 느린 반면 항암제에 대한 반응이 우수하지 않은 것으로 알려져 있다[2]. 또한 LGSC는 HGSC에서 p53의 돌연변이가 자주 발견되는 것에 비해 MARK 경로의 돌연변이가 더 자주 발견된다[3]. 이러한 LGSC 환자군에서 MEK 저해제에 대한 연구들이 진행되고 있다. MEK 저해제인 selumetinib에 대한 Gynecologic Oncology Group 제2상 임상시험에 의하면 52명의 재발성 LGSC 환자 중 15%가 반응성을 보였고 65%의 환자가 종양 유지 상태를 보였다[4]. 재발성 혹은 지속성 LGSC 환자를 대상으로 하는 MEK 저해제 제3상 임상시험인 MILO 시험(MEK Inhibitor in Low-grade Serous Ovarian Cancer)이 현재 진행중에 있다(NCT01849874). MEK 경로를 통한 내성이 PI3K/AKT 경로를 통한다는 점에 착안하여 이 두 가지 경로를 모두 차단하는 제제를 이용한 전임상 연구결과들이 상당한 효과를 보이고 있어 이를 이용한 시험도 장차 시행될 것으로 기대 된다[5]. 이 밖에도 nutlin-2와 같은 물질을 이용하여 p53의 분해과정을 막는 방법으로 세포사멸을 유도하는 연구들이 진행되고 있다[6].

Cancer-related Inflammation

다양한 사이토카인 및 염증세포들이 종양 증식, 전이, 세포사멸 회피, 신생혈과 합성 등의 과정에서 중요한 역할을 한다는 사실들은 상피성 난소암에서도 지속적으로 밝혀져 왔는데, 최근의 이행성 연구 결과들은 이러한 염증 반응 중 일부가 치료 표적이 될 수 있음을 보여 주고 있다. 종양괴사인자 α (tumor necrosis factor α, TNF-α)와 이로 인해 유도되는 interleukin (IL)-6가 난소암의 암화 과정에 관여한다는 점을 바탕으로 Madhusudan 등[7]은 TNF-α의 길항제인 etanercept를 이용한 제2상 임상시험을 시행하였는데, 저자들은 30명의 진행성 난소암 환자 중 여섯 명이 종양 안정화 상태를 보이고 17명 중 11명이 IL-6의 유의한 감소를 보였다고 보고하였다.
Brown 등[8]은 항-TNF 항체인 infliximab을 이용한 제 1상 임상시험을 시행하였는데, 여덟 명의 난소암 환자를 포함한 41명의 진행성 암환자가 참여하였다. 혈장 IL-6는 항체 투여 후 24시간 및 48시간에 유의하게 감소하였고 여덟 명의 난소암 환자 중 한 명은 종양 안정화 상태를 보였다. 이 결과를 토대로 Coward 등[9]은 상피성 난소암 환자를 대상으로 siltuximab을 이용한 항-IL-6 치료 제2상 임상시험을 진행하였다. 참여한 18명의 환자 중 한 명은 부분반응을 보였고 일곱 명은 종양 안정화 상태를 보였다. 심각한 이상 반응은 없었으며 siltuximab은 잘 견딜 수 있는 치료로 평가되었다.

Aflibercept

혈관내피 성장인자(vascular endothelial growth factor, VEGF) 수용체 단클론 항체인 bevacizumab이 상피성 난소암 치료에 이미 도입되어 사용되면서 VEGF등의 신호전달 경로 저해제에 대한 연구가 지속되었고 보다 효율적인 제제를 개발하려는 노력에서 VEGF-trap이라고도 불리는 aflibercept에 대한 연구와 임상시험이 진행되고 있다. Aflibercept은 VEGF 수용체 1의 세 도메인과 면역글로블린 G의 Fc 부위가 결합된 단백질로서 VEGF 리간드에 대한 친화력이 매우 뛰어나 암세주 실험에서 우수한 항암효과를 보였다. 그러나 양전하를 띤 성질 상 생체 내 반감기가 매우 짧다는 단점이 있었다. 이를 개선하기 위해 면역글로블린 G의 Fc 부위에 VEGF 수용체 1의 두 도메인과 VEGF 수용체 2의 세 도메인을 결합시키는 시도를 했고 이 결과 VEGF 리간드 친화력은 유지한 채 생체 내 반감기가 길어지게 되었다.
이후 aflibercept에 대한 2상 임상시험들이 재발성 난소암 환자를 대상으로 진행되었다. Coleman 등[10]은 46명의 재발성 난소암 환자들에게 aflibercept (6 mg/kg iv)와 docetaxel (75 mg/m2 iv)를 3주 간격으로 병용투여 하고 54%의 반응률을 보고하였는데, 이 중 10명의 환자는 완전 관해를 보였고 무병생존기간은 6.2개월(중앙값), 전체생존기간은 24.3개월(중앙값)이었다. Gotlieb 등[11]은 55명의 환자를 대상으로 이중 맹검무작위배정 임상시험을 실시하였는데 aflibercept (4 mg/kg iv)을 매 2주마다 투여한 군에서 복수천자 횟수가 유의하게 감소함을 보고하였으나 전체생존율에는 차이가 없었던 반면 위장관계 이상반응 등 3도 이상의 독성이 aflibercept군에서 더 많이 나타났다. 이 밖에도 alflibercept의 복강 내 투여가 전체생존율 개선에 도움이 되는지에 대한 임상시험도 진행 중이다.

Nintedanib

Nintedanib (BIBF 1120)은 경구투여가 가능하고 세 가지 혈관형성 경로를 표적으로 하는 물질로서 VEGF 수용체, 섬유모세포 성장인자 수용체 및 혈소판 기원 성장인자 수용체를 표적으로 한다. 83명의 재발성 난소암 환자를 대상으로 위약대조 제2상 임상시험을 진행한 결과 nintedanib (250 mg twice daily)을 투여 받은 군에서 무병생존율의 개선을 보였다. 이후 시행된 이중 맹검 제3상 임상시험은 1,366명의 난소암 환자의 수술 후 일차 항암 치료로 carboplatin/paclitaxel 조합에 nintedanib 또는 위약을 추가하는 것이었는데 위약군에 비해 nintedanib군에서 무병생존기간이 유의하게 개선된 결과를 보였다(18.3 vs. 16.6 months; hazard ratio, 0.84; 95% confidence interval, 0.72 to 0.98; P=0.0239).

Trebananib (AMG 386)

안지오텐신 1 (angiopoietin-1, Ang1)과 안지오텐신 2 (angiopoietin-2, Ang2)는 혈관내피 세포의 Tie2 수용체에 작용하여 VEGF와는 다른 경로를 통해 혈관 리모델링을 중재한다. Trebananib (AMG 386)은 Ang1과 Ang2에 Fc를 결합하여 만든 단백으로 이들이 Tie2 수용체에 작용하는 것을 차단한다[12]. Trebananib은 전임상 동물모델에서 혈관형성 억제효과를 보였으며 재발성 난소암 환자를 대상으로 한 제1상 연구에서도 단일 제제로서 투여되어 효과를 보였으며, 재발성 난소암 환자를 대상으로 한 무작위배정 제2상 연구에서도 무병생존기간을 연장시켰다. 혈관형성 억제라는 작용은 동일하나 trebananib은 기존의 VEGF 경로 저해제와는 달리 가장 유의한 독성으로 부종이 나타났다. TRINOVA-1 제3상 임상시험에서는 32개국 919명의 재발성 상피성 난소암 환자를 paclitaxel/trebananib 혹은 paclitaxel/위약군으로 무작위 배정하여 치료하였는데 그 결과 trebananib 포함군의 무병생존기간이 유의하게 길었다(7.2 vs. 5.4 months; hazard ratio, 0.66; 95% confidence interval, 0.57 to 0.77; P<0.001) [13]. 이어진 TRINOVA-2 임상시험은 223명의 재발성 상피성 난소암 환자를 무작위 배정하여 pegylated liposomal doxorubicin/trebananib군과 pegylated liposomal doxorubicin/ placebo군을 비교하고(Clinicaltrials.gov identifier NCT01281254) TRINOVA-3 임상시험은 2,000명의 상피성 난소암 환자의 일차 치료에 carboplatin/paclitaxel과 함께 trebananib 혹은 위약을 투여 하여 그 결과를 비교하고 있다(NCT01493505).

HER family

표피성장인자(epidermal growth factor receptor, EGFR or ERBB1)는 난소암의 25-50%에서 발견되며 일부 연구에서 예후 인자로 보고 되고 있다[14]. 그러나 EGFR 단클론 항체인 cetuximab이나 EGFR tyrosine kinase 저해제인 gefitinib을 이용한 제2상 연구에 의하면 EGFR 경로 차단은 난소암 환자에서는 제한적인 효과만을 보이고 있음을 알 수 있다[14,15]. 이와 유사하게 erlotinib 역시 일차 항암치료를 마친 고위험군 난소암 환자를 대상으로 한 제3상 임상시험에서 유의한 이득을 증명하지 못했다[16].
Bookman 등[17]은 HER2 차단제인 trastuzumab을 이용하여 재발성 혹은 지속성 난소암 환자 837명을 대상으로 제2상 임상시험을 진행하였는데, 7.3%의 반응률과 2개월의 무병생존기간을 보였는데 이는 동일한 방법으로 시행한 유방암 환자들의 반응에 비해 현저히 낮은 결과였다. 또한 같은 연구에서 HER2의 종양 내 과발현이 11.4%에 불과하다는 점과 예후와는 유의한 상관 관계가 없음을 보고하였다. 유사한 결과가 Gynecologic Oncology Group의 다른 이행성 연구에서도 보고 되었는데 FISH (fluorescence in situ hybridization)를 이용하여 검출한 결과 난소암에서 HER2 유전자의 증폭은 단 7%에서만 발견되었으며 예후를 예측하지 못했다[18].
한편, 최근의 이행성 연구결과는 난소암에서 HER3와 HER4의 과발현을 보고 하고 있다. 202명의 난소암 환자의 조직 array를 용한 분석에서 HER3의 활성은 79%에서, 간세포 성장인자의 활성은 56%에서 발현되는 것으로 나타났다[19]. HER3는 PI3K 신호경로를 활성화 시키는 것으로 알려져 있으며 난소암의 불량한 예후와 연관이 있음이 보고 되었다[20]. 이러한 연구를 바탕으로 지속된 연구에서 HER3의 단클론 항체인 MM-121은 세포주 실험과 이종이식 동물 모델에서 종양의 성장을 유의하게 저해하는 것으로 밝혀졌고[21], MM-121을 이용한 무작위배정 제2상 임상시험이 진행 중이다(NCT01447706).

결론

난소암 치료를 위한 이행성 연구동향은 표적발굴과 검증 및 표적치료로 이어지는 과정이라고 요약할 수 있다. 이 과정에서 치료표적을 가지고 있는 적절한 환자군을 선별하는 것이 중요하기 때문에 동반진단 연구가 점차 자리 잡고 있다. 동반진단은 치료반응을 높이고 부작용을 최소화하는 개인 맞춤형 치료를 가능케 할 것이므로 향후 이러한 연구경향은 바뀌지 않을 것으로 예상 된다.
난소암은 종양 자체의 조직학적 구성과 악성도, 분화도가 매우 이질적일 수 있으며 환자 개개인 마다 조직학적 아형이 다르고 유전자 발현 프로파일이나 DNA 복제수가 달라 동일한 치료에 다양한 반응과 상이한 예후를 보일 수 있다. 따라서 이행성 연구결과를 치료에 적정하게 적용하기 위해서는 최초 진단 및 수술 과정, 이후 재발 및 추적 과정에서 환자의 검체를 이용한 정확하고 포괄적인 동반진단이 이루어질 수 있도록 해야 하는데 이 부분은 임상의의 관심과 노력이 없이는 성취되기 어렵다.
또한 최근의 이행성 연구들은 표적발굴 후 검증, 임상시험까지의 일정이 매우 빨라지고 있으므로 연구자, 임상의, 인허가 기관, 환자/보호자 단체 등 관련된 모든 이해 당사자들이 이행성 연구의 특징과 전개에 대해 잘 이해하고, 연구결과를 안전하게 적용하려는 노력이 한층 더 필요하다.

본 논문은 난소암 분야에서 최근 시행되고 있는 이행성 연구를 종합적으로 분석하여 현재의 연구 흐름을 제시한 자료이다. 현재 진행중인 다양한 표적치료제들의 연구현황에 대한 설명으로 바탕으로 이러한 새로운 표적치료제가 효과를 보일 수 있는 특정 환자군을 선별하는 진단적 tool을 개발하는 과정이 현재의 trend 임을 체계적으로 기술하고 있다. 이는 난소암을 연구하는 임상의사 및 기초 과학자들에 향후의 연구 진행방향에 대한 방향제시를 해 주고 있다는 점에서 의의가 있는 논문이라 여겨진다.
정책과제를 제안하여 발전을 위한 방향제시를 했다는 점에서 의의가 있는 논문이라 판단된다.
